L'interesse e la disponibilità di cocaina, marijuana e oppiacei hanno complicato le indagini a lungo termine sugli effetti del diffuso uso ricreativo di sostanze facilmente accessibili come alcol e tabacco. Rimane impossibile determinare in un unico sito, un piccolo numero di studi quali effetti possono essere correlati all'uso di un farmaco specifico. Accedendo alla vasta popolazione multi-sito di neonati e delle loro madri disponibile nella rete di ricerca neonatale NICHD, questo studio sta valutando gli effetti a breve e lungo termine dell'uso di cocaina e/o oppiacei da parte delle madri durante la gravidanza a termine o pretermine neonati.
Le pratiche materne valutate in questo studio includono l'uso e l'abuso di oppiacei, cocaina, alcol, marijuana e nicotina. Questo studio affronterà gli eventi perinatali acuti e gli esiti medici, di sviluppo, sociali, ambientali e neurocomportamentali a lungo termine dei bambini le cui madri si sono impegnate in queste pratiche materne. Lo studio determinerà se specifici effetti acuti ea lungo termine possono essere attribuiti all'uso e all'abuso di determinate sostanze.
In 2 anni, sono stati sottoposti a screening circa 20.000 neonati con l'obiettivo di arruolarne 16.000. È stato stimato che circa il 20% dei bambini sarebbe stato esposto a cocaina o oppiacei. La determinazione dell'esposizione si è basata sull'autovalutazione della madre o sul test positivo per il meconio.
La prima fase dello studio ha valutato gli effetti acuti delle pratiche materne sui neonati. Questa fase ha coinvolto tutte le madri che hanno accettato di rispondere al questionario iniziale e che hanno permesso di eseguire lo screening farmacologico del meconio sui loro bambini. Gli esiti acuti vengono confrontati tra i bambini che sono stati esposti a cocaina e oppiacei attraverso l'uso delle loro madri (il gruppo esposto) ei bambini che non sono stati esposti (il gruppo non esposto). Gli esiti acuti includono abruptio placenta, ritardo della crescita fetale, malformazioni congenite non pericolose per la vita, sindrome da distress respiratorio, malattia polmonare cronica, emorragia periventricolare-intraventricolare, enterocolite necrotizzante, retinopatia del prematuro e leucomalacia periventricolare.
La seconda fase dello studio ha confrontato 1.400 neonati esposti e non esposti rispetto agli esiti dello sviluppo neurologico a lungo termine. Questi neonati erano tra i 16.000 neonati arruolati nella Fase I. È stato stimato che il 70% della popolazione sottoposta a screening avrebbe acconsentito a partecipare alla Fase II dello studio e che il 50% di questi partecipanti avrebbe completato tutte le visite durante il periodo iniziale di 3 anni dello studio (2.000 neonati esposti arruolati nella Fase II e 1.000 esposti i neonati completerebbero tutte le visite di follow-up). Per ogni bambino esposto, è stato selezionato un bambino di età, razza, sesso e anamnesi alcolica o età materna simili dalla popolazione non esposta e sottoposta a screening. Tutti i bambini avevano valutazioni fisiche, neurologiche, dell'età gestazionale e della crescita alla nascita. I neonati esposti e non esposti sono stati esaminati a 1, 4, 7, 9, 12, 18, 24 e 36 mesi di età corretta. Le valutazioni di follow-up includono l'anamnesi e gli esiti sullo sviluppo, comportamentali, sociali e ambientali.
La terza fase dello studio ha confrontato i bambini di età compresa tra 4 e 7 anni. La quarta fase sta ora confrontando i risultati nei bambini di età compresa tra 8 e 11 anni. Le valutazioni includono misure di cognizione, rendimento scolastico, comportamento antisociale, inizio dell'uso di sostanze, psicopatologia, funzione neuroendocrina e disturbi della salute. Il 71% del campione originale è ancora arruolato.
