Descrizione dettagliata
L'obiettivo primario di questo studio è valutare se il trattamento con I-DXd migliora il tasso di risposta obiettiva (ORR) e prolunga la sopravvivenza globale (OS) rispetto al trattamento scelto dal medico tra i partecipanti con SCLC recidivante.
Gli obiettivi secondari dello studio sono valutare ulteriormente l'efficacia/sicurezza di I-DXd, l'economia sanitaria e le misure di ricerca sui risultati (compresi i risultati riportati dai pazienti), l'immunogenicità di I-DXd e l'espressione della proteina B7-H3.

Descrizione
Criterio di inclusione
I partecipanti devono soddisfare tutti i seguenti criteri per essere idonei alla randomizzazione nello studio:
- Firmare e datare il modulo di consenso informato prima dell'inizio di qualsiasi procedura di qualificazione specifica per lo studio.
- Adulti di età pari o superiore a 18 anni o l'età minima legale per adulti (a seconda di quale sia maggiore) al momento della firma del modulo di consenso informato.
- Presenta SCLC documentato istologicamente o citologicamente.
- Il partecipante deve fornire campioni tumorali di base adeguati con quantità e qualità sufficienti di contenuto di tessuto tumorale.
- Ha ricevuto una precedente terapia con almeno una precedente linea a base di platino come terapia sistemica per SCLC con almeno 2 cicli di terapia.
- Presenta almeno una lesione misurabile secondo RECIST v1.1 valutata dallo sperimentatore.
- Ha documentazione della progressione radiologica della malattia durante o dopo la terapia sistemica più recente.
- Ha un PS ECOG pari a ≤1.
- I partecipanti con metastasi cerebrali non trattate e asintomatiche o i soggetti con metastasi cerebrali trattate che non sono più sintomatiche e che non richiedono alcun trattamento con steroidi possono essere inclusi nello studio se si sono ripresi dall'effetto tossico acuto della radioterapia.
Criteri di esclusione
I partecipanti che soddisfano uno dei seguenti criteri saranno squalificati dall'accesso allo studio:
- Ha ricevuto un precedente trattamento con orlotamab, enoblituzumab o altri agenti umanizzati anti-B7 omologo 3 (B7-H3), incluso I-DXd.
- Ha ricevuto un precedente trattamento con un ADC costituito da un derivato dell'exatecan (ad esempio, trastuzumab deruxtecan).
- Ha ricevuto uno qualsiasi dei comparatori utilizzati in questo studio o qualsiasi inibitore della topoisomerasi I.
- Ha un periodo di washout inadeguato prima della randomizzazione come specificato nel protocollo.
- Presenta una qualsiasi delle seguenti condizioni negli ultimi 6 mesi: incidente cerebrovascolare, attacco ischemico transitorio o altro evento tromboembolico arterioso.
- Ha una malattia cardiovascolare incontrollata o significativa.
- Presenta una malattia corneale clinicamente significativa.
- Ha una storia di malattia polmonare interstiziale (ILD)/polmonite (non infettiva) che ha richiesto corticosteroidi, ILD/polmonite in corso o sospetta ILD/polmonite che non può essere esclusa mediante imaging allo screening.
- Presenta una compromissione polmonare clinicamente grave derivante da malattie polmonari intercorrenti, inclusi, ma non limitati a, qualsiasi disturbo polmonare sottostante e potenziale coinvolgimento polmonare causato da qualsiasi disturbo autoimmune, del tessuto connettivo o infiammatorio, precedente pneumonectomia completa o necessità di ossigeno supplementare.

Sperimentale: Ifinatamab deruxtecan (I-DXd)
Participants will be randomized to receive 12 milligrams per kilogram (mg/kg) I-DXd, intravenously (IV), as monotherapy on Day 1 of each 21-day cycle, once every 3 weeks (Q3W), until clinical or radiologic evidence of progression of disease determined by the investigator, unacceptable toxicity, withdrawal of informed consent, death, loss to follow-up, or other treatment discontinuation criteria are met as specified in the protocol.
