L'étude est conçue comme une étude prospective randomisée et contrôlée en bloc à 2 bras. L'étude vise à recruter environ 348 participants présentant un risque accru d'infections sexuellement transmissibles (IST) - participants qui ont été en contact avec CT/NG/MG (y compris les participants qui déclarent avoir connaissance ou soupçonner une exposition à une IST, malgré l'incertitude de l'agent pathogène spécifique de l'IST). ou résultats de tests de contact), ou des participants présentant des symptômes évocateurs d'une IST - pour participer à cette étude. Les participants qui donnent leur consentement fourniront des informations démographiques et des échantillons urogénitaux pour les tests CT/NG/MG sur le système cobas® liat et pour les tests de réaction en chaîne par polymérase en laboratoire externe (EL-PCR). Les participants seront randomisés dans un rapport 1:1 en blocs (de multiples de 2) stratifiés par le site dans l'un des bras suivants :
- Norme de soins (SOC)
- Point de service (POC) Dans les deux bras, le clinicien verra et évaluera le participant et remplira un formulaire standardisé, connu sous le nom de plan de prise en charge du patient (PMP), à la fin de la rencontre du participant. La différence essentielle entre les bras est que le clinicien du bras POC recevra les résultats des tests POC sur lesquels il pourra choisir de fonder ses décisions cliniques, tandis que le bras SOC ne recevra pas les résultats des tests POC.

Critère d'intégration:
- Personnes sexuellement actives
- Les personnes recherchant des services médicaux pour des symptômes évoquant une infection sexuellement transmissible (IST) et/ou une exposition connue à une IST
Critère d'exclusion:
- Déjà inscrit à l'étude
- Incapable de fournir un consentement éclairé
- Actuellement enceinte
- Refuse les tests POC
- Cadeaux pour le dépistage systématique des IST (asymptomatiques)
- Utilisation d'agents antimicrobiens actifs contre CT, NG ou MG au cours des 21 jours précédant le prélèvement de l'échantillon. Des exemples de tels agents antimicrobiens comprennent les suivants : Macrolides (par exemple, azithromycine et érythromycine) ; Pénicillines (par ex. amoxicilline); Tétracyclines (par exemple, doxycycline); Fluoroquinolones (par ex. ciprofloxacine, ofloxacine, lévofloxacine et moxifloxacine); Céphalosporines (par ex. ceftriaxone et céfixime)
- Utilisation de médicaments contre la douleur urinaire contenant de la phénazopyridine (c'est-à-dire Azo ou Pyridium) dans les 2 jours précédant le prélèvement de l'échantillon
- Utilisation de produits d'hygiène féminine en vente libre (en interne ou en externe), tels que des hydratants vaginaux, des lubrifiants (par exemple Replens, RepHresh, etc.) et des lavages féminins/douches vaginales, etc. dans les 3 jours précédant l'échantillonnage. collection. L'utilisation de tampons ou de serviettes pendant les règles n'est pas un critère d'exclusion.

Expérimental: Bras au point de service (POC) : cobas® liat CT/NG/MG
Dans le bras POC, le clinicien évaluera le participant et recevra les résultats des tests POC sur lesquels il pourra choisir de fonder ses décisions cliniques.

Le test d'acide nucléique cobas® CT/NG/MG à utiliser sur le système cobas® Liat® est un test de diagnostic d'acide nucléique in vitro expérimental, automatisé et qualitatif qui utilise la réaction en chaîne par polymérase (PCR) en temps réel pour la détection directe de Chlamydia. trachomatis (CT), Neisseria gonorrhoeae (NG) et Mycoplasma genitalium (MG) dans l'urine masculine/femelle et les écouvillons vaginaux (collectés par le clinicien et auto-collectés) dans le milieu PCR cobas®.
