Hos personer med och utan diabetes är förhöjt blodtryck (BP) den främsta orsaken till stroke och en viktig riskfaktor för andra hjärt-kärlsjukdomar, inklusive kranskärlssjukdom och hjärtsvikt. Strategier som effektivt sänker blodtrycket inkluderar läkemedelsbehandling och livsstilsförändringar. Livsstilsförändringar, särskilt kostmetoder, har visat sig sänka blodtrycket hos personer utan diabetes. Det finns dock en slående brist på bevis på BP-sänkande, livsstilsförändringar, förutom viktminskning, hos personer med diabetes. DASH4D-försöket är utformat för att tillhandahålla dessa bevis.

DASH4D-prövningen bygger på utredarnas erfarenhet av fyra NIH-sponsrade matningsstudier (DASH, DASH-Sodium, OmniHeart och OmniCarb). Försöket kommer att inkludera cirka 100 vuxna med typ 2-diabetes, systoliskt BP 120-159 mmHg och diastoliskt BP

Kärndesignen är en fyra-periods, single site, crossover-matningsstudie med 5-veckorsperioder. Deltagarna matas med var och en av fyra isokaloriska dieter, presenterade i slumpmässig ordning:

  1. DASH4D-diet med lägre natrium
  2. DASH4D-diet med högre natrium

Den primära kontrasten av intresse är DASH4D-dieten med lägre natrium jämfört med jämförelsedieten med högre natrium.

DASH4D-dieten liknar den ursprungliga DASH-dieten, men är lägre i kolhydrater och högre i omättat fett än den ursprungliga DASH-dieten, och är därför mer förenlig med kostrekommendationer för personer med diabetes än den ursprungliga DASH-dieten. Jämförelsedieten speglar vad många personer med diabetes för närvarande konsumerar. Det lägre natriumintaget på cirka 1500 mg/dag (vid 2000 kcal) har visat sig sänka blodtrycket hos personer utan diabetes och har rekommenderats i vissa kostråd. Det högre natriumintaget på cirka 3700 mg/dag (vid 2000 kcal) är baserat på uppskattat genomsnittligt intag i USA.

Resultaten mäts i slutet av varje utfodringsperiod. Det primära resultatet är kontorsbaserat systoliskt blodtryck i slutet av perioden. Andra utfall är diastoliskt blodtryck, mått på glykemi, plasmalipidriskfaktorer, patientsymptom och uppskattad risk för hjärt-kärlsjukdom.

I likhet med utredarnas tidigare utfodringsstudier förväntar sig utredarna att resultaten av DASH4D-studien kommer att omedelbart tillämpas på folkhälsan och kliniska riktlinjer och kommer att påverka näringspolitiken. INKLUSIONSKRITERIER:

  • 18 år eller äldre
  • Diabetes mellitus typ 2 definierad av HbA1c ≥6,5 % eller behandling av diabetes med diabetesmedicin(er)
  • Baslinje systoliskt BP på 120-159 mmHg (baserat i genomsnitt över 3 screeningbesök)
  • Baslinje diastoliskt BP
  • Vill och kan äta på plats för en måltid per dag, 3 dagar i veckan, och endast äta och all mat som tillhandahålls som en del av studiedieten under de kontrollerade matningsperioderna (inkörningsperioder och fyra 5-veckors utfodringsperioder). EXKLUSIONS KRITERIER:

    • Serumkalium ≥5,2 mmol/L eller
