Critère d'intégration
- Éligible au traitement par la dapagliflozine selon les indications actuelles approuvées par la FDA, y compris : 1) insuffisance cardiaque (classe II-IV de la NYHA) avec fraction d'éjection ventriculaire gauche réduite (
- Prévu pour subir l'ablation d'une FA paroxystique ou persistante symptomatique avant la date de randomisation selon les indications des lignes directrices actuelles.
- Un taux d'hémoglobine glyquée < 10,5 % au cours des 6 derniers mois calendaires précédant le consentement uniquement chez les patients atteints de diabète sucré de type 2
- Âge > 18 ans
- CIED fonctionnel existant ou prévu de subir un implant CIED en tant que SOC avant ou dans les 24 heures suivant la procédure d'ablation de la FA et avant la date de randomisation.
Critère d'exclusion
- Diabète de type 1
- Antécédents d'acidocétose diabétique
- Maladie du foie de classe C de Child Pugh
- GFR estimé pour la dernière mesure < 25 ml/minute/1,73 m2
- Grossesse, projet de tomber enceinte
- Traitement actuel avec un inhibiteur du SGLT2
- Hypersensibilité à la dapagliflozine
- Sur la liste des greffes cardiaques ou susceptible de subir une greffe cardiaque
- Refus ou incapacité de coopérer avec le protocole
- Participation à d'autres essais cliniques (les registres d'observation sont autorisés avec approbation).
- Refus de signer le consentement à la participation

Comparateur actif: Bras de gestion des interventions (dapagliflozine)
Une méthode de randomisation en bloc sera utilisée pour randomiser les sujets recevant un traitement par dapagliflozine 10 mg une fois par jour.
Les sujets prendront 1 comprimé en aveugle du médicament à l'étude (dapagliflozine) administré une fois par jour, selon le schéma de randomisation, pendant 12 mois.
Chaque sujet recevra 35 doses en aveugle par mois, au cours de chaque mois de participation à l'essai.
Des comptages trimestriels de pilules seront effectués pour suivre l'observance.

Pourcentage moyen de temps passé en fibrillation auriculaire

Pour le critère d’évaluation principal du fardeau de la FA, défini comme le pourcentage du temps passé en FA (c.-à-d. temps passé en FA divisé par la durée totale pendant laquelle un patient a été surveillé), le test t de l'étudiant sera utilisé pour comparer statistiquement le groupe placebo par rapport au groupe de traitement actif. Cette approche est soumise à un examen de la répartition du pourcentage de temps en AF. Les données AF seront collectées à l'aide d'un dispositif électronique implantable cardiaque (CIED). Charge de FA évaluée 6 à 12 mois après l'ablation par cathéter de la FA, définie comme le pourcentage du temps passé en FA (c'est-à-dire temps passé en AF divisé par la durée totale de surveillance du sujet). |

Le questionnaire AFEQT est un questionnaire sur la qualité de vie lié à la santé spécifique à la FA, conçu pour être utilisé en recherche clinique.
Les réponses à l'AFEQT sont notées sur une échelle de Likert, de 1 = « Pas du tout… » à 7 = « Extrêmement ».
Les scores vont de 0 à 100.
Un score de 0 correspond à un handicap complet et un score de 100 correspond à aucun handicap.
