このパイロット研究は、MR-proADM の使用を評価および検証するために、スイスのチューリッヒ大学病院の医療集中治療室 (ICU) に連続して入院した 100 人の重症患者を含む、単一施設の前向き観察研究です。重症患者の予後マーカーとしての CT-proET1/MR-proADM-ratio。 患者のインフォームドコンセントまたは法定代理人の承認を得て、包含の会議を取得した後、除外基準を欠いている患者は登録されます。
研究登録時に、患者の健康状態は、健康関連の診断およびチャールソン併存疾患指数によって評価されます。 この情報は、ICU のすべての患者で定期的に取得されます。 チャールソン併存疾患指数は、患者の病歴に基づいて計算されます。 MR-proADM および CT-proET-1 の値は、ICU 滞在の最初の 7 日間にわたって毎日測定されます。 患者が 7 日目より前に退院した場合、それ以上の測定は行われません。 血液サンプルは、入院時および毎朝の定期的な血液検査と一緒に収集されます。 患者の臨床評価は、血行動態、器官機能障害、微小循環、および昇圧剤の必要性の評価を含めて、毎朝行われます。
患者の病気の重症度は、SAPS II スコアと SOFA スコアを使用して評価されます。 SAPS-II は、最初の 24 時間以内の患者の生理学的状態 (入院の重症度) を反映し、SOFA スコアは機能不全の臓器 (呼吸、凝固、肝臓、心血管、CNS、腎臓) の数を反映します。 SAPS-IIは、通常の臨床検査中に得られた情報とその日の検査結果を使用して、Visit 3およびSOFAで毎日評価されます。 SAPS II および SOFA スコアは、ICU 内のすべての患者の日常的な評価の一部であり、品質指標として使用されます。
微小循環の状態は、臨床徴候 (まだらスコア、毛細血管再充満時間、舌下微小循環の機能状態) と、SaO2-SvO2 ギャップ、Pv-aCO2 ギャップ、動脈乳酸濃度などの組織酸素抽出の程度を反映する検査パラメータを使用して評価されます。 これらのパラメータはすべて、ICU にある血液ガス分析装置によって、それぞれ動脈ラインと中心静脈ラインを介して採取された 1.5 ml の動脈血と静脈血サンプルから提供されます。 これらのカテーテルと血液サンプルは、ICU での重症患者の定期的な血行動態モニタリングと評価の一部です。
患者の生存状態は、研究登録後7日目と28日目に評価されます。 さらに、ICU および病院での死亡率が記録されます。 患者が 28 日目にまだ入院している場合、生存データは医療記録から取得されます。 患者が退院した場合、研究チームは電話で患者に連絡します。 彼に連絡が取れない場合 (文書化された試み)、研究チームは彼の一般開業医に連絡します。 データ保護要件に準拠するために、患者はインフォームド コンセント フォームで一般開業医に連絡することに同意します。 この段階では、生存状況以外の情報は収集されません。 研究のデータ収集は、登録後 28 (± 3 日) 日で終了します。

- 18歳以上の男性および女性の参加者
