Cette étude est conçue pour évaluer l'efficacité et l'innocuité du datopotamab deruxtecan (Dato-DXd) en association avec le pembrolizumab par rapport au pembrolizumab seul chez les participants atteints d'un cancer du poumon non à petites cellules (NSCLC) avancé ou métastatique.

Aperçu de l'étude

Description détaillée

L'objectif principal de l'étude est de comparer l'efficacité de Dato-DXd et du pembrolizumab avec le pembrolizumab seul en termes de survie sans progression (PFS) ou de survie globale (OS) pour les participants atteints de NSCLC avancé ou métastatique sans altérations génomiques actionnables dont la tumeur a expression élevée du ligand de mort programmé 1 (PD-L1) (TPS ≥ 50 %) et qui n'ont pas reçu auparavant de traitement systémique pour le NSCLC avancé ou métastatique.

Les participants éligibles seront randomisés selon un ratio 1:1 dans le bras contrôle (pembrolizumab seul) ou dans le bras expérimental (Dato-DXd et pembrolizumab). L'étude sera divisée en 4 périodes : période de dépistage des tissus, période de dépistage, période de traitement et période de suivi.

Critère d'intégration:

Les participants éligibles à l'inclusion dans l'étude doivent répondre à tous les critères d'inclusion dans les 28 jours suivant la randomisation dans l'étude.

  • Signez et datez les formulaires de dépistage des tissus et de consentement éclairé principal, avant le début de toute procédure de qualification spécifique à l'étude.
  • Adultes ≥ 18 ans ou ayant l'âge minimum légal de l'adulte (selon le plus élevé) au moment du consentement éclairé.
  • CBNPC histologiquement documenté qui répond à tous les critères suivants :

    1. Maladie de stade IIIB ou IIIC et non candidats à une résection chirurgicale ou à une chimioradiothérapie définitive, ou maladie NSCLC de stade IV au moment de la randomisation (basé sur l'American Joint Committee on Cancer, Eighth Edition).
    2. Résultats de test négatifs documentés pour les altérations génomiques actionnables du récepteur du facteur de croissance épidermique (EGFR), de la lymphome kinase (ALK) et du proto-oncogène1 (ROS1) sur la base de l'analyse du tissu tumoral.
    3. Aucune altération génomique actionnable connue dans la kinase neurotrophique du récepteur de la tyrosine (NTRK), le proto-oncogène B-raf (BRAF), réarrangé pendant la transfection (RET), le facteur de transition mésenchymateuse-épithéliale (MET) ou d'autres kinases conductrices actionnables avec des thérapies approuvées localement.
  • A fourni un échantillon de tissu tumoral fixé au formol pour la mesure de l'expression de la protéine de surface des cellules trophoblastiques 2 (TROP2) et pour l'évaluation d'autres biomarqueurs exploratoires.
  • La tumeur a une expression élevée du récepteur de mort programmée-1 (PD-L1) (TPS ≥ 50 %) comme déterminé par le test PD-L1 immunohistochimie (IHC) 22C3 pharmDx par test central (minimum de 6 lames).
  • A une période de sevrage de traitement adéquate avant le Cycle 1 Jour 1.
  • Maladie mesurable basée sur l'évaluation par imagerie locale à l'aide de RECIST version 1.1.
  • A une fraction d'éjection ventriculaire gauche (FEVG) ≥ 50 % par un échocardiogramme (ECHO) ou un balayage d'acquisition multi-fenêtres (MUGA) dans les 28 jours précédant la randomisation.
  • Statut de performance (PS) de l'Eastern Cooperative Oncology Group (ECOG) de 0 ou 1 lors de la sélection.
  • A une espérance de vie d'au moins 3 mois.
  • Fonction adéquate de la moelle osseuse dans les 7 jours précédant la randomisation.

Critère d'exclusion:

  • A déjà reçu un traitement systémique pour un NSCLC avancé ou métastatique.
  • A déjà reçu un traitement avec l'un des traitements suivants, y compris en situation adjuvante/néoadjuvante :

    1. Tout agent, y compris un conjugué anticorps-médicament, contenant un agent chimiothérapeutique ciblant la topoisomérase I.
    2. Thérapie ciblée TROP2.
    3. Tout agent anti-récepteur de mort programmé 1 (PD-1), anti-PD-L1 ou anti-PD-ligand 2 (L2) ou avec un agent dirigé vers un autre récepteur de lymphocytes T stimulateur ou co-inhibiteur (p. CTLA-4, OX40, CD137).
    4. Tout autre inhibiteur de point de contrôle immunitaire. Les participants qui ont reçu un traitement adjuvant ou néoadjuvant AUTRE que ceux énumérés ci-dessus sont éligibles si le traitement adjuvant/néoadjuvant a été terminé au moins 6 mois avant le diagnostic de maladie avancée/métastatique.
  • A une compression de la moelle épinière ou des métastases actives et non traitées du système nerveux central (SNC) et/ou une méningite carcinomateuse. Les participants avec des métastases cérébrales déjà traitées peuvent participer à condition qu'ils soient radiologiquement stables.
  • A reçu une radiothérapie antérieure ≤ 4 semaines après le début de l'intervention de l'étude ou plus de 30 Gy (unité de dose de rayonnement ionisant dans le Système international d'unités) au poumon dans les 6 mois suivant le cycle 1 jour 1.
  • Antécédents d'une autre tumeur maligne primaire (au-delà du NSCLC) à l'exception de :

