Hypertensjon hos elever på videregående skole: genetiske og miljømessige faktorer

Pediatrisk hypertensjon er assosiert med kardiovaskulær organskade, som igjen kan føre til kardiovaskulær sykdom (CVD) og akutte hendelser i voksen alder.1 I løpet av de siste tiårene har forekomsten av hypertensjon gradvis økt hos barn og ungdom, noe som har generert et betydelig folkehelseproblem og betydelig mengde forskning. BP-verdier blant ungdom har en tendens til å være mindre heterogene enn hos voksne, og virkningen av genetiske faktorer, selv om de er små, kan være uttrykk for patogene mekanismer som har vært tilstede siden fødselen.
Med denne studien, kalt HYGEF (Hypertension in high school students: Genetic and Environmental Factors), ønsket forfatterne å: 1. bestemme utbredelsen av fedme og hypertensjon (HT) i en ungdomskull, 2. utføre genetiske analyser for å fastslå en kobling mellom spesifikke polymorfismer og/eller mutasjoner av gener involvert i utviklingen av HT hos voksne som Adducingenene og Endogen Ouabain (EO) og HT og 3. undersøke om urin Na+/K+ utskillelse og urinmarkører for betennelse og oksidativt stress er knyttet til HT og kardiovaskulær risiko hos deltakere med de utvalgte genotypene.
Ungdom vil bli registrert for studiet ved videregående skoler i nord (Milano), sentrum (Livorno) og sør (Grottaglie) i Italia, for å få en prøve som kan være representativ for de italienske ungdommene. Under besøket på videregående skaffer etterforskerne spørreskjemaer for å ha litt informasjon om spisevaner, livsstil og familiehistorie, to (eller tre av de var svært uenige) blodtrykksmålinger, vekt og høyde til elevene, en urinprøve og en prøve av spytt for å trekke ut DNA.
Genotyping: På grunnlag av våre tidligere genetiske studier utført på voksne pasienter med hypertensjon og kardiovaskulære og nyre-relaterte sykdommer, er 15 SNP-er lokalisert i 13 kandidatgener valgt ut: rs1045642 i ABCB1, rs4343 i ACE (angiotensin-konverterende enzym), rs4961 ADD1 (alfa-adducin), rs4984 i ADD2 (beta-adducin), rs3731566 i ADD3 (gamma-adducin), rs11638442 i CYP11A1 (cytokrom P450 familie 11 underfamilie A medlem 1), rs1827 i 9P, rs1827 in 9 236780, rs6203 og rs10923835 i HSD3B1 genetisk område, rs9536314 i KL, rs2254524 i LSS, rs4149601 i nevrale forløpercelle uttrykt, utviklingsmessig nedregulert 4-lignende, NEDD4L (E3 ubiquitin-protein-protein, PRK1-ligase-protein-protein 4MP1,9MP,9MP,9MP) -avhengig, type I), rs4238595 i UMOD (uromodulin).
Analyse av urin: Na+, K+, albumin og kreatinin ble målt på en første fastende morgenurinprøve. Kawasaki-formelen ble brukt til å estimere 24-timers natriumutskillelse i urin (24 timer UNa) uttrykt som mEq/24 timer. I en undergruppe vil en urinprøve for hver elev umiddelbart fryses ned for proteinanalyse. Etterforskere planlegger å måle samtidig IL1β, IL6, IL10, IL12p70, TNF-α, MCP1 (monocytt kjemoattractant protein), PTX3 (pentraxin-3) ved ELISA-test ved bruk av LXSAHM Human Magnetic Luminex Screening Assay. Løselig α-Klotho, 8-Oxo-2'-deoksyguanosin vil bli målt ved hjelp av Elisa-settanalyse. Nitrogenoksid (NO) ble målt med det kolorimetriske nitrogenoksidanalysesettet. Advanced Oxidation Protein Products vil bli målt ved hjelp av det kolorimetriske OxiSelect Advanced Oxidation Protein Products Assay Kit.
