Critère d'intégration:
Les patients peuvent être inclus dans l'étude s'ils répondent à tous les critères suivants :
- Homme ou femme non enceinte d'au moins 18 ans
Patients subissant une intervention coronarienne percutanée (ICP) :
- Patients souffrant d'angine de poitrine stable (SA) ayant subi une coronarographie diagnostique dans les 90 jours précédant la randomisation démontrant une athérosclérose
- Patients atteints d'un syndrome coronarien aigu sans élévation du segment ST (NSTE-ACS) ayant subi une coronarographie diagnostique dans les 72 heures précédant la randomisation démontrant une athérosclérose
- Patients atteints d'infarctus du myocarde avec élévation du segment ST (STEMI) (angiographie diagnostique non requise)
- Fournir un consentement éclairé écrit
Critère d'exclusion:
Les patients seront exclus de l'étude si l'un des critères d'exclusion suivants s'applique avant la randomisation :
- Réception de tout inhibiteur de P2Y12 à tout moment dans les 7 jours précédant la randomisation
- Utilisation d'eptifibatide et de tirofiban dans les 12 heures précédant la randomisation (la dose la plus récente doit avoir été administrée ≥ 12 heures avant la randomisation)
- Utilisation d'abciximab dans les 7 jours précédant la randomisation
- Réception d'un traitement fibrinolytique dans les 12 heures précédant la randomisation
- Risque hémorragique accru : AVC ischémique au cours de la dernière année ou tout AVC hémorragique antérieur ; tumeur, malformation artério-veineuse cérébrale ou anévrisme intracrânien ; traumatisme récent (<1 mois) ou chirurgie majeure (y compris pontage coronarien); recevant actuellement de la warfarine ; saignement actif

Expérimental: cangrélor
Le cangrelor a été administré sous forme de bolus de 30 µg/kg suivi d'une perfusion IV de cangrelor de 4,0 µg/kg/min pendant au moins 2 heures ou jusqu'à la fin de la procédure d'index, selon la durée la plus longue. A la discrétion du médecin traitant, la perfusion pourra être poursuivie pendant une durée totale de 4 heures. Suite à l'arrêt de la perfusion de cangrelor, 600 mg de clopidogrel ont été administrés. |

Comparateur actif: clopidogrel
Clopidogrel 300 mg ou 600 mg administré avant ou après ICP. La sélection de la dose et le moment de l'administration étaient à la discrétion de l'investigateur. Au cours de l'ICP, une perfusion de placebo a été administrée pour maintenir l'insu de l'essai. De plus, des gélules placebo ont été administrées à la fin de la perfusion pour imiter la dose post-perfusion de 600 mg fournie dans le bras cangrelor. |
