Uno studio randomizzato, in aperto, di fase 3 di Dato-DXd Plus Pembrolizumab vs Pembrolizumab da solo in soggetti naïve al trattamento con carcinoma polmonare non a piccole cellule avanzato o metastatico ad alto livello di PD-L1 (TPS ≥50%) senza alterazioni genomiche attuabili (TROPION -polmone08)

Descrizione dettagliata
L'obiettivo principale dello studio è confrontare l'efficacia di Dato-DXd e pembrolizumab con il solo pembrolizumab in termini di sopravvivenza libera da progressione (PFS) o sopravvivenza globale (OS) per i partecipanti con NSCLC avanzato o metastatico senza alterazioni genomiche attuabili il cui tumore ha alta espressione del ligando di morte programmata 1 (PD-L1) (TPS ≥50%) e che non hanno ricevuto in precedenza una terapia sistemica per NSCLC avanzato o metastatico.
I partecipanti idonei saranno randomizzati in un rapporto 1: 1 al braccio di controllo (solo pembrolizumab) o al braccio sperimentale (Dato-DXd e pembrolizumab). Lo studio sarà suddiviso in 4 periodi: Periodo di screening dei tessuti, Periodo di screening, Periodo di trattamento e Periodo di follow-up.

Criterio di inclusione:
I partecipanti idonei per l'inclusione nello studio devono soddisfare tutti i criteri di inclusione entro 28 giorni dalla randomizzazione nello studio.
- Firmare e datare lo screening dei tessuti e i principali moduli di consenso informato, prima dell'inizio di qualsiasi procedura di qualificazione specifica per lo studio.
- Adulti ≥18 anni o l'età adulta minima legale (qualunque sia maggiore) al momento del consenso informato.
NSCLC documentato istologicamente che soddisfa tutti i seguenti criteri:
- - Malattia in stadio IIIB o IIIC e non candidati alla resezione chirurgica o alla chemioradioterapia definitiva, o malattia NSCLC in stadio IV al momento della randomizzazione (basata sull'American Joint Committee on Cancer, Eighth Edition).
- Risultati dei test negativi documentati per le alterazioni genomiche attivabili del recettore del fattore di crescita epidermico (EGFR), della linfoma chinasi (ALK) e del proto-oncogene1 (ROS1) basate sull'analisi del tessuto tumorale.
- Nessuna alterazione genomica attuabile nota nella chinasi neurotrofica del recettore della tirosina (NTRK), nel proto-oncogene B-raf (BRAF), nel riarrangiamento durante la trasfezione (RET), nel fattore di transizione epiteliale mesenchimale (MET) o in altre chinasi driver attivabili con terapie approvate a livello locale.
- Ha fornito un campione di tessuto tumorale fissato in formalina per la misurazione dell'espressione della proteina 2 della proteina di superficie cellulare del trofoblasto (TROP2) e per la valutazione di altri biomarcatori esplorativi.
- Il tumore ha un'espressione elevata del recettore per la morte programmata-1 (PD-L1) (TPS ≥50%) come determinato dall'analisi immunoistochimica PD-L1 (IHC) 22C3 pharmDx mediante test centrale (minimo 6 vetrini).
- Avere un adeguato periodo di sospensione del trattamento prima del Giorno 1 del Ciclo 1.
- Malattia misurabile basata sulla valutazione dell'imaging locale utilizzando RECIST versione 1.1.
- - Ha una frazione di eiezione ventricolare sinistra (LVEF) ≥50% da un ecocardiogramma (ECHO) o da una scansione di acquisizione multigated (MUGA) entro 28 giorni prima della randomizzazione.
- Performance status (PS) dell'Eastern Cooperative Oncology Group (ECOG) pari a 0 o 1 allo screening.
- Ha un'aspettativa di vita di almeno 3 mesi.
- Adeguata funzionalità del midollo osseo entro 7 giorni prima della randomizzazione.
Criteri di esclusione:
- - Ha ricevuto un precedente trattamento sistemico per NSCLC avanzato o metastatico.
Ha ricevuto un trattamento precedente con uno qualsiasi dei seguenti, anche in ambito adiuvante/neoadiuvante:
- Qualsiasi agente, incluso un coniugato anticorpo-farmaco, contenente un agente chemioterapico mirato alla topoisomerasi I.
- Terapia mirata a TROP2.
- Qualsiasi agente anti-recettore della morte programmata-1 (PD-1), anti-PD-L1 o anti-PD-ligando 2 (L2) o con un agente diretto a un altro recettore delle cellule T stimolatore o co-inibitore (p. es., CTLA-4, OX40, CD137).
- Qualsiasi altro inibitore del checkpoint immunitario. I partecipanti che hanno ricevuto una terapia adiuvante o neoadiuvante DIVERSA da quelle sopra elencate, sono idonei se la terapia adiuvante/neoadiuvante è stata completata almeno 6 mesi prima della diagnosi di malattia avanzata/metastatica.
