L'alcool et le tabagisme ont causé une pancrétite aiguë récurrente pourraient être évités. La condition est connue pour son effet causal de la pancréatite chronique et son effet non bénéfique sur la qualité de vie. L'équilibre clinique concernant l'impact de l'arrêt de l'alcool et du tabac existe toujours, en raison de l'absence d'essais cliniques bien conçus. Le but de l'étude REAPPEAR est d'étudier l'effet d'un programme d'arrêt de la consommation d'alcool et de tabac sur la récurrence de la pancréatite aiguë.
L'étude REAPPEAR est un essai clinique combiné impliquant un essai clinique multicentrique randomisé (REAPPEAR-T), qui évalue l'effet du programme de sevrage sur la récurrence de la pancréatite aiguë, et une analyse de cohorte, qui étudie l'effet du sevrage tabagique et alcoolique. sur la récidive de la pancréatite aiguë. Les fumeurs quotidiens, hospitalisés pour une pancréatite induite par l'alcool seront inclus dans l'essai. Une intervention de cessation standard sera fournie à tous les patients avant l'inscription. Des tests de laboratoire, la mesure de la pression artérielle et de l'IMC seront effectués, tandis que des échantillons de cheveux, d'urine et de sang seront récupérés pour une mesure ultérieure des biomarqueurs. L'évaluation de la motivation au changement, de la dépendance, de la qualité de vie et du statut socio-économique sera enregistrée à chaque visite, qui aura lieu tous les 3 mois ou annuellement selon l'attribution aléatoire. Pour les patients qui se présentent à la visite tous les 3 mois, une brève intervention sera fournie, ainsi qu'un test de laboratoire pour fournir un retour d'information. Le principal critère d'évaluation composite de cette étude sera le taux de récidive de la pancréatite aiguë, quelle que soit l'étiologie et la mortalité toutes causes confondues, sur une période de 2 ans. Le rapport coût-efficacité sera également évalué.

Critère d'intégration:
- Patient hospitalisé avec PA induite par l'alcool
- Consommation régulière d'au moins 40 g (femmes)/ 50 g (hommes) d'alcool par jour ou 280 g (femmes)/ 350 g (hommes) d'alcool au cours de la semaine précédant l'apparition des douleurs abdominales
- Fumeur quotidien (un adulte qui a fumé au moins 100 cigarettes au cours de sa vie et qui fume maintenant tous les jours ; définition du CDC), avec au moins un an d'antécédents de tabagisme
- personnes âgées de 18 à 65 ans
- terminé l'intervention standard
- fourni un consentement éclairé écrit
Critère d'exclusion:
- Autres étiologies possibles de la PA (par ex. calculs biliaires, hypertriglycéridémie supérieure à 11,5 mM, hypercalcémie, infection virale) et les cas présentant plus d'un facteur étiologique identifié seront exclus
- Maladies psychiatriques majeures (schizophrénie, trouble bipolaire, démence)
- recevant actuellement une thérapie pour un trouble lié à la consommation d'alcool
- participe actuellement à un programme de sevrage tabagique
- au moins 3 épisodes documentés de PA ou de pancréatite chronique diagnostiquée
- suivre un traitement actif ou palliatif pour une tumeur maligne
- grossesse
- pancréatite chronique
- l'espérance de vie est inférieure à deux ans

Comparateur actif: Intervention standard plus intervention répétée
Patients randomisés dans l'intervention standard plus le bras d'intervention répétée

L'intervention standard (IS) sera effectuée comme décrit ci-dessus.
L'intervention répétée sera délivrée par la même infirmière mentionnée précédemment et sera structurée de manière similaire à l'intervention standard.
Chaque visite et intervention sera modifiée individuellement en fonction de la motivation du patient, mais elles suivront la même structure.
Les séances peuvent être divisées en trois parties principales : premièrement, mettre en évidence les effets néfastes du tabac et de l'alcool sur les fonctions pancréatiques.
Deuxièmement, une discussion sur la motivation du patient aura lieu.
La dernière partie de la session se concentrera sur la responsabilité de l'individu pour atteindre l'objectif souhaité sera mis en évidence.
Afin d'améliorer l'efficacité de notre intervention, nous souhaitons fournir au patient un feed-back basé sur des tests de laboratoire : les valeurs de volume corpusculaire moyen (VMC) et de gamma glutamyl-transférase (GGT) seront mesurées juste avant l'entretien.
L'infirmière de l'étude ne participera pas aux soins des patients.

Comparateur actif: Intervention standard uniquement
Patients randomisés dans le bras d'intervention standard uniquement
|
L'intervention standard (IS) fera partie des soins standard dans tous les centres participants et sera fournie à tous les patients atteints de pancréatite aiguë, qui sont hospitalisés et dont l'état est induit par l'alcool.
Une infirmière spécialement formée à l'étude effectuera l'intervention, car elle s'est avérée la plus efficace en ce qui concerne la diminution de la consommation d'alcool et du tabagisme.
Le rapport coût-efficacité de l'intervention et la faisabilité ont également été pris en compte.
L'intervention brève liée à l'assistance selon l'Organisation mondiale de la santé (OMS) servira de base à l'intervention, avec une moyenne de 30 minutes, basée sur une revue contenant 69 essais contrôlés randomisés, qui ont conclu qu'une intervention plus longue n'a pas de bénéfices.
Les patients seront également informés de l'évolution de la pancréatite aiguë au cours de l'intervention standard.

Le diagnostic de PA repose sur la règle des deux sur trois.
(Au moins 2 des 3 caractéristiques typiques sont présentes - douleur abdominale supérieure, élévation des enzymes pancréatiques sériques d'au moins 3 fois la valeur de référence et résultats d'imagerie compatibles avec l'AP.)
