Inclusiecriteria
- Komt in aanmerking voor behandeling met dapagliflozine volgens de huidige door de FDA goedgekeurde indicaties, waaronder: 1) hartfalen (NYHA klasse II-IV) met verminderde linkerventrikelejectiefractie (
- Gepland om ablatie te ondergaan van symptomatische paroxismale of aanhoudende AF voorafgaand aan de datum van randomisatie volgens de huidige richtlijnindicaties.
- Een geglyceerd hemoglobinegehalte < 10,5% gedurende de afgelopen 6 kalendermaanden voorafgaand aan toestemming alleen bij patiënten met diabetes mellitus type 2
- Leeftijd > 18 jaar
- Bestaande functionele CIED of geplande CIED-implantatie als SOC vóór of binnen 24 uur na de AF-ablatieprocedure en vóór de randomisatiedatum.
Uitsluitingscriteria
- Diabetes mellitus type 1
- Geschiedenis van diabetische ketoacidose
- Child Pugh Klasse C leverziekte
- Laatst gemeten geschatte GFR < 25 ml/minuut/1,73 m2
- Zwangerschap, plan om zwanger te worden
- Huidige therapie met een SGLT2-remmer
- Overgevoeligheid voor dapagliflozine
- Staat op de lijst van harttransplantaties of zal waarschijnlijk een harttransplantatie ondergaan
- Niet willen of kunnen meewerken aan het protocol
- Deelname aan andere klinische onderzoeken (observatieregistraties zijn toegestaan met goedkeuring).
- Niet bereid om de toestemming voor deelname te ondertekenen

Actieve vergelijker: Interventie Management Arm (Dapagliflozine)
Er zal een blokrandomisatiemethode worden gebruikt om proefpersonen te randomiseren voor behandeling met dapagliflozine 10 mg eenmaal daags.
Proefpersonen nemen gedurende 12 maanden 1 geblindeerde tablet van het onderzoeksgeneesmiddel (dapagliflozine), eenmaal daags gedoseerd, volgens het randomisatieschema.
Elke proefpersoon krijgt 35 geblindeerde doses per maand toegediend, gedurende elke maand van deelname aan het onderzoek.
Driemaandelijkse pillentellingen zullen worden uitgevoerd om de naleving te volgen.

Proefpersonen nemen gedurende 12 maanden 1 geblindeerde tablet van het placebogeneesmiddel, eenmaal daags gedoseerd, volgens het randomisatieschema.
Elke proefpersoon krijgt 35 geblindeerde doses per maand toegediend, gedurende elke maand van deelname aan het onderzoek.

Voor het primaire eindpunt van AF-last, gedefinieerd als percentage van de tijd doorgebracht in AF (d.w.z. hoeveelheid tijd doorgebracht in AF gedeeld door de totale hoeveelheid tijd dat een patiënt werd gevolgd), zal de t-test van de student worden gebruikt om de placebogroep statistisch te vergelijken met de actieve behandelingsgroep. Deze aanpak is onderworpen aan een onderzoek naar de verdeling van het percentage tijd in AF. AF-gegevens worden verzameld met behulp van een cardiaal implanteerbaar elektronisch apparaat (CIED).
