A gyermekkori hipertónia szív- és érrendszeri szervkárosodással jár, ami viszont szív- és érrendszeri betegségekhez (CVD) és felnőttkori akut eseményekhez vezethet. Az elmúlt néhány évtizedben a magas vérnyomás prevalenciája fokozatosan nőtt a gyermekek és serdülők körében, ami jelentős közegészségügyi problémát és jelentős mennyiségű kutatást generált. A serdülőkorúak vérnyomásértékei általában kevésbé heterogének, mint a felnőtteknél, és a genetikai tényezők hatása, még ha kicsi is, a születés óta jelen lévő patogén mechanizmusok kifejeződése lehet.
Ezzel a HYGEF-nek (Hypertonia in high school students: Genetic and Environmental Factors) a szerzők a következőket akarták: 1. meghatározni az elhízás és a magas vérnyomás (HT) prevalenciáját serdülők egy csoportjában, 2. genetikai elemzéseket végezni a kapcsolat meghatározására. A felnőtteknél a HT kialakulásában szerepet játszó gének specifikus polimorfizmusai és/vagy mutációi között, mint az Adducin gének és az endogén ouabain (EO) és a HT, és 3. vizsgálja meg, hogy a vizelet Na+/K+ kiválasztás, valamint a gyulladás és az oxidatív stressz vizeletbeli markerei összefüggésben vannak-e HT és kardiovaszkuláris kockázat a kiválasztott genotípusú résztvevőkben.
A serdülőket Olaszország északi (Milánó), középső (Livorno) és déli (Grottaglie) középiskoláiba íratják be, hogy olyan mintát kapjanak, amely reprezentatív lehet az olasz serdülők számára. A középiskolában tett látogatás során a vizsgálók kérdőíveket kapnak, amelyekben információkat kapnak az étkezési szokásokról, életmódról és családtörténetről, két (vagy három nagyon eltérő) vérnyomásmérésről, a tanulók súlyáról és magasságáról, vizeletmintát és nyálminta a DNS kivonásához.
Genotipizálás: Korábbi, magas vérnyomásban, szív- és érrendszeri és vesebetegségben szenvedő felnőtt betegeken végzett genetikai vizsgálataink alapján 15 SNP-t választottunk ki 13 jelölt génben: rs1045642 az ABCB1-ben, rs4343 az ACE-ben (angiotenzin konvertáló enzim), rs4961 ADD1 (alfa-adducin), rs4984 az ADD2-ben (béta-adducin), rs3731566 az ADD3-ban (gamma-adducin), rs11638442 a CYP11A1-ben (citokróm P450 család 11. alcsalád A-tag 1), rs1797al synase 3rs2terone2998 , rs6203 és rs10923835 a HSD3B1 genetikai régióban, rs9536314 a KL-ben, rs2254524 az LSS-ben, rs4149601 a neurális prekurzor sejtekben expresszálva, fejlődésileg leszabályozott 4-szerű, NEDD4L (E3 ubiquitin-protein liase in P4-9-90rs (P4-9-9-9-protein) függő, I. típus), rs4238595 az UMOD-ban (uromodulin).
A vizelet elemzése: Na+, K+, albumin és kreatinin szinteket mértünk az első éhgyomorra vett vizeletmintán. A Kawasaki képletet használták a 24 órás vizelettel történő nátrium (24 óra UNa) kiválasztására, mEq/24 h egységben kifejezve. Egy alcsoportban minden tanuló vizeletfoltmintáját azonnal lefagyasztják fehérjeelemzés céljából. A kutatók az IL1β, IL6, IL10, IL12p70, TNF-α, MCP1 (monocita kemoattraktáns fehérje), PTX3 (pentraxin-3) egyidejű mérését tervezik ELISA teszttel, LXSAHM Human Magnetic Luminex Screening Assay segítségével. Az oldható α-Klotho, 8-Oxo-2'-dezoxiguanozin mennyiségét Elisa kit assay segítségével mérjük. A nitrogén-oxidot (NO) a kolorimetriás Nitrogén-oxid vizsgálati kittel mértük. Az Advanced Oxidation Protein Products kolorimetrikus OxiSelect Advanced Oxidation Protein Products Assay Kit segítségével mérjük.
