PCR ciblée et endophtalmie aiguë (Targeted PCR)
Apport de l'identification bactérienne moléculaire rapide par PCR en temps réel dans la prise en charge de l'endophtalmie aiguë postopératoire
L'endophtalmie est une infection oculaire grave d'origine exogène (post-opératoire, post-traumatique) ou endogène (métastatique). Il s'agit d'une urgence diagnostique et thérapeutique.
Chaque patient atteint d'endophtalmie doit bénéficier immédiatement d'un prélèvement oculaire, d'une injection intra oculaire d'antibiotiques et d'une couverture antibiotique systémique.
Le traitement étiologique sera adapté en fonction de l'agent infectieux.

Description détaillée
L'objectif principal de cet essai prospectif multicentrique est d'améliorer la sensibilité et la rapidité d'identification de l'agent infectieux impliqué dans les cas d'endophtalmie, en particulier les espèces virulentes (comme Staphylococcus aureus, Streptococcus pneumoniae...) à partir de prélèvements oculaires à l'aide de données réelles. temps PCR.
Cela aidera l'ophtalmologiste à être plus efficace en fonction du type de bactérie.
Les données permettront également de comparer les deux techniques de PCR utilisées dans cette étude. L'objectif secondaire de l'étude prospective est de caractériser la résistance des espèces bactériennes rencontrées au cours de l'endophtalmie aiguë par l'antibiogramme et par l'étude des gènes de résistance, afin de permettre de corréler la résistance in vitro avec la réponse thérapeutique in vivo et d'obtenir de précieuses données épidémiologiques pour adapter l'antibiotique prophylactique.

Critère d'intégration:
- Les sujets affiliés au système de sécurité sociale
- Documents de consentement éclairé signés
- Patients de plus de 18 ans
- Personne de sexe masculin ou féminin
- Patients hospitalisés pour évaluation diagnostique et thérapeutique d'une endophtalmie aiguë post-opératoire, ou dans le cadre d'une intervention chirurgicale programmée avec prélèvement d'humeur aqueuse.
- A donné son consentement après avoir reçu des informations détaillées concernant le prélèvement intraoculaire.
Critère d'exclusion:
- antibiothérapie par voie intraveineuse (< 8 jours) pour les patients NON atteints d'endophtalmie (groupe contrôle)
- La participation du patient à l'étude a été refusée
- Patients de moins de 18 ans
- Patient majeur sous tutelle ou curatelle, ou incapable d'exprimer son consentement.
