Description détaillée
Le contrôle local des tumeurs malignes des glandes salivaires est dose-dépendant. Des taux de contrôle locaux élevés dans les carcinomes adénoïdes kystiques pourraient être atteints par des techniques de radiothérapie hautement conformationnelles et une thérapie par particules (neutrons/ions carbone). Étant donné que des doses élevées sont nécessaires pour obtenir un contrôle local, toutes les tumeurs malignes des glandes salivaires bénéficient probablement de l'utilisation de la thérapie par particules, qui, dans le cas d'un traitement par ions carbone, s'est avérée ne s'accompagner que d'effets secondaires légers.
Méthodes/ conception :
L'essai COSMIC est un essai prospectif, monocentrique, de phase II évaluant la toxicité dans le traitement combiné avec radiothérapie avec modulation d'intensité (IMRT) et boost d'ions carbone (C12) chez 54 patients avec histologiquement prouvé (≥R1-réséqué, inopérable ou Pn+) tumeurs malignes des glandes salivaires. Les patients reçoivent 24 ions carbone GyE (8 fractions) et IMRT (2,0 Gy/fraction).
Objectifs de l'étude :
L'incidence de la mucite ≥ CTC°3 sera évaluée comme critère d'évaluation principal de l'essai, le contrôle local, la survie sans maladie, la toxicité (incl. mucosite CTC °I-II et toxicité tardive à 3 ans post RT)

La description
Critère d'intégration:
- Tumeur maligne histologiquement confirmée ou enlevée chirurgicalement des glandes salivaires (tête et cou)
- Tumeur inopérable
- G2/3
- Tumeur résiduelle macroscopique ou microscopique (R2/ R1) ou
- ≥T3/T4 ou
- envahissement périneural (Pn+)
- consentement éclairé écrit
- pts âgés de 18 à 80 ans
- contraception efficace pour les pts en âge de procréer (<12 mois après le début de la ménopause)
Critère d'exclusion:
- Radio- ou chimiothérapie antérieure pour les tumeurs de la tête et du cou
- Autre malignité antérieure au cours des 5 dernières années, à l'exception d'un carcinome basocellulaire de la peau ou d'un carcinome pré-invasif du col de l'utérus, traité de manière adéquate
- Affection neurologique ou psychiatrique importante, y compris démence ou convulsions ou autre affection médicale grave interdisant la participation du patient à l'essai par jugement des enquêteurs
- Incapacité juridique ou capacité juridique limitée
- Sérum/urine positif bêta-HCG/grossesse
- Abus de drogue

EXPÉRIMENTAL: IMRT + boost C12
(8 x 3 GyE) thérapie par ions carbone suivie d'une IMRT de 50 Gy (2 Gy/Fx) correspondant à une dose totale d'environ 74 GyE.

8 fractions de thérapie aux ions carbone (8 x 3 GyE C12) suivies de 25 fractions d'IMRT correspondant à une dose totale d'environ 74 GyE.
La durée du traitement est d'environ 61/2-7 semaines
