Dépistage TPMD chez les non-fumeurs à Taïwan
Étude de dépistage par tomodensitométrie à faible dose chez des non-fumeurs présentant des facteurs de risque de cancer du poumon à Taïwan

Description détaillée
Le cancer du poumon se classe au premier rang des cancers mortels à Taïwan, avec un taux de survie à cinq ans (SR à 5 ans) de seulement 15 %. La plupart des patients atteints d'un cancer du poumon précoce sont asymptomatiques, tandis que 75 % des patients présentant des symptômes liés au cancer au moment du diagnostic présentent des métastases locales ou à distance. Comparé à un SR à 5 ans de 70 à 90 % dans le cancer du poumon de stade I/II, le SR à 5 ans au stade avancé IIIb/IV est inférieur à 15 %. Malgré les progrès du traitement pharmaceutique de la maladie à un stade avancé, la détection précoce reste le seul moyen d'améliorer les résultats et même de guérir cette maladie potentiellement mortelle. La tomodensitométrie à faible dose (LDCT) est reconnue comme le meilleur outil disponible pour le diagnostic précoce du cancer du poumon. Cependant, l'exposition aux rayonnements, le coût élevé et le taux élevé de faux positifs sont les principales préoccupations pour adapter cet outil de détection à la population générale. Des études récentes menées en Europe et aux États-Unis ont montré que le dépistage par LDCT dans la population de fumeurs de cigarettes à haut risque peut réduire efficacement la mortalité par cancer du poumon. Cependant, le principal type de cancer du poumon à Taïwan est l'adénocarcinome, qui survient généralement chez les non-fumeurs. Par conséquent, le modèle de prédiction des risques généré à partir d'études sur les pays occidentaux peut ne pas être adapté directement à notre pays. Le dépistage LDCT pour la détection du cancer du poumon chez les non-fumeurs reste un sujet controversé dans le monde entier, y compris à Taïwan. Face au besoin non satisfait, il est urgent d'établir le modèle d'évaluation des risques pour prédire les risques de cancer du poumon chez les non-fumeurs et d'évaluer l'efficacité du dépistage LDCT dans la population à haut risque non-fumeur. Dans cette étude, nous prévoyons de recruter 12 000 sujets non fumeurs présentant un risque donné de cancer du poumon ; la moitié d'entre eux devraient avoir des antécédents familiaux de cancer du poumon. Chaque participant recevra le TPMD au départ, annuellement pendant deux ans et tous les deux ans pendant 6 ans. Notre objectif global est d'établir un modèle d'évaluation des risques de cancer du poumon chez les non-fumeurs et de fournir des suggestions au gouvernement pour les considérations relatives à la politique de prévention et de dépistage du cancer du poumon à Taïwan.
Notre projet d'étude vise trois objectifs spécifiques :
Objectif 1 : Évaluer le modèle d'évaluation des risques pour prédire le cancer du poumon chez les non-fumeurs Selon les résultats du dépistage LDCT, nous prévoyons d'améliorer le modèle de prédiction des risques pour les femmes non fumeuses établi par l'étude GELAC. Le modèle de risque révisé aidera à reconnaître la population à risque plus élevé.
Objectif 2 : Établir le protocole de l'étude pilote de dépistage du TPMD à Taïwan Établir un protocole de travail et des procédures de dépistage du TPMD pour assurer un comportement homogène entre les sites d'étude
Objectif 3 : Établir l'efficacité du dépistage par LDCT du cancer du poumon chez les non-fumeurs Le taux de détection du cancer du poumon, le taux de faux positifs et l'analyse coût-efficacité du dépistage par LDCT du cancer du poumon chez les non-fumeurs à Taïwan seront établis.

Critère d'intégration:
- N'a jamais fumé ou a fumé moins de 10 paquets-années et arrêté de fumer depuis plus de 15 ans.
- âge de 55 à 75 ans pour les sujets sans antécédent familial de cancer du poumon ; âge 50-75 ans pour les sujets ayant des antécédents familiaux de cancer du poumon (moins de 50 ans mais plus âgé que l'âge au moment du diagnostic du plus jeune candidat au cancer du poumon est également éligible)
- Avoir au moins une des conditions suivantes :
(1) avec des antécédents familiaux de cancer du poumon chez des parents au 1er, 2e ou 3e degré (2) avec des antécédents d'exposition à la fumée de tabac ambiante sur le lieu de travail ou à la maison (3) des antécédents de maladie pulmonaire chronique (tuberculose ou BPCO) (4) la cuisine indice égal ou supérieur à 110 [indice de cuisson = 2/7 x (jours de cuisson à la poêle, sauté ou friture en une semaine) x (années de cuisson)] (5)cuisson sans ventilation de cuisson
Critère d'exclusion:
- avec des antécédents de cancer du poumon, ou avec des cancers à l'exception du cancer de la peau ou du cancer du col de l'utérus in situ au cours des 5 dernières années
- ne peut pas accepter une procédure de diagnostic invasive ou une intervention chirurgicale
- ont été examinés par tomodensitométrie thoracique au cours des 18 derniers mois
- avoir une hémoptysie inexpliquée dans un mois
- perte de poids supérieure à 6 kg avec raison inexpliquée sur un an
- enceinte
