Essai de non-infériorité de la réticulation ionophorétique du collagène cornéen (CXL) par rapport à la réticulation standard du collagène cornéen dans le kératocône progressif. (IONTO-CXL)
L'objectif principal du protocole est de comparer le CXL standard actuel avec le CXL ionophorétique pour fournir des preuves en ce qui concerne l'efficacité et la sécurité dans le kératocône progressif.
La réticulation du collagène cornéen (CXL) est la première intervention chirurgicale qui semble stopper la progression du kératocône. Dans le CXL standard actuel, l'épithélium cornéen central doit être débridé pour imprégner efficacement le stroma cornéen de riboflavine. Le débridement de l'épithélium peut provoquer une douleur intense, une infection et un voile stromal. L'hypothèse de l'étude est que l'iontophorèse CXL est aussi efficace que la CXL standard pour stabiliser un kératocône progressif.

Description détaillée
Le kératocône est une maladie extatique cornéenne progressive bilatérale courante provoquant une déficience visuelle en induisant un astigmatisme irrégulier et des opacités cornéennes. Ce trouble débute typiquement à l'adolescence, progresse jusqu'à l'âge de 30 à 40 ans et, dans les formes sévères, peut nécessiter une greffe de cornée. CXL a changé l'évolution naturelle du kératocône. Il crée des liens entre les fibrilles de collagène afin de rigidifier le stroma cornéen et de ralentir la progression du kératocône. Le stroma cornéen est imbibé d'une solution de riboflavine avant d'être exposé au rayonnement ultraviolet-A.
Dans la norme actuelle CXL, l'épithélium cornéen central doit être débridé pour permettre la pénétration de la riboflavine dans la cornée avec un risque d'effets secondaires, tels que douleur pendant les deux premiers jours post-opératoires, perte temporaire d'acuité visuelle pendant les trois premiers mois et des complications graves telles qu'une infection et une opacité du stroma dues à des cicatrices cornéennes.
L'iontophorèse est une technique non invasive dans laquelle un faible courant électrique est utilisé pour améliorer la pénétration de la riboflavine dans la cornée. La technique d'iontophorèse pourrait permettre la diffusion intrastromale de la riboflavine, tout en maintenant l'épithélium cornéen, combinant l'efficacité de la procédure standard sans les effets secondaires du débridement épithélial.

Critère d'intégration:
- Âge ≥ 18 ans
- Épaisseur cornéenne ≥ 400 µm
- Kératocône progressif de stade 1 à 3 (classification de Krumeich)
Critère d'exclusion:
- Épaisseur cornéenne < 400µm
- Kératocône de stade 4 (classification de Krumeich)
- Maladie cornéenne concomitante
- Antécédents de chirurgie cornéenne

Expérimental: CXL ionophorétique
Le CXL ionophorétique implique une source de courant constant et deux électrodes.
L'électrode principale est une coupelle circulaire, entourée d'un anneau d'aspiration annulaire pour fixer le dispositif sur la cornée pendant la procédure.
L'électrode elle-même est une grille en acier inoxydable, placée dans la coupelle à une distance minimale de la cornée.
Le réservoir est rempli de solution de riboflavine.
Le générateur applique un courant constant de 1mA pendant une durée prédéfinie de 5 min.
Après l'administration de riboflavine par iontophorèse, la cornée est irradiée par une lumière UVA de 3mW/cm2 pendant 30 minutes.

Comparateur actif: Norme CXL
Dans le CXL standard, l'épithélium est enlevé mécaniquement.
Ensuite, une solution de riboflavine est instillée chaque minute pendant 30 minutes.
Les cornées sont irradiées par une lumière UVA à 3 mW/cm2 pendant 30 minutes.
