Effet d'un complément alimentaire contenant de l'EGCG, de l'acide folique, de la vitamine B12 et de l'acide hyaluronique dans le soutien de l'équilibre génital masculin chez des sujets exposés au risque de HPV (PERVI-M)

Cette étude pilote, randomisée et interventionnelle vise à évaluer l'effet d'un complément alimentaire contenant du gallate d'épigallocatéchine (EGCG), de la vitamine B12, de l'acide hyaluronique et de l'acide folique sur le maintien de l'équilibre physiologique et des défenses naturelles du tractus génital masculin chez les hommes atteints d'une infection à HPV fréquentant des centres de procréation naturelle. L'étude cherche également à évaluer si la supplémentation peut soutenir la qualité du sperme, améliorer le bien-être reproductif des couples participants et réduire les facteurs de risque associés à la transmission du HPV à la partenaire féminine.
Au total, 48 couples (96 participants) seront inscrits à l'Istituto Scientifico Internazionale "Paolo VI" du Policlinico A. Gemelli. Les couples éligibles, caractérisés par un partenaire masculin positif au HPV et une partenaire féminine négative au HPV, seront randomisés en deux groupes : un groupe supplémenté, dans lequel les deux partenaires recevront une supplémentation orale avec de l'EGCG (200 mg), de la vitamine B12 (1 mg), de l'acide hyaluronique (50 mg) et de l'acide folique (400 µg) une fois par jour pendant 6 mois, et un groupe témoin ne recevant aucune supplémentation. La randomisation sera effectuée à l'aide d'une liste générée par ordinateur.
Le critère d'évaluation principal est la proportion de participants masculins obtenant un test d'ADN HPV négatif à la fin de la période d'intervention de 6 mois. Les critères d'évaluation secondaires incluent les changements dans les paramètres de qualité du sperme évalués par analyse du sperme selon les directives de l'OMS 2021, les mesures des résultats reproductifs tels que le potentiel de fertilité et la réalisation d'une grossesse, et le maintien de la santé de la muqueuse génitale, y compris l'absence d'infections ou de récidives et la réduction du risque de transmission du HPV. Le génotypage du HPV sera évalué par PCR sur des échantillons d'écouvillons urétraux. La durée totale de l'étude devrait être de 18 mois après l'approbation du comité d'éthique.
