Målrettet PCR og Akut Endophthalmitis (Targeted PCR)
Bidrag til hurtig molekylær bakteriel identifikation ved realtids-PCR til håndtering af postoperativ akut endophthalmitis
Endophthalmitis er en alvorlig øjeninfektion af eksogen oprindelse (post-operationel, posttraumatisk) eller endogen oprindelse (metastatisk). Dette er en diagnostisk og terapeutisk nødsituation.
Hver patient, der lider af endophthalmitis, skal straks have en øjenprøve, en intraokulær injektion af antibiotika og et systemisk antibiotikadæksel.
Den ætiologiske behandling vil blive tilpasset efter smittestoffet.

Detaljeret beskrivelse
Hovedformålet med dette prospektive multicenterforsøg er at forbedre følsomheden og hurtigheden af det infektiøse agens, der er involveret i tilfælde af endophthalmitis, især virulente arter (såsom Staphylococcus aureus, Streptococcus pneumoniae...) fra øjenprøver ved hjælp af ægte- tid PCR.
Dette vil hjælpe øjenlægen til at være mere effektiv i overensstemmelse med typen af bakterier.
Data vil også gøre det muligt at sammenligne begge PCR-teknikker brugt i denne undersøgelse. Det sekundære mål med det prospektive studie er at karakterisere resistensen af bakteriearter fundet under akut endophthalmitis med antibiogrammet og ved studiet af resistensgener, for at muliggøre at korrelere resistensen in vitro med den terapeutiske respons in vivo og få dyrebare epidemiologiske data til tilpasse profylaktisk antibiotika.

Inklusionskriterier:
- Emner tilknyttet det sociale sikringssystem
- Underskrevet informeret samtykkedokumentation
- Patienter over 18 år
- Mandlig eller kvindelig person
- Patienter indlagt til diagnostisk og terapeutisk evaluering af en postoperativ akut endophthalmitis eller inden for rammerne af en planlagt operation med prøveudtagning af vandig humor.
- Har givet sit samtykke efter at have modtaget detaljerede oplysninger om intraokulær prøvetagning.
Ekskluderingskriterier:
- antibiotika via intravenøs (< 8 dage) til patienter IKKE, der lider af endophthalmitis (kontrolgruppe)
- Patientens deltagelse i undersøgelsen afviste
- Patienter under 18 år
- Patienter voksne under værgemål eller kuratorskab, eller ude af stand til at udtrykke sit samtykke.
- Gravid kvinde
- Ammende kvinder
- Frihedsberøvet person
