ACHTERGROND:
CHF treft ongeveer vijf miljoen Amerikanen en is de belangrijkste oorzaak van ziekenhuisopname bij personen ouder dan 65 jaar. Hoewel oefentraining verschillende klinische metingen verbetert bij personen met CHF (bijv. piek-VO2, hartslagvariabiliteit en noradrenalinespiegels in het plasma), is het niet bekend of oefentraining de mortaliteit bij personen met CHF vermindert.
ONTWERP VERHAAL:
Deze multicenter gerandomiseerde studie zal bepalen of oefentraining de mortaliteit en ziekenhuisopname vermindert bij personen met matige tot ernstige CHF. Het secundaire doel is om te evalueren of een oefenprogramma dat is ontworpen voor personen met CHF de levenskwaliteit en het functioneren verbetert, economisch voordelig is en medische complicaties voorkomt.
Drieduizend personen met matige tot ernstige CHF zullen willekeurig worden toegewezen aan standaard medische therapie en onderwijs, of standaard medische therapie en onderwijs plus een trainingsprogramma voor oefeningen onder toezicht. De oefentraining omvat 36 gesuperviseerde trainingssessies in de kliniek, gevolgd door oefeningen thuis en periodieke gesuperviseerde sessies voor versterking. Deelnemers die zijn toegewezen aan het oefentrainingsprogramma onder toezicht zullen een loopband of hometrainer gebruiken, die voor hen wordt verstrekt.
Wervingsstatus: Op 9 november 2006 heeft HF-ACTION 2180 proefpersonen ingeschreven en zal de inschrijving eind februari 2007 afronden, met een verwachte inschrijving van ongeveer 2300 proefpersonen.

Beschrijving
Inclusiecriteria:
- LVEF kleiner dan of gelijk aan 35%
- New York Heart Association (NYHA) klasse II, III of IV CHF-diagnose in de 3 maanden voorafgaand aan deelname aan de studie, met een minimum van 6 weken behandeling
- Moet optimale therapie voor hartfalen volgen volgens de richtlijnen voor hartfalen van de American Heart Association (AHA), American College of Cardiology (ACC) en Heart Failure Society of America (HFSA), inclusief behandeling met angiotensine II-converterende enzymremmers (ACEI) en bèta - blocker-therapie, of documentatie hebben die rechtvaardigt waarom optimale therapie niet wordt gebruikt, inclusief intolerantie, contra-indicatie, voorkeur van de deelnemer of het oordeel van een arts
- Moet gedurende 6 weken voorafgaand aan deelname aan de studie stabiele doses medicijnen gebruiken (bijv. Bètablokkers, ACEI en aanvullende medicijnen zoals vermeld in de onderzoeksrichtlijnen)
- Moet in een stabiele medische toestand verkeren en in staat zijn om met een oefenprogramma te beginnen, zoals bepaald door de onderzoeksarts
Uitsluitingscriteria:
- Comorbide ziekte, gedragsbeperkingen of andere beperkingen die de training zouden belemmeren of die de voltooiing van 1 jaar training zouden belemmeren
- Zwanger of van plan zwanger te worden in het jaar na deelname aan de studie
- Ernstige hartgebeurtenis of hartprocedure binnen de 6 weken voorafgaand aan deelname aan de studie
- Hartprocedure of ziekenhuisopname om welke reden dan ook gepland in de toekomst
- Verwacht een harttransplantatie te krijgen in de 6 maanden na deelname aan de studie
- CHF veroorzaakt door significante niet-gecorrigeerde primaire klepaandoening (behalve mitralisinsufficiëntie secundair aan linkerventrikeldisfunctie); als klepvervanging is uitgevoerd, mag gedurende 12 maanden na de procedure niet deelnemen
- CHF veroorzaakt door aangeboren hartziekte of obstructieve cardiomyopathie
- Uitvoeren van oefentraining met regelmatige tussenpozen (meer dan een keer per week) met een matige tot krachtige intensiteit op elk moment in de 6 weken voorafgaand aan de start van het onderzoek
- Resultaten van inspanningstests die veilige training in de weg zouden staan, zoals gedefinieerd door de richtlijnen van de American Association of Cardiovascular and Pulmonary Rehabilitation (AACVPR), waaronder abnormale bloeddrukrespons, vroege ischemische veranderingen en onverwachte levensbedreigende aritmie
- Gebruik van pacemakers met een vaste frequentie, pacemakers die de doelhartfrequentie niet kunnen bereiken of automatische implanteerbare cardioverter-defibrillatorapparaten (AICD) met hartslaglimieten die lager zijn dan de doelhartslag voor inspanningstraining
- Gebruik van een intracardiaal apparaat zoals een implanteerbare cardioverter-defibrillator (ICD) of een pacemaker voor cardiale resynchronisatietherapie in de 6 maanden voorafgaand aan het begin van de studie (moet stabiliteit aantonen gedurende 6 weken na de procedure)
- De primaire arts beschouwt plaatsing van een intracardiaal apparaat zoals een ICD of een pacemaker voor cardiale resynchronisatietherapie als waarschijnlijk binnen 6 maanden na aanvang van het onderzoek; wordt uitgesloten totdat een dergelijk apparaat is geplaatst en er 6 weken stabilisatie zijn verstreken
- Deelname aan een andere klinische studie die de studiedeelname, follow-up of gegevensverzameling kan verstoren, of die cardiovasculaire morbiditeit of mortaliteit kan beïnvloeden
