Critères d'éligibilité pour les soignants :
- Adultes (18 ans ou plus)
- Maîtrise de l'anglais ou de l'espagnol
- Capable de fournir un consentement éclairé
- Identifié comme soignant principal (parent ou tuteur) d'un enfant noir ou latino(a/x) hospitalisé dans l'équipe de pédiatrie générale.
- Nous exclurons les soignants d'enfants en cours d'évaluation pour maltraitance / négligence envers les enfants ou d'autres cas dans lesquels l'équipe médicale estime que les visites centrées sur la famille ne sont pas appropriées.
Critères d'éligibilité pour les cliniciens :
- pédiatres traitants qui prodiguent des soins hospitaliers aux enfants au Duke Children's Hospital.
- Capable de fournir un consentement éclairé
- Nous exclurons les cliniciens qui ne prodiguent pas de soins hospitaliers aux enfants de l'équipe de pédiatrie générale.

Comparateur actif: Coaching clinicien Bras d'intervention immédiate-- Aidants
Les cliniciens qui reçoivent l'intervention participeront jusqu'à 12 mois, ce qui comprend l'achèvement de la didactique, 8 rencontres cliniques enregistrées audio, 4 séances de rétroaction et la réalisation d'une brève entrevue et d'un sondage après l'intervention. Après avoir terminé les éléments de formation didactique, les cliniciens recevront un encadrement et des commentaires professionnels sur leur communication avec les soignants des enfants à l'hôpital. Les enquêteurs fourniront aux cliniciens des exemples illustratifs tirés de leurs rencontres pour susciter la discussion et l'autoréflexion. Les soignants d'enfants hospitalisés ne sauront pas si leur clinicien a reçu l'intervention de communication. Les enquêteurs recruteront 40 soignants dans ce bras. |

Autre: Groupe de contrôle de la liste d'attente de coaching des cliniciens - Aidants
Les cliniciens du groupe témoin sur liste d'attente serviront initialement de groupe témoin, puis recevront l'intervention pour fournir des données de faisabilité et d'acceptabilité. Les cliniciens ont randomisé le groupe témoin qui subira une didactique et une rétroaction une fois que leurs enregistrements audio pré-intervention seront terminés. Les soignants d'enfants hospitalisés ne sauront pas si leur clinicien a déjà reçu l'intervention de communication. Les enquêteurs recruteront 40 soignants dans ce bras avant que les cliniciens ne reçoivent l'intervention. |

Trust was measured using a validated 5-item measure, the Wake Forest Physician Trust Scale.
Responses to each item were a 5-point Likert scale, which was averaged together for these analyses for a total score range of 1 to 5, where a higher score indicates greater trust.

Change in Caregiver Salivary Cortisol
Délai: 30-60 minutes before and 20-30 minutes after FCR
|
The investigators will assess for reductions in caregiver salivary cortisol following interactions with clinical teams.
The investigators will collect saliva samples from caregivers at two time points: 30-60 minutes before and 20-30 minutes after family-centered rounds (FCR).
The investigators anticipate lower salivary cortisol levels in the intervention compared to control arm at 20-30 minutes following FCR.
The investigators will also compare differences in the change in cortisol from before and after FCR for each caregiver; the investigators hypothesize caregivers in the intervention arm will have a smaller change from baseline following FCR.

Using validated approach described by Lion et al. the caregiver description of "the main condition child was admitted for" will be compared to the diagnoses listed by the child's doctors in the electronic health record.
