Em pessoas com e sem diabetes, a pressão arterial (PA) elevada é a principal causa de acidente vascular cerebral e um importante fator de risco para outras doenças cardiovasculares, incluindo doença coronariana e insuficiência cardíaca. As estratégias que efetivamente reduzem a PA incluem terapia medicamentosa e modificação do estilo de vida. Modificações no estilo de vida, particularmente abordagens dietéticas, demonstraram reduzir a PA em pessoas sem diabetes. No entanto, há uma escassez impressionante de evidências sobre a redução da pressão arterial e modificações no estilo de vida, além da perda de peso, em pessoas com diabetes. O estudo DASH4D foi desenvolvido para fornecer essa evidência.
O estudo DASH4D baseia-se na experiência dos investigadores em quatro estudos de alimentação patrocinados pelo NIH (DASH, DASH-Sodium, OmniHeart e OmniCarb). O estudo envolverá aproximadamente 100 adultos com diabetes tipo 2, PA sistólica 120-159 mmHg e PA diastólica
O projeto principal é um estudo de alimentação cruzada de quatro períodos, em um único local, com períodos de 5 semanas. Os participantes são alimentados com cada uma das quatro dietas isocalóricas, apresentadas em ordem aleatória:
- Dieta DASH4D com baixo teor de sódio
- Dieta DASH4D com alto teor de sódio
- dieta de comparação com baixo teor de sódio
- dieta de comparação com alto teor de sódio.
O principal contraste de interesse é a dieta DASH4D com baixo teor de sódio versus a dieta de comparação com alto teor de sódio.
A dieta DASH4D é semelhante à dieta DASH original, mas é mais baixa em carboidratos e mais rica em gordura insaturada do que a dieta DASH original e, portanto, é mais consistente com as recomendações dietéticas para pessoas com diabetes do que a dieta DASH original. A dieta de comparação reflete o que muitas pessoas com diabetes consomem atualmente. A menor ingestão de sódio de aproximadamente 1.500 mg/dia (a 2.000 kcal) demonstrou reduzir a PA em pessoas sem diabetes e foi recomendada em algumas diretrizes dietéticas. A maior ingestão de sódio de aproximadamente 3.700 mg/dia (a 2.000 kcal) é baseada na ingestão média estimada nos EUA.
Os resultados são medidos no final de cada período de alimentação. O desfecho primário é a PA sistólica no final do período menstrual. Outros resultados são PA diastólica, medidas de glicemia, fatores de risco de lipídios plasmáticos, sintomas do paciente e risco estimado de doença cardiovascular.
Semelhante aos estudos de alimentação anteriores dos investigadores, os investigadores esperam que os resultados do estudo DASH4D sejam imediatamente aplicáveis à saúde pública e às diretrizes clínicas e influenciem as políticas de nutrição. Além disso, o estudo fornecerá uma plataforma rigorosa para avaliar o impacto da dieta e da ingestão de sódio em diversos outros resultados em pessoas com diabetes tipo 2.

Descrição
CRITÉRIO DE INCLUSÃO:
- 18 anos ou mais
- Diabetes Mellitus Tipo 2 definido por HbA1c ≥6,5% ou tratamento do diabetes com medicamento(s) para diabetes
- PA sistólica basal de 120-159 mmHg (com base na média de 3 visitas de triagem)
- PA diastólica basal
- Disposto e capaz de comer no local para uma refeição por dia, 3 dias por semana, e comer apenas e todos os alimentos fornecidos como parte das dietas do estudo durante os períodos de alimentação controlada (run-in e quatro períodos de alimentação de 5 semanas). Observe que a frequência real de refeições no local pode ser inferior a 3 dias por semana devido a restrições relacionadas ao COVID, mas os participantes ainda precisarão estar no local para pegar comida e serem pesados 3 dias por semana, e ainda serão esperados ter refeições monitoradas (presencial ou remotamente) para uma refeição por dia, 3 dias por semana.

- Ter acesso a um dispositivo móvel ou computador com recursos de videoconferência ou estar disposto a usar um dispositivo para videoconferência fornecido pelo estudo
CRITÉRIO DE EXCLUSÃO:
Exclusões de Laboratório
- Potássio sérico ≥5,2 mmol/L ou
- Taxa de filtração glomerular estimada (eGFR)
- HbA1c>9,0%
Exclusões de medicamentos
Dose instável (ou seja, alteração nos 2 meses anteriores à triagem ou antes da randomização) de qualquer um dos seguintes:
- Medicamentos anti-hipertensivos
- Inibidores do cotransportador de sódio-glicose 2 (SGLT2) ou agonistas do receptor do peptídeo-1 semelhante ao glucagon (GLP-1)
- Estimulantes, incluindo medicamentos orais para asma ou doença pulmonar obstrutiva crônica (DPOC)
- Terapia de reposição hormonal ou hormônio tireoidiano
- Agentes psicotrópicos para aumento de peso, incluindo agentes antipsicóticos, lítio e mirtazapina
Uso de qualquer um dos seguintes medicamentos:
- Suplemento de potássio, exceto se fizer parte de um multivitamínico
- Insulina prandial ou de ação curta
- Agonista do receptor GLP-1 se estiver em dose para perda de peso
- Varfarina (Coumadin)
- Corticosteróide oral crônico (uso intermitente é aceitável)
- Medicamentos para emagrecer
- Falta de vontade de manter a mesma dose de suplementos vitamínicos, minerais e botânicos
- Qualquer medicamento não compatível com a participação conforme determinado pelos investigadores
Exclusões de histórico médico
- diabetes tipo 1
- Hipoglicemia requerendo hospitalização ou assistência de outra pessoa nos últimos 12 meses
- Doença cardiovascular ativa ou qualquer evento nos últimos 6 meses, incluindo cirurgia de revascularização do miocárdio (CABG), angioplastia coronária transluminal percutânea (PTCA), infarto do miocárdio (IM), acidente vascular cerebral (AVC) ou exacerbação de insuficiência cardíaca congestiva (ICC) que exija admissão hospitalar
- Diagnóstico ou tratamento de câncer nos últimos 2 anos (câncer de pele não melanoma ou câncer de mama ou próstata localizado que não requeira terapia sistêmica é aceitável)
- Doença inflamatória intestinal ativa, ressecção intestinal, síndrome de má absorção, pancreatite (episódio no último ano) ou história de cirurgia bariátrica
- Gravidez ou lactação ou gravidez planejada
- Qualquer visita ao departamento de emergência (DE) por asma ou doença pulmonar obstrutiva crônica (DPOC) nos últimos 6 meses
- Qualquer outra doença ou condição grave não compatível com a participação, conforme determinado pelos investigadores

After the participant has rested quietly in the seated position for at least 5 minutes, blood pressure will be measured in triplicate, using an Omron device, with each reading separated by 30 seconds, and the average of the three readings (in mmHg) will also be recorded.
Blood pressure will be measured on five separate occasions during the last 2 weeks of the feeding period, and the mean systolic blood pressure from these five blood pressures (15 total readings) will be used as the primary outcome.

Participants will complete a questionnaire about symptoms experienced while on each diet, including symptoms related to fall risk (e.g., feeling faint), food intake and output (e.g., bloating, constipation), fluid intake and output (e.g., excessive thirst), and general symptoms (e.g., fatigue or low energy).
Participants will be asked to rate each listed symptom as: did not occur, mild, moderate, or severe.
