Non-inferiority-forsøg med iontoforetisk hornhindekollagen-tværbinding (CXL) sammenlignet med standard hornhindekollagen-tværbinding i progressiv keratokonus. (IONTO-CXL)
Det primære formål med protokollen er at sammenligne den nuværende standard CXL med iontoforetisk CXL for at give beviser i forhold til effektivitet og sikkerhed ved progressiv keratoconus.
Hornhindekollagen tværbinding (CXL) er den første kirurgiske procedure, der ser ud til at standse progressionen af keratoconus. I den nuværende standard CXL skal det centrale hornhindeepitel debrideres for effektivt at gennemtrænge hornhindens stroma med riboflavin. Epiteldebridementet kan forårsage alvorlig smerte, infektion og stromal uklarhed. Studiehypotesen er, at iontoforese CXL er lige så effektiv som standard CXL til at stabilisere en progressiv keratoconus.

Keratoconus er en almindelig bilateral progressiv ekstatisk hornhindesygdom, der forårsager synsnedsættelse ved at inducere uregelmæssig astigmatisme og uklarhed i hornhinden. Denne lidelse begynder typisk i teenageårene, udvikler sig indtil alderen 30 til 40 år og kan i alvorlige former have behov for en hornhindetransplantation. CXL har ændret den naturlige udvikling af keratoconus. Det skaber forbindelser mellem kollagenfibriller for at stivne hornhindens stroma og bremse udviklingen af keratoconus. Hornhindens stroma gennemblødes med en riboflavinopløsning, før den udsættes for ultraviolet-A-stråling.
I den nuværende standard CXL skal det centrale hornhindeepitel debrideres for at tillade penetration af riboflavin ind i hornhinden med risiko for bivirkninger, såsom smerter i de første to postoperative dage, midlertidigt tab af synsstyrke i løbet af de første tre måneder, og alvorlige komplikationer såsom infektion og stromal opacitet på grund af ardannelse i hornhinden.
Iontoforese er en ikke-invasiv teknik, hvor en svag elektrisk strøm bruges til at øge penetrationen af riboflavin i hornhinden. Iontoforeseteknikken kunne tillade intrastromal riboflavindiffusion, samtidig med at hornhindeepitelet holdes tændt, og kombinere effektiviteten af standardproceduren uden bivirkningerne af epitelial debridement.

- Alder ≥ 18 år
- Hornhindetykkelse ≥ 400 µm
- Progressiv trin 1 til 3 keratoconus (Krumeich klassifikation)
Ekskluderingskriterier:
- Hornhindetykkelse < 400µm
- Trin 4 keratoconus (Krumeich klassificering)
- Samtidig hornhindesygdom
- Historie om hornhindekirurgi

Eksperimentel: Iontoforetisk CXL
Den iontoforetiske CXL involverer en konstant strømkilde og to elektroder.
Hovedelektroden er en cirkulær kop med en omgivende ringformet sugering til at fastgøre enheden på hornhinden under proceduren.
Selve elektroden er et rustfrit stålgitter, placeret i koppen i minimal afstand fra hornhinden.
Reservoiret er fyldt med riboflavinopløsning.
Generatoren anvender en konstant strøm på 1mA i en forudindstillet periode på 5 min.
Efter riboflavinadministrationen ved iontoforese bestråles hornhinden med et UVA-lys i 3mW/cm2 i løbet af 30 minutter.

I standard CXL fjernes epitelet mekanisk.
Derefter inddryppes en opløsning af riboflavin hvert minut i 30 minutter.
Hornhinder bestråles med et UVA-lys i 3mW/cm2 i løbet af 30 minutter.