- Presenta compressione del midollo spinale o metastasi del sistema nervoso centrale (SNC) attive e non trattate e/o meningite carcinomatosa. I partecipanti con metastasi cerebrali precedentemente trattate possono partecipare a condizione che siano radiologicamente stabili.
- - Ha ricevuto una precedente radioterapia ≤4 settimane dall'inizio dell'intervento dello studio o più di 30 Gy (unità di dose di radiazioni ionizzanti nel Sistema internazionale di unità) al polmone entro 6 mesi dal Ciclo 1 Giorno 1.
Anamnesi di un altro tumore maligno primario (oltre il NSCLC) ad eccezione di:
- Tumori maligni trattati con intento curativo e senza malattia attiva nota ≥3 anni prima della prima dose del trattamento in studio e con basso rischio potenziale di recidiva.
- Cancro della pelle non melanoma adeguatamente trattato o lentigo maligna senza evidenza di malattia.
- Carcinoma in situ adeguatamente trattato senza evidenza di malattia.
- - Partecipanti con una storia di cancro alla prostata (stadio tumore/nodo/metastasi) di stadio ≤T2cN0M0 senza recidiva o progressione biochimica e che, secondo l'opinione dello sperimentatore, non richiedono un intervento attivo.
- - Ha una storia di malattia polmonare interstiziale (ILD) / polmonite (non infettiva) che ha richiesto steroidi, ha ILD / polmonite in corso o dove il sospetto di ILD / polmonite non può essere escluso mediante imaging allo screening.
- Compromissione polmonare clinicamente grave derivante da malattie polmonari intercorrenti incluse, ma non limitate a, qualsiasi disturbo polmonare sottostante o qualsiasi disturbo autoimmune, del tessuto connettivo o infiammatorio con coinvolgimento polmonare o precedente pneumonectomia completa.
Malattie cardiovascolari non controllate o significative, tra cui:
- Intervallo QT medio corretto per la frequenza cardiaca utilizzando la formula di Fridericia (QTcF) intervallo >470 msec indipendentemente dal sesso (basato sulla media della determinazione dell'elettrocardiogramma a 12 derivazioni allo screening).
- Infarto del miocardio entro 6 mesi prima della randomizzazione.
- Angina pectoris incontrollata entro 6 mesi prima della randomizzazione.
- LVEF
- Insufficienza cardiaca congestizia (CHF) di classe da 2 a 4 della New York Heart Association allo screening.
- Ipertensione incontrollata (pressione arteriosa sistolica a riposo >180 mmHg o pressione arteriosa diastolica >110 mmHg) nei 28 giorni precedenti la randomizzazione.
I partecipanti con una storia di CHF di Classe da 2 a 4 prima dello screening, devono essere tornati alla CHF di Classe 1 e avere LVEF ≥50% (da una scansione ECHO o MUGA entro 28 giorni prima della randomizzazione) per essere idonei.
- Malattia corneale clinicamente significativa.
- - Ha ricevuto un vaccino vivo o un vaccino vivo attenuato entro 30 giorni prima della prima dose del farmaco in studio. Per tutti i partecipanti che ricevono un vaccino approvato per la sindrome respiratoria acuta grave coronavirus 2 (SARS-CoV2), si prega di seguire l'etichetta del vaccino e/o le linee guida locali.
- Malattia autoimmune attiva, nota o sospetta (ha una malattia autoimmune attiva che ha richiesto un trattamento sistemico negli ultimi 2 anni).
- - Ha una diagnosi di immunodeficienza o sta ricevendo una terapia steroidea sistemica cronica (in dosaggio> 10 mg al giorno di prednisone equivalente) o qualsiasi altra forma di terapia immunosoppressiva ≤7 giorni prima della prima dose del farmaco in studio.
- Ha conosciuto un'infezione da virus dell'immunodeficienza umana (HIV) che non è ben controllata.
- Ha un'epatite attiva o un'epatite B non controllata o un'infezione da epatite C attiva; è positivo per il virus dell'epatite B o C in base alla valutazione dei risultati dei test per l'epatite B (antigene di superficie dell'epatite B, anti-HBs, anti-HBc o virus dell'epatite B [HBV] DNA) o virus dell'epatite C (HCV RNA) infezione.
- Ha un'infezione incontrollata che richiede antibiotici EV, antivirali o antimicotici.
- Aveva un tessuto allogenico/trapianto di organi solidi.
- Ha una storia di gravi reazioni di ipersensibilità alle sostanze del farmaco o agli ingredienti inattivi (incluso ma non limitato al polisorbato 80) di Dato-DXd o pembrolizumab.

Sperimentale: Pembrolizumab + Datopotamab Deruxtecan (Dato-DXd)
I partecipanti saranno randomizzati a ricevere 200 mg di pembrolizumab seguiti da 6,0 mg/kg di Dato-DXd.

Dato-DXd verrà somministrato come infusione endovenosa (IV) ogni 3 settimane il giorno 1 di ogni ciclo di 21 giorni.
