Door alcohol en roken veroorzaakte terugkerende acute pancretitis zou kunnen worden voorkomen. De aandoening staat bekend om het oorzakelijke effect van chronische pancreatitis en het niet-gunstige effect op de kwaliteit van leven. Klinische evenwichtigheid met betrekking tot de impact van alcohol en stoppen met roken bestaat nog steeds, vanwege het ontbreken van goed opgezette klinische onderzoeken. Het doel van de REAPPEAR-studie is om het effect te onderzoeken van het stoppen met alcohol- en tabaksgebruik op het opnieuw optreden van acute pancreatitis.
De REAPPEAR-studie is een gecombineerde klinische studie met een gerandomiseerde multicenter klinische studie (REAPPEAR-T), die het effect van het stopprogramma op het opnieuw optreden van de acute pancreatitis beoordeelt, en een cohortanalyse, die het effect van stoppen met roken en het stoppen met alcohol onderzoekt. op het terugkeren van acute pancreatitis. Dagelijkse rokers die in het ziekenhuis zijn opgenomen met door alcohol veroorzaakte pancreatitis zullen in de proef worden opgenomen. Standaard stopinterventie zal voor alle patiënten worden verstrekt vóór inschrijving. Laboratoriumtesten, meting van bloeddruk en BMI zullen worden uitgevoerd, terwijl ook haar, urinemonsters en bloed zullen worden opgehaald voor latere biomarkermeting. De evaluatie van motivatie om te veranderen, verslaving, kwaliteit van leven en sociaal-economische status zal worden geregistreerd bij elk bezoek, dat elke 3 maanden of jaarlijks zal plaatsvinden volgens de willekeurige toewijzing. Voor patiënten die elke 3 maanden op bezoek komen, zal een korte interventie worden gegeven, samen met een laboratoriumtest om feedback te geven. Het primaire samengestelde eindpunt van deze studie zal het recidiefpercentage van acute pancreatitis zijn, ongeacht de etiologie en mortaliteit door alle oorzaken in een tijdsbestek van 2 jaar. Ook wordt de kosteneffectiviteit geëvalueerd.

Beschrijving
Inclusiecriteria:
- Patiënt opgenomen in het ziekenhuis met door alcohol geïnduceerde AP
- Regelmatige consumptie van ten minste 40 g (vrouwen)/50 g (mannen) alcohol per dag of 280 g (vrouwen)/350 g (mannen) alcohol gedurende de voorafgaande week van het begin van de buikpijn
- Dagelijkse roker (een volwassene die in zijn of haar leven minstens 100 sigaretten heeft gerookt, en die nu elke dag rookt; CDC-definitie), met een geschiedenis van minstens 1 jaar roken
- personen tussen de 18 en 65 jaar
- voltooide de standaardinterventie
- mits schriftelijke geïnformeerde toestemming
Uitsluitingscriteria:
- Andere mogelijke etiologieën voor AP (bijv. galstenen, hypertriglyceridemie boven 11,5 mM, hypercalciëmie, virale infectie) en gevallen met meer dan één geïdentificeerde etiologische factor worden uitgesloten
- Ernstige psychiatrische aandoeningen (schizofrenie, bipolaire stoornis, dementie)
- momenteel in therapie voor alcoholgebruiksstoornis
- momenteel deelneemt aan een programma om te stoppen met roken
- ten minste 3 gedocumenteerde episodes van AP of gediagnosticeerde chronische pancreatitis
- actieve of palliatieve behandeling ondergaan voor maligniteit
- zwangerschap
- chronische pancreatitis
- levensverwachting is minder dan twee jaar

Actieve vergelijker: Standaard interventie plus herhaalde interventie
Patiënten gerandomiseerd naar de arm met standaardinterventie plus herhaalde interventie

Standaardinterventie (SI) zal worden uitgevoerd zoals hierboven beschreven.
De herhalingsinterventie wordt uitgevoerd door de eerder genoemde verpleegkundige en is op dezelfde manier gestructureerd als de standaardinterventie.
Elk bezoek en elke interventie wordt individueel aangepast aan de motivatie van de patiënt, maar ze volgen dezelfde structuur.
De sessies kunnen in drie hoofddelen worden verdeeld: ten eerste, aandacht voor de schadelijke effecten van roken en alcohol op de alvleesklierfuncties.
Ten tweede vindt er een discussie plaats over de motivatie van de patiënt.
Het laatste deel van de sessie zal zich richten op de verantwoordelijkheid van het individu om het gewenste doel te bereiken.
Om de doeltreffendheid van onze interventie te vergroten, willen we de patiënt feedback geven op basis van laboratoriumtesten: het gemiddelde corpusculaire volume (MCV) en de waarden voor gammaglutamyltransferase (GGT) worden vlak voor het interview gemeten.
De studieverpleegkundige neemt geen deel aan de patiëntenzorg.

Actieve vergelijker: Alleen standaard interventie
Patiënten gerandomiseerd naar de arm met standaard alleen interventie

Standaardinterventie (SI) zal deel uitmaken van de standaardzorg in alle deelnemende centra en zal worden verstrekt aan alle patiënten met acute pancreatitis die in het ziekenhuis zijn opgenomen en hun toestand door alcohol is veroorzaakt.
Een speciaal opgeleide studieverpleegkundige zal de interventie uitvoeren, aangezien deze het meest effectief bleek te zijn wat betreft de vermindering van alcoholgebruik en roken.
Ook werd rekening gehouden met de kosteneffectiviteit van de interventie en de haalbaarheid.
De aan Assist gekoppelde korte interventie zal volgens de Wereldgezondheidsorganisatie (WHO) als basis dienen voor de interventie, met een gemiddelde duur van 30 minuten, gebaseerd op een review met 69 gerandomiseerde gecontroleerde onderzoeken, waaruit bleek dat een langere interventie geen aanvullende voordeel.
Tijdens de standaardinterventie krijgen de patiënten ook voorlichting over het ziekteverloop van acute pancreatitis.
