L'hypertension pédiatrique est associée à des lésions des organes cardiovasculaires, qui peuvent à leur tour entraîner des maladies cardiovasculaires (MCV) et des événements aigus à l'âge adulte.1 Au cours des dernières décennies, la prévalence de l’hypertension a progressivement augmenté chez les enfants et les adolescents, générant un problème de santé publique important et un nombre considérable de recherches. Les valeurs de tension artérielle chez les adolescents ont tendance à être moins hétérogènes que chez les adultes, et l'impact des facteurs génétiques, même s'il est faible, peut être l'expression de mécanismes pathogènes présents depuis la naissance.
Avec cette étude, baptisée HYGEF (Hypertension chez les lycéens : Facteurs Génétiques et Environnementaux), les auteurs ont souhaité : 1. déterminer la prévalence de l'obésité et de l'hypertension (HT) dans une cohorte d'adolescents, 2. réaliser des analyses génétiques pour déterminer un lien entre des polymorphismes spécifiques et/ou des mutations de gènes impliqués dans le développement de l'HT chez l'adulte comme les gènes Adducine et la Ouabaïne endogène (EO) et l'HT et 3. explorer si l'excrétion urinaire de Na+/K+ et les marqueurs urinaires de l'inflammation et du stress oxydatif sont liés à HT et risque cardiovasculaire chez les participants avec les génotypes sélectionnés.
Les adolescents seront inscrits à l'étude dans des lycées du nord (Milan), du centre (Livourne) et du sud (Grottaglie) de l'Italie, pour obtenir un échantillon qui pourrait être représentatif des adolescents italiens. Lors de la visite au lycée, les enquêteurs obtiennent des questionnaires pour avoir des informations sur les habitudes alimentaires, le mode de vie et les antécédents familiaux, deux (ou trois, très discordantes) mesures de la tension artérielle, le poids et la taille des élèves, un échantillon d'urine et un échantillon de salive pour extraire l’ADN.
Génotypage : Sur la base de nos précédentes études génétiques réalisées chez des patients adultes souffrant d'hypertension et de maladies cardiovasculaires et rénales, 15 SNP localisés dans 13 gènes candidats ont été sélectionnés : rs1045642 dans ABCB1, rs4343 dans ACE (enzyme de conversion de l'angiotensine), rs4961 dans ADD1 (alpha-adducine), rs4984 dans ADD2 (bêta-adducine), rs3731566 dans ADD3 (gamma-adducine), rs11638442 dans CYP11A1 (cytochrome P450 famille 11 sous-famille A membre 1), rs1799998 dans CYP11B2 (aldostérone synthase), rs2236780, rs 6203 et rs10923835 dans la région génétique HSD3B1, RS9536314 dans KL, RS2254524 dans LSS, RS4149601 dans la cellule de précurseur neuronale exprimée, développement à la baisse à la baisse, NEDD4L (E3 Ubiquitine-Protein ligase), Prkg1 (Protein Kinase, Rs1904690 -dépendant, type I), rs4238595 en UMOD (uromoduline).
Analyse des urines : Na+, K+, albumine et créatinine ont été mesurés sur un premier échantillon d'urine du matin à jeun. La formule de Kawasaki a été utilisée pour estimer l’excrétion urinaire de sodium (24h UNa) sur 24 heures, exprimée en mEq/24 h. Dans un sous-groupe, un échantillon d'urine pour chaque élève sera immédiatement congelé pour l'analyse des protéines. Les enquêteurs prévoient de mesurer simultanément IL1β, IL6, IL10, IL12p70, TNF-α, MCP1 (protéine chimioattractante monocytaire), PTX3 (pentraxine-3) par test ELISA à l'aide du test de dépistage Luminex magnétique humain LXSAHM. L'α-Klotho soluble, la 8-Oxo-2'-désoxyguanosine sera mesurée par le test du kit Elisa. L'oxyde nitrique (NO) a été mesuré à l'aide du kit de dosage colorimétrique de l'oxyde nitrique. Les produits protéiques d'oxydation avancés seront mesurés à l'aide du kit de test colorimétrique OxiSelect Advanced Oxidation Protein Products.
