Primaire hypothese: De strategie van PCI plus intensieve medische therapie zal superieur zijn aan alleen intensieve medische therapie bij het verminderen van mortaliteit door alle oorzaken of niet-fataal MI bij patiënten met gedocumenteerde myocardischemie die voldoen aan een AHA-taskforce Klasse I-indicatie voor PCI.
Secundaire hypothesen: Vergelijkingen van middelengebruik en kwaliteit van leven en ziekenhuisopname voor acute coronaire syndromen zullen superieur zijn bij PCI plus medische therapie in vergelijking met alleen medische therapie.
Primaire uitkomsten: sterfte door alle oorzaken, niet-fataal MI.
Interventies: Alle patiënten zullen worden behandeld met intensieve medische therapie. Daarnaast krijgt de helft van hen percutane coronaire interventie (PCI).
Studiesamenvatting: De COURAGE-studie is een grootschalige, multicenter, gerandomiseerde, gecontroleerde studie waarin medische therapie wordt vergeleken met PCI plus medische therapie die wordt aangedreven voor "harde" klinische eindpunten. Patiënten die in aanmerking komen voor opname in COURAGE zullen alle proefpersonen omvatten, behalve zeer risicovolle patiënten, en omvatten degenen met chronische angina pectoris (Canadian Cardiovascular Society [CCS] Klasse I-III), recent ongecompliceerd MI en asymptomatisch (of "stil") myocardinfarct. ischemie. Patiënten kunnen coronaire hartziekte in één of meerdere vaten hebben en kunnen eerder een bypassoperatie of PCI hebben ondergaan. We projecteren cumulatieve 3-jarige gebeurtenispercentages van respectievelijk 16,4% en 21%, wat een absoluut verschil oplevert van 4,6% of een relatief verschil van 22%. Met een minimale follow-upduur van 2 1/2 jaar, maximaal 7 jaar, gebruikmakend van een tweezijdige significantietest op het 0,05-niveau, en uitgaande van een cross-overpercentage van 3%, vervolgens 2% en vervolgens 1% elk voor 2 jaar van medicijnen tot PCI, en een jaarlijks verlies tot follow-uppercentage van 1%. Inclusiecriteria:
- Patiënten die in aanmerking komen voor opname in COURAGE zullen alle proefpersonen omvatten, behalve zeer risicovolle patiënten, en omvatten degenen met chronische angina pectoris (Canadian Cardiovascular Society [CCS] Klasse I-III), ongecompliceerd MI, afgekoelde ACS en asymptomatische (of "stille" ") myocardiale ischemie.
- Patiënten kunnen coronaire hartziekte in één of meerdere vaten hebben en kunnen eerder een bypassoperatie of PCI hebben ondergaan.
Het is belangrijk om te benadrukken dat zoveel mogelijk soorten CAD-patiënten - een afspiegeling van het spectrum van patiënten waarmee men in de hedendaagse klinische praktijk wordt geconfronteerd - zullen worden ingeschreven in COURAGE.
