Description détaillée
Cette étude testera si le tislelizumab seul et combiné à d'autres agents expérimentaux peut être utilisé pour améliorer les résultats du traitement chez les participants atteints d'un carcinome épidermoïde de la tête et du cou. Les principaux objectifs de l'étude sont de déterminer combien de participants peuvent ne plus avoir de signes de cancer ou avoir une certaine amélioration des signes et symptômes du cancer après le traitement et de déterminer quels événements indésirables ou effets secondaires les participants pourraient ressentir.
Le tislelizumab est utilisé pour bloquer la voie programmée de la protéine 1 de mort cellulaire afin que les cellules du système immunitaire (cellules T) puissent mieux protéger le corps contre les infections et trouver les cellules tumorales à attaquer. Le tislelizumab peut être utilisé en association avec d'autres thérapies en tant qu'approche prometteuse avec des avantages thérapeutiques potentiels pour traiter les participants atteints de cancer. L'étude recrutera environ 160 participants. Les participants seront assignés au hasard (par hasard, comme si vous jetiez une pièce de monnaie) à l'un des différents groupes de traitement. Le tislelizumab et les agents expérimentaux seront administrés par perfusion dans une veine à intervalles réguliers.
L'étude aura lieu dans plusieurs centres à travers le monde. Les traitements se poursuivront jusqu'à ce que les participants ne ressentent aucun avantage, trop d'effets secondaires ou retirent leur consentement.

Critère d'intégration:
Participants atteints d'un HNSCC R/M confirmé histologiquement ou cytologiquement et considéré comme incurable par les thérapies locales
- Les localisations tumorales primaires éligibles sont l'oropharynx, la cavité buccale, l'hypopharynx et le larynx
- Les participants ne doivent pas avoir reçu de thérapie systémique antérieure administrée dans le cadre R/M ; la thérapie systémique qui a été achevée avant la randomisation/le recrutement si elle est administrée dans le cadre d'un traitement multimodal pour une maladie localement ou locorégionalement avancée est autorisée
- Les participants doivent avoir une expression PD-L1 positive (score positif combiné [CPS] ≥ 1)
- Avoir au moins 1 lésion mesurable telle que définie par RECIST v1.1
- Statut de performance du groupe d'oncologie coopérative de l'Est de 0 ou 1
- Fonction hématologique et organique adéquate, comme indiqué par des valeurs de laboratoire spécifiques dans les 7 jours suivant la première dose du médicament à l'étude
- - Disposé à utiliser une méthode de contraception très efficace pendant toute la durée de l'étude et pendant ≥ 120 jours après la dernière dose du ou des médicaments à l'étude
Critère d'exclusion:
- Carcinome épidermoïde récurrent ou métastatique du nasopharynx (toute histologie), carcinome épidermoïde primitif inconnu, carcinome épidermoïde provenant d'une tumeur primitive de la peau et des glandes salivaires ou histologies non squameuses (p. ex., mélanome muqueux)
- Traitement antérieur avec un anti-PD-1, anti-PD-L1, PD-L2, une immunoglobuline des lymphocytes T et un domaine de mucine contenant-3 (TIM-3), LAG-3 ou tout autre anticorps ou médicament ciblant spécifiquement T- costimulation cellulaire ou voies de contrôle immunitaire
- Toute tumeur maligne active ≤ 2 ans avant la randomisation/le recrutement, à l'exception du cancer spécifique faisant l'objet de l'étude dans cette étude, ceux présentant un risque négligeable de métastases ou de décès, et tout cancer récidivant localement qui a été traité de manière curative (p. cancer, cancer superficiel de la vessie, cancer localisé de la prostate et carcinome in situ du col de l'utérus ou du sein)
- Antécédents de maladie pulmonaire interstitielle, de pneumonite non infectieuse ou de maladies pulmonaires non contrôlées, y compris la fibrose pulmonaire et les maladies pulmonaires aiguës
- Antécédents de réactions d'hypersensibilité sévères à d'autres anticorps monoclonaux ou a subi un événement indésirable à médiation immunitaire (imEA) sévère, un imAE ayant conduit à l'arrêt du traitement ou un imAE cardiaque ou oculaire de tout grade avec une immunothérapie antérieure

Number of Participants With Treatment-Emergent Adverse Events

Number of participants with adverse events (AEs), including laboratory values, vital signs, physical examinations, and electrocardiogram findings. AEs were graded on a scale of Grade 1 to Grade 5, with Grade 1 being least severe and Grade 5 being most severe. An AE is defined as any unfavorable and unintended sign, symptom, or disease temporally associated with the use of a study drug, whether considered related to study drug or not. A serious adverse event (SAE) is any untoward medical occurrence that, at any dose resulted in death, was life-threatening, required hospitalization or prolongation of existing hospitalization, resulted in disability/incapacity, was a congenital anomaly/birth defect, or was considered a significant medical AE by the investigator based on medical judgement. |
