Ensayo de no inferioridad del entrecruzamiento del colágeno corneal iontoforético (CXL) en comparación con el entrecruzamiento del colágeno corneal estándar en el queratocono progresivo. (IONTO-CXL)
El propósito principal del protocolo es comparar el CXL estándar actual con el CXL iontoforético para proporcionar evidencia en relación con la eficacia y seguridad en el queratocono progresivo.
El entrecruzamiento de colágeno corneal (CXL) es el primer procedimiento quirúrgico que parece detener la progresión del queratocono. En el CXL estándar actual, el epitelio corneal central debe desbridarse para penetrar eficientemente el estroma corneal con riboflavina. El desbridamiento del epitelio puede causar dolor intenso, infección y neblina estromal. La hipótesis del estudio es que la iontoforesis CXL es tan efectiva como la CXL estándar para estabilizar un queratocono progresivo.

El queratocono es una enfermedad extática corneal progresiva bilateral común que causa deterioro visual al inducir astigmatismo irregular y opacidades corneales. Este trastorno generalmente comienza durante la adolescencia, progresa hasta la edad de 30 a 40 años y, en formas graves, puede necesitar un trasplante de córnea. CXL ha cambiado la evolución natural del queratocono. Crea enlaces entre las fibrillas de colágeno para endurecer el estroma corneal y ralentizar la progresión del queratocono. El estroma corneal se empapa con una solución de riboflavina antes de exponerlo a la radiación ultravioleta-A.
En el CXL estándar actual, se debe desbridar el epitelio corneal central para permitir la penetración de la riboflavina en la córnea con riesgo de efectos secundarios, como dolor durante los primeros dos días postoperatorios, pérdida temporal de agudeza visual durante los primeros tres meses y complicaciones graves como infección y opacidad del estroma debido a la cicatrización de la córnea.
La iontoforesis es una técnica no invasiva en la que se utiliza una corriente eléctrica débil para mejorar la penetración de la riboflavina en la córnea. La técnica de iontoforesis podría permitir la difusión de riboflavina intraestromal, manteniendo el epitelio corneal, combinando la eficiencia del procedimiento estándar sin los efectos secundarios del desbridamiento epitelial.

- Edad ≥ 18 años
- Espesor corneal ≥ 400 µm
- Queratocono progresivo de etapa 1 a 3 (clasificación de Krumeich)
Criterio de exclusión:
- Espesor corneal < 400 µm
- Queratocono en etapa 4 (clasificación de Krumeich)
- Enfermedad corneal concomitante
- Historia de la cirugía de córnea

Experimental: CXL iontoforético
El CXL iontoforético implica una fuente de corriente constante y dos electrodos.
El electrodo principal es una copa circular, con un anillo de succión anular circundante para colocar el dispositivo en la córnea durante el procedimiento.
El electrodo en sí es una rejilla de acero inoxidable, colocada en la copa a una distancia mínima de la córnea.
El depósito está lleno de solución de riboflavina.
El generador aplica una corriente constante de 1mA por un período predeterminado de 5 min.
Tras la administración de riboflavina por iontoforesis, se irradia la córnea con luz UVA de 3mW/cm2 durante 30 minutos.

Comparador activo: CXL estándar
En el CXL estándar, el epitelio se elimina mecánicamente.
Luego, se instila una solución de riboflavina cada minuto durante 30 minutos.
Las córneas se irradian con una luz UVA de 3mW/cm2 durante 30 minutos.