    1. Malignité traitée à visée curative et sans maladie active connue ≥ 3 ans avant la première dose du traitement à l'étude et présentant un faible risque potentiel de récidive.
    2. Cancer de la peau non mélanome ou lentigo maligna traité de manière adéquate sans signe de maladie.
    3. Carcinome in situ traité de manière adéquate sans signe de maladie.
    4. Participants ayant des antécédents de cancer de la prostate (stade tumeur/ganglion/métastase) de stade ≤T2cN0M0 sans récidive ou progression biochimique et qui, de l'avis de l'investigateur, ne sont pas considérés comme nécessitant une intervention active.
  • A des antécédents de maladie pulmonaire interstitielle (MPI)/pneumopathie interstitielle (non infectieuse) nécessitant des stéroïdes, a une MPI/pneumonie en cours ou une suspicion de MPI/pneumonie ne peut être exclue par imagerie lors du dépistage.
  • Affection pulmonaire cliniquement grave résultant de maladies pulmonaires intercurrentes, y compris, mais sans s'y limiter, tout trouble pulmonaire sous-jacent, ou tout trouble auto-immun, du tissu conjonctif ou inflammatoire avec atteinte pulmonaire ou pneumonectomie complète antérieure.
  • Maladie cardiovasculaire non contrôlée ou importante, y compris :

    1. Intervalle QT moyen corrigé pour la fréquence cardiaque à l'aide de l'intervalle de la formule de Fridericia (QTcF) > 470 msec quel que soit le sexe (basé sur la moyenne de la détermination de l'électrocardiogramme à 12 dérivations lors du dépistage).
    2. Infarctus du myocarde dans les 6 mois précédant la randomisation.
    3. Angine de poitrine non contrôlée dans les 6 mois précédant la randomisation.
    4. FEVG
    5. Insuffisance cardiaque congestive (ICC) de classe 2 à 4 de la New York Heart Association lors du dépistage.
    6. Hypertension non contrôlée (pression artérielle systolique au repos> 180 mmHg ou pression artérielle diastolique> 110 mmHg) dans les 28 jours précédant la randomisation.

Les participants ayant des antécédents d'ICC de classe 2 à 4 avant le dépistage doivent être revenus à l'ICC de classe 1 et avoir une FEVG ≥ 50 % (par un scan ECHO ou MUGA dans les 28 jours précédant la randomisation) pour être éligibles.

  • Maladie cornéenne cliniquement significative.
  • A reçu un vaccin vivant ou un vaccin vivant atténué dans les 30 jours précédant la première dose du médicament à l'étude. Pour tout participant recevant un vaccin approuvé contre le coronavirus 2 du syndrome respiratoire aigu sévère (SRAS-CoV2), veuillez suivre l'étiquette du vaccin et/ou les directives locales.
  • Maladie auto-immune active, connue ou suspectée (a une maladie auto-immune active qui a nécessité un traitement systémique au cours des 2 dernières années).
  • A un diagnostic d'immunodéficience ou reçoit une corticothérapie systémique chronique (à une dose> 10 mg par jour d'équivalent prednisone) ou toute autre forme de traitement immunosuppresseur ≤ 7 jours avant la première dose du médicament à l'étude.
  • A une infection connue par le virus de l'immunodéficience humaine (VIH) qui n'est pas bien contrôlée.
  • A une hépatite active ou une hépatite B non contrôlée ou une hépatite C active ; est positif pour le virus de l'hépatite B ou C sur la base de l'évaluation des résultats des tests de dépistage de l'hépatite B (antigène de surface de l'hépatite b, anti-HBs, anti-HBc ou ADN du virus de l'hépatite B [VHB]) ou du virus de l'hépatite C (ARN du VHC) infection.
  • A une infection non contrôlée nécessitant des antibiotiques IV, des antiviraux ou des antifongiques.
  • A eu une allogreffe de tissu/organe solide.
  • A des antécédents de réactions d'hypersensibilité graves aux substances médicamenteuses ou aux ingrédients inactifs (y compris, mais sans s'y limiter, le polysorbate 80) de Dato-DXd ou de pembrolizumab.

Les participants seront randomisés pour recevoir 200 mg de pembrolizumab suivi de 6,0 mg/kg de Dato-DXd.
