Description détaillée
Le traitement des patients atteints d'ISR reste un défi. Actuellement, les stents à élution médicamenteuse (DES) et les ballonnets enrobés de médicament (DCB) sont considérés comme les stratégies de choix dans ce contexte. Cependant, les données sur la valeur du BVS chez les patients atteints d'ISR sont rares. Les BVS sont très efficaces pour inhiber la prolifération néointimale et ils évitent la nécessité d'implanter une nouvelle couche métallique permanente. En conséquence, il existe actuellement un intérêt majeur à élucider la valeur potentielle du BVS chez les patients atteints de RIS.
Cette étude prospective multicentrique espagnole évaluera les résultats cliniques et angiographiques des patients atteints de RIS traités par BVS. BVS sera implanté chez des patients sélectionnés (remplissant les critères d'inclusion et d'exclusion) présentant soit BMS-ISR soit DES-ISR. Des soins seront apportés pour assurer l'optimisation de l'appareil. Un suivi angiographique sera obtenu à 6-9 mois. Un corelab angiographique centralisé sera utilisé pour fournir des mesures QCA. Un suivi clinique sera également obtenu à 1 an puis annuel. Les événements cliniques seront jugés par un comité indépendant des événements cliniques.

Resténose intra-stent avec ischémie. Consentement éclairé signé Approbation IRB
CRITÈRE D'INTÉGRATION:
PATIENT
- Âge > 20 et < 85 ans
- Acceptation de l'évaluation angiographique tardive
- Angine de poitrine ou preuve objective d'ischémie
LÉSION
- ISR (> 50 % de sténose de diamètre lors de l'évaluation visuelle)
- Emplacement antérieur du stent connu
CRITÈRE D'EXCLUSION:
PATIENT
- Inclusion dans un autre protocole de recherche clinique
- Allergie aux agents antiplaquettaires
- Femmes en âge de procréer
- Maladies systémiques associées sévères (y compris insuffisance rénale ou hépatique) ou maladies affectant l'espérance de vie
- Infarctus du myocarde récent
- Délai depuis l'implantation du stent index < 1 mois
- Difficultés anticipées pour une évaluation angiographique tardive
LÉSION
- Thrombose du stent ou gros thrombus dans le stent
- Échec angiographique lors de l'implantation initiale du stent ou persistance ou grande dissection.
- Tortuosité ou calcification sévères ou difficultés majeures lors de l'implantation précédente d'un stent
- Diamètre du vaisseau < 2,25 mm (évaluation visuelle)
- Sténose à l'extérieur du bord de l'endoprothèse (les ISR de bord sont éligibles)
- ISR très diffus (>30 mm de longueur)
