Beschrijving
Geschiktheidscriteria voor zorgverleners:
- Volwassenen (18 jaar of ouder)
- Vaardig in Engels of Spaans
- In staat om geïnformeerde toestemming te geven
- Geïdentificeerd als primaire verzorger (ouder of voogd) van een in het ziekenhuis opgenomen zwart of latino(a/x) kind in het algemeen kindergeneeskundig team.
- We sluiten verzorgers uit van kinderen die worden beoordeeld op kindermishandeling/verwaarlozing of andere gevallen waarin het medisch team van mening is dat gezinsgerichte rondes niet gepast zijn.
Geschiktheidscriteria voor clinici:
- pediatrische behandelende artsen die ziekenhuiszorg verlenen aan kinderen in het Duke Children's Hospital.
- In staat om geïnformeerde toestemming te geven
- We zullen clinici die geen ziekenhuiszorg verlenen aan kinderen uitsluiten van het algemeen kindergeneeskundig team.

Actieve vergelijker: Clinicus Coaching Onmiddellijke Interventie Arm-- Verzorgers
Clinici die de interventie ontvangen, zullen maximaal 12 maanden deelnemen, inclusief het voltooien van de didactiek, 8 audio-opgenomen klinische ontmoetingen, 4 feedbacksessies en het voltooien van een kort interview en onderzoek na de interventie. Na het voltooien van didactische trainingselementen, krijgen clinici coaching en professionele feedback over hun communicatie met verzorgers van kinderen in het ziekenhuis. De onderzoekers zullen clinici voorzien van illustratieve voorbeelden van hun ontmoetingen om discussie en zelfreflectie op gang te brengen. Verzorgers van gehospitaliseerde kinderen weten niet of hun arts de communicatie-interventie heeft ontvangen. De onderzoekers zullen 40 zorgverleners rekruteren in deze arm. |

Ander: Clinicus Coaching Wachtlijst Controle Arm-- Zorgverleners
Artsen in de wachtlijstcontrolegroep zullen in eerste instantie dienen als controlegroep en vervolgens de interventie ontvangen om haalbaarheids- en aanvaardbaarheidsgegevens te verstrekken. De clinici die de controle-arm gerandomiseerd hebben ondergaan didactiek en feedback zodra hun pre-interventie audio-opnames voltooid zijn. Verzorgers van gehospitaliseerde kinderen weten niet of hun arts de communicatie-interventie al heeft ontvangen. De onderzoekers zullen 40 zorgverleners in deze arm rekruteren voordat clinici de interventie krijgen. |

Trust was measured using a validated 5-item measure, the Wake Forest Physician Trust Scale.
Responses to each item were a 5-point Likert scale, which was averaged together for these analyses for a total score range of 1 to 5, where a higher score indicates greater trust.

De onderzoekers zullen de deelname van zorgverleners tijdens klinische ontmoetingen meten door het aantal keren te tellen dat zorgverleners (1) een vraag stellen, (2) een voorkeur uiten en (3) een emotie uiten per ontmoeting met een arts.
Deze drie gedragingen zullen bij elkaar worden opgeteld en in totaal worden geanalyseerd als een telling van het participatieve gedrag van de zorgverlener.

Change in Caregiver Salivary Cortisol
Tijdsspanne: 30-60 minutes before and 20-30 minutes after FCR
|
The investigators will assess for reductions in caregiver salivary cortisol following interactions with clinical teams.
The investigators will collect saliva samples from caregivers at two time points: 30-60 minutes before and 20-30 minutes after family-centered rounds (FCR).
The investigators anticipate lower salivary cortisol levels in the intervention compared to control arm at 20-30 minutes following FCR.
The investigators will also compare differences in the change in cortisol from before and after FCR for each caregiver; the investigators hypothesize caregivers in the intervention arm will have a smaller change from baseline following FCR.

Using validated approach described by Lion et al. the caregiver description of "the main condition child was admitted for" will be compared to the diagnoses listed by the child's doctors in the electronic health record.
