Klinikai vizsgálat a PROXERA PSOMED 20 és PROXERA PSOMED 40 orvostechnikai eszközökkel a plakkos pikkelysömör és a nem puszulásos palmoplantáris pikkelysömör kezelésére (PASI)
A PROXERA PSOMED 20 orvostechnikai eszköz hatékonyságának és biztonságosságának értékelése a psoriás plakkok csökkentésében, valamint a PROXERA PSOMED 40 orvostechnikai eszköz hatékonyságának értékelése a nem puszta palmoplantáris psoriasis foltok csökkentésében plakkos psoriasisban szenvedő betegeknél: támogatott, intervenciós, egyközpontú, randomizált, egyszemélyes, betegen belüli kontrollált vizsgálat (PASI)
Ez az egyközpontú intervenciós vizsgálat a két CE-jelölésű topikális orvostechnikai eszköz hatékonyságának és biztonságosságának értékelésére készült: a PROXERA PSOMED 20-et nem tenyér-talp típusú plakkos psoriasisban szenvedő résztvevőknél vizsgálják, míg a PROXERA PSOMED 40-et nem puszulásos tenyér-talp psoriasisban szenvedő résztvevőknél.
A vizsgálat célja annak megállapítása, hogy a kijelölt orvostechnikai eszközzel történő célterület-kezelés nagyobb klinikai javulást eredményez-e ugyanannak a résztvevőnek a kezeletlen kontralaterális kontrollterületéhez képest. A PROXERA PSOMED 20 esetében a fő értékelést a 4 hetes kezelés után végzik. A PROXERA PSOMED 40 esetében a fő értékelést a 8 hetes kezelés után végzik.
A másodlagos célkitűzések közé tartozik a biztonságosság, a helyi tolerálhatóság, a psoriasis általános előrehaladása, az életminőségre gyakorolt hatása, valamint a célterületeken a helyi betegségaktivitás további paramétereinek értékelése. A résztvevők a kijelölt vizsgálati eszközt naponta egyszer alkalmazzák a kiválasztott célterületre, ütemezett látogatásokon klinikai értékeléseken vesznek részt, és a vizsgálat során biztonsági és tolerálhatósági információkat szolgáltatnak.

Kísérleti: Intra-patient kezelés PROXERA PSOMED 20-val
Egycsoportos, randomizált, egyvak, betegen belüli kontrollált vizsgálati ág.
A résztvevőket diagnózis szerint osztják be kezelésre: a nem tenyér-talpi plakkos psoriasisban szenvedő résztvevők a PROXERA PSOMED 20-at kapják naponta egyszer, 4 héten keresztül egy célplakkra, míg a nem gennyes tenyér-talpi psoriasisban szenvedő résztvevők a PROXERA PSOMED 40-et kapják naponta egyszer, 8 héten keresztül egy célként megjelölt tenyér- vagy talpterületen.
Mindkét kohorszban egy hasonló, kontralaterális célterület kezelés nélkül marad kontrollként.
A PROXERA PSOMED 40 esetében a kezelést 3 napra megszakítják minden 14. napon.

Proxera Psomed 20: Topikai orvostechnikai eszköz krém formában, 20%-os karbamidot (karbamid) tartalmazó, 200 mL-es tubusban forgalmazott. A résztvevő a kiválasztott célplakkra önállóan alkalmazza naponta egyszer, naponta azonos időpontban 4 héten át, az ujjbegy egység szabálya szerint. Az ellenoldali összehasonlítható célterület kezeletlenül marad kontrollként. Proxera Psomed 40: Topikai orvostechnikai eszköz gél formában, 40%-os karbamidot (karbamid) tartalmazó, 100 mL-es tubusban forgalmazott. A résztvevő a kiválasztott cél tenyér- vagy talpterületre önállóan alkalmazza naponta egyszer, naponta azonos időpontban 8 héten át, az ujjbegy egység szabálya szerint. A kezelés 14 naponként 3 napra szünetel. Az ellenoldali összehasonlítható célterület kezeletlenül marad kontrollként.

Difference Between Treated and Untreated Target Plaques in Change From Baseline in Target Lesion Score at Week 4
Időkeret: Baseline to Week 4 (end of treatment)
|
The Target Lesion Score assesses the severity of a representative psoriatic plaque based on erythema, induration (thickness), and scaling.
Each component is scored from 0 to 4, and the total score ranges from 0 to 12, with higher scores indicating greater lesion severity.
In participants treated with PROXERA PSOMED 20, the outcome compares the change from baseline to Week 4 in the treated target plaque with the change in the contralateral untreated control plaque.

Difference Between Treated and Untreated Target Areas in Change From Baseline in Local Modified Palmoplantar Psoriasis Area and Severity Index at Week 8
Időkeret: Baseline to Week 8 (end of treatment)
|
The local modified Palmoplantar Psoriasis Area and Severity Index (mPPPASI) assesses erythema, induration, scaling, and extent of involvement in each target palm or sole area.
Erythema, induration, and scaling are each scored from 0 to 4, while extent of involvement is scored from 0 to 6.
For each target site, the local score is calculated as (erythema + induration + scaling) × extent and ranges from 0 to 72, with higher scores indicating more severe disease.
In participants treated with PROXERA PSOMED 40, the primary analysis compares the change from baseline to Week 8 in the treated target area with the change in the contralateral untreated control area.
