Lokale controle bij maligne speekselkliertumoren is dosisafhankelijk. Hoge lokale controlepercentages bij adenoïde cystische carcinomen zouden kunnen worden bereikt door zeer conforme radiotherapietechnieken en deeltjestherapie (neutron/koolstofionen). Aangezien hoge doses nodig zijn om lokale controle te bereiken, hebben alle kwaadaardige speekselkliertumoren waarschijnlijk baat bij het gebruik van partikeltherapie, waarvan is aangetoond dat deze in het geval van behandeling met koolstofionen slechts gepaard gaat met milde bijwerkingen.
Methoden / ontwerp:
De COSMIC-studie is een prospectieve, monocentrische, fase II-studie die de toxiciteit evalueert bij de gecombineerde behandeling met intensiteitsgemoduleerde bestralingstherapie (IMRT) en boost met koolstofionen (C12) bij 54 patiënten met histologisch bewezen (≥R1-geresecteerde, inoperabele of Pn+) maligniteiten van de speekselklieren. Patiënten krijgen 24 GyE-koolstofionen (8 fracties) en IMRT (2,0 Gy/fractie).
Studiedoelen:
Incidentie van mucositis ≥ CTC°3 wordt beoordeeld als het primaire eindpunt van de studie, lokale controle, ziektevrije overleving, toxiciteit (incl. mucositis CTC °I-II en late toxiciteit 3 jaar na RT)

Beschrijving
Inclusiecriteria:
- Histologisch bevestigde of operatief verwijderde kwaadaardige tumor van de speekselklieren (hoofd en hals)
- Inoperabele tumor
- G2/3
- Macroscopische of microscopische resttumor (R2/R1) of
- ≥T3/T4 of
- perineurale invasie (Pn+)
- schriftelijke geïnformeerde toestemming
- punten van 18 - 80 jaar
- effectieve anticonceptie voor patiënten in de vruchtbare leeftijd (<12 maanden na het begin van de menopauze)
Uitsluitingscriteria:
- Voorafgaande radio- of chemotherapie voor tumoren van het hoofd en de nek
- Andere eerdere maligniteiten in de afgelopen 5 jaar behalve eerder, adequaat behandeld basaalcelcarcinoom van de huid of pre-invasief carcinoom van de baarmoederhals
- Significante neurologische of psychiatrische aandoening, waaronder dementie of epileptische aanvallen, of een andere ernstige medische aandoening die de deelname van de patiënt aan het onderzoek naar het oordeel van de onderzoekers verbiedt
- Wettelijke onbekwaamheid of beperkte handelingsbekwaamheid
- Positief serum/urine bèta-HCG/zwangerschap
- Drugsmisbruik

EXPERIMENTEEL: IMRT + C12-boost
(8 x 3 GyE) koolstofiontherapie gevolgd door 50 Gy IMRT (2 Gy/Fx) wat overeenkomt met een totale dosis van ongeveer 74 GyE.

8 fracties koolstofion (8 x 3 GyE C12) therapie gevolgd door 25 fracties IMRT overeenkomend met een totale dosis van ongeveer 74 GyE.
De duur van de behandeling is ongeveer 61/2-7 weken
