Основная цель этого исследования — оценить, улучшает ли лечение I-DXd частоту объективного ответа (ЧОО) и продлевает общую выживаемость (ОВ) по сравнению с лечением по выбору врача среди участников с рецидивирующим МРЛ.

Вторичными целями исследования являются дальнейшая оценка эффективности/безопасности I-DXd, экономики здравоохранения и показателей исследования результатов (включая результаты, сообщаемые пациентами), иммуногенности I-DXd и экспрессии белка B7-H3.

Участники должны соответствовать всем следующим критериям, чтобы иметь право на рандомизацию в исследование:

  1. Подпишите и поставьте дату на форме информированного согласия до начала любых квалификационных процедур, связанных с исследованием.
  2. Взрослые ≥18 лет или минимальный возраст совершеннолетия (в зависимости от того, что больше) на момент подписания формы информированного согласия.
  3. Имеет гистологически или цитологически подтвержденный МРЛ.
  4. Участник должен предоставить адекватные исходные образцы опухоли с достаточным количеством и качеством содержимого опухолевой ткани.
  5. Ранее получал терапию по крайней мере одной линией на основе платины в качестве системной терапии МРЛ, по крайней мере, с 2 циклами терапии.

Критерий исключения

Участники, соответствующие любому из следующих критериев, будут дисквалифицированы от участия в исследовании:

  1. Ранее получал лечение орлотамабом, эноблитузумабом или другими гуманизированными таргетными агентами, гомологами анти-B7 3 (B7-H3), включая I-DXd.
  2. Ранее получал лечение ADC, состоящим из производного экзатекана (например, трастузумаба дерукстекана).
  3. Получал любой из препаратов сравнения, использованных в этом исследовании, или любой ингибитор топоизомеразы I.

Participants will be randomized to receive 12 milligrams per kilogram (mg/kg) I-DXd, intravenously (IV), as monotherapy on Day 1 of each 21-day cycle, once every 3 weeks (Q3W), until clinical or radiologic evidence of progression of disease determined by the investigator, unacceptable toxicity, withdrawal of informed consent, death, loss to follow-up, or other treatment discontinuation criteria are met as specified in the protocol.
