Verhinderung des Wiederauftretens einer akuten Pankreatitis durch Alkohol und Raucherentwöhnung (REAPPEAR)

Detaillierte Beschreibung
Eine durch Alkohol und Rauchen verursachte rezidivierende akute Pankretitis könnte verhindert werden. Die Erkrankung ist bekannt für ihre ursächliche Wirkung auf eine chronische Pankreatitis und ihre nicht vorteilhafte Wirkung auf die Lebensqualität. Aufgrund des Fehlens gut konzipierter klinischer Studien besteht immer noch klinisches Gleichgewicht bezüglich der Auswirkungen von Alkohol und Raucherentwöhnung. Das Ziel der REAPPEAR-Studie ist es, die Wirkung eines Programms zur Beendigung des Alkohol- und Tabakkonsums auf das Wiederauftreten einer akuten Pankreatitis zu untersuchen.
Die REAPPEAR-Studie ist eine kombinierte klinische Studie, bestehend aus einer randomisierten multizentrischen klinischen Studie (REAPPEAR-T), die die Wirkung des Entwöhnungsprogramms auf das Wiederauftreten der akuten Pankreatitis untersucht, und einer Kohortenanalyse, die die Wirkung von Rauch- und Alkoholentwöhnung untersucht über das Wiederauftreten einer akuten Pankreatitis. Tägliche Raucher, die mit alkoholinduzierter Pankreatitis ins Krankenhaus eingeliefert wurden, werden in die Studie aufgenommen. Vor der Aufnahme wird allen Patienten eine standardmäßige Einstellungsintervention angeboten. Laboruntersuchungen, Blutdruck- und BMI-Messungen werden durchgeführt, während auch Haare, Urinproben und Blut für eine spätere Biomarker-Messung entnommen werden. Die Evaluation von Wechselmotivation, Sucht, Lebensqualität und sozioökonomischem Status wird bei jedem Besuch, der je nach Randomisierung alle 3 Monate oder jährlich stattfindet, erfasst. Für Patienten, die sich alle 3 Monate bei einem Besuch vorstellen, wird eine kurze Intervention zusammen mit einem Labortest durchgeführt, um Feedback zu geben. Der primäre zusammengesetzte Endpunkt dieser Studie ist die Rezidivrate einer akuten Pankreatitis, unabhängig von der Ätiologie, und die Gesamtmortalität in einem Zeitraum von 2 Jahren. Auch die Wirtschaftlichkeit wird bewertet.

Beschreibung
Einschlusskriterien:
- Patient mit alkoholinduzierter AP ins Krankenhaus eingeliefert
- Regelmäßiger Konsum von mindestens 40 g (Frauen)/ 50 g (Männer) Alkohol täglich oder 280 g (Frauen)/ 350 g (Männer) Alkohol in der Woche vor Beginn der Bauchschmerzen
- Täglicher Raucher (ein Erwachsener, der in seinem Leben mindestens 100 Zigaretten geraucht hat und jetzt jeden Tag raucht; CDC-Definition), mit mindestens 1-jähriger Rauchergeschichte
- Personen zwischen 18 und 65 Jahren
- den Standardeingriff abgeschlossen
- schriftliche Einverständniserklärung vorgelegt
Ausschlusskriterien:
- Andere mögliche Ursachen für AP (z. Gallensteine, Hypertriglyzeridämie über 11,5 mM, Hyperkalzämie, Virusinfektion) und Fälle mit mehr als einem identifizierten ätiologischen Faktor werden ausgeschlossen
- Schwere psychiatrische Erkrankungen (Schizophrenie, bipolare Störung, Demenz)
- derzeit in Therapie wegen Alkoholkonsumstörung
- Sie nehmen derzeit an einem Raucherentwöhnungsprogramm teil
- mindestens 3 dokumentierte Episoden von AP oder diagnostizierter chronischer Pankreatitis
- sich einer aktiven oder palliativen Behandlung einer bösartigen Erkrankung unterziehen
- Schwangerschaft
- chronische Pankreatitis
- Die Lebenserwartung beträgt weniger als zwei Jahre

Der Standardeingriff (SI) wird wie oben beschrieben durchgeführt.
Der wiederholte Eingriff wird von der zuvor erwähnten Pflegekraft durchgeführt und ist ähnlich wie der Standardeingriff strukturiert.
Jeder Besuch und jede Intervention wird individuell nach der Motivation des Patienten angepasst, aber sie folgen der gleichen Struktur.
Die Sitzungen können in drei Hauptteile unterteilt werden: Erstens Hervorhebung der schädlichen Auswirkungen von Rauchen und Alkohol auf die Funktionen der Bauchspeicheldrüse.
Zweitens findet eine Diskussion über die Motivation des Patienten statt.
Der letzte Teil der Sitzung konzentriert sich auf die Verantwortung des Einzelnen, das gewünschte Ziel zu erreichen.
Um die Wirksamkeit unserer Intervention zu verbessern, möchten wir dem Patienten basierend auf Labortests ein Feedback geben: Das mittlere korpuskuläre Volumen (MCV) und die Gamma-Glutamyl-Transferase (GGT)-Werte werden direkt vor dem Interview gemessen.
Die Study Nurse nimmt nicht an der Patientenversorgung teil.

Aktiver Komparator: Nur Standardeingriff
Patienten, die in den Arm nur mit Standardintervention randomisiert wurden

Die Standardintervention (SI) wird Teil der Standardversorgung in allen teilnehmenden Zentren sein und allen Patienten mit akuter Pankreatitis angeboten werden, die stationär behandelt werden und deren Zustand alkoholinduziert ist.
Eine speziell ausgebildete Studienschwester wird die Intervention durchführen, da sie sich als am effektivsten in Bezug auf die Verringerung des Alkoholkonsums und des Rauchens erwiesen hat.
Auch die Wirtschaftlichkeit der Intervention und die Durchführbarkeit wurden berücksichtigt.
Die Assist-Linked Short Intervention nach Angaben der Weltgesundheitsorganisation (WHO) dient als Basis für die Intervention, mit einem Durchschnitt von 30 Minuten, basierend auf einer Überprüfung mit 69 randomisierten kontrollierten Studien, die zu dem Schluss kam, dass längere Interventionen keine zusätzlichen haben Vorteil.
Im Rahmen des Standardeingriffs werden die Patienten auch über den Krankheitsverlauf der akuten Pankreatitis aufgeklärt.
