Lymfoedeem komt met variërende frequentie voor bij patiënten met kanker, maar ongeveer 10-15% van alle borstkankerpatiënten ontwikkelt lymfoedeem na behandeling van borstkanker. Lymfoedeem bij borstkankerpatiënten na okselklierdissectie wordt veroorzaakt door de onderbreking van het oksellymfestelsel door een operatie of bestraling, wat een ophoping van vocht in het onderhuidse weefsel van de arm veroorzaakt, met verminderde uitrekbaarheid van weefsel rond de gewrichten en toegenomen gewicht van de extremiteit. De primaire huidige toegepaste therapie omvat volledige decongestieve therapie (CDT), die het gebruik van manuele lymfedrainage (MLD) en compressiezwachtels (CB) omvat voor het aangedane ledemaat.
De specifieke bijdrage van het vasculaire systeem aan de ontwikkeling van lymfoedeem is onduidelijk. Vasculaire permeabiliteit is een complex proces dat voornamelijk wordt gecontroleerd door de interactie van de ligand vasculaire endotheliale groeifactor (VEGF). Als gevolg van het begrip van de biologie van VEGF en de anekdotische waardering van vrouwen met lymfoedeem die een verbetering van hun lymfoedeem hebben opgemerkt tijdens behandeling met een VEGF-remmer, wordt verondersteld dat de vermindering van de vasculaire permeabiliteit als gevolg van het gebruik van een VEGF-remmer ofwel alleen of in combinatie met standaard decongestieve lymfoedeemtherapie kan de uitkomst voor patiënten met deze postoperatieve complicatie aanzienlijk verbeteren.
Bevacizumab is een recombinant gehumaniseerd monoklonaal antilichaam gericht tegen VEGF. Bevacizumab blokkeert de ontwikkeling van nieuwe bloedvaten bij kanker en is door de FDA goedgekeurd voor de behandeling van darmkanker in combinatie met chemotherapie. Hoewel bevacizumab is toegediend aan duizenden patiënten met kanker, is er slechts beperkte informatie over het gebruik van bevacizumab bij proefpersonen zonder actieve kanker.
In deze studie wordt het veiligheidsprofiel geëvalueerd van stijgende doses bevacizumab die alleen intraveneus worden toegediend gedurende 4 weken, gevolgd door 4 weken therapie in combinatie met manuele lymfedrainage (MLD) en compressiezwachtels (CB) bij patiënten met matig tot ernstig unilateraal lymfoedeem van de bovenste ledematen als gevolg aan eerdere borstkankertherapie.

Beschrijving
Inclusiecriteria:
- Vrouwen met een voorgeschiedenis van borstkankerstatus na (s/p) eerdere chirurgische resectie (d.w.z. ofwel lumpectomie en bestraling, gemodificeerde radicale mastectomie of radicale mastectomie) met lymfoedeem gedefinieerd als een verschil in ledemaatvolume van ten minste 500 ml door perometrische beoordeling
- Lymfoedeem kan nieuw gediagnosticeerd of eerder behandeld zijn zolang het stadium I (pitting) of II (fibrose) is op het moment van aanvang van het onderzoek.
- Geen bekend bewijs van recidiverende of actieve uitgezaaide borstkanker
- Geen eerdere chemotherapie binnen 6 maanden na deelname aan de studie en hersteld tot graad 1 of minder van de toxiciteit van alle eerdere chemotherapie of bestralingstherapie (met uitzondering van alopecia); voortdurende anti-oestrogeentherapie voor postmenopauzale overlevenden is toegestaan.
- Normale eindorgaanfunctie gedefinieerd als: serumcreatinine < 1,5 mg/dl of een berekende creatinineklaring > 50 ml/min; SGOT en SGPT < 2,5 x bovengrens van normaal (ULN); totaal bilirubine < 1,5 X ULN; absoluut aantal neutrofielen (ANC) > 1.500 cellen/µl; hemoglobine > 10 g/dl (zonder transfusies); aantal bloedplaatjes > 100.000/µl; serumalbumine binnen normale grenzen (WNL).
Uitsluitingscriteria:
- Stadium III (lymfostatische olifantiasis) lymfoedeem
- Klinisch bewijs van bilateraal lymfoedeem. Patiënten die een bilaterale borstkankeroperatie of een profylactische borstamputatie van de niet-kankerachtige borst hebben ondergaan, worden uitgesloten.
- Elke voorgeschiedenis van diepe veneuze trombose (DVT) of longembolie (met uitzondering van eerdere lijngerelateerde trombotische gebeurtenissen) of myocardinfarct (MI), cerebrovasculair accident (CVA) of enige andere arteriële trombo-embolische gebeurtenis (d.w.z. voorbijgaande ischemische aanval). TIA], reversibel ischemisch neurologisch tekort [RIND], voorgeschiedenis van angina pectoris, klinisch significante perifere vasculaire ziekte met claudicatio, enz.)
- Patiënten met problemen met wondgenezing (bijv. diabetische ulcera), gastro-intestinale fistels
- Patiënten die therapeutische anticoagulantia krijgen, waaronder een volledige dosis aspirine of niet-steroïde anti-inflammatoire middelen waarvan bekend is dat ze de bloedplaatjesfunctie remmen (een lage dosis coumadine voor poortprofylaxe en een lage dosis aspirine zijn toegestaan)
- Onbehandelde hypertensie met een baseline systolische bloeddruk (SBP) van > 150 mmHg of een diastolische bloeddruk (DBP) > 100 mmHg wordt uitgesloten (stabiele behandelde hypertensie met waarden die lager zijn dan de aangegeven waarden komen in aanmerking).
- Een voorgeschiedenis van infectieuze complicaties van de aangedane arm of die met enige contra-indicatie voor MLD + CB [bijv. congestief hartfalen (CHF), DVT, acuut of chronisch nierfalen] wordt uitgesloten.
- Vrouwen met een voorgeschiedenis van CHF [New York Heart Association (NYHA) klasse II of hoger] worden uitgesloten.
- Zwanger of borstvoeding
- Niet bereid om een geschikte vorm van barrière-anticonceptie te gebruiken voor de duur van het onderzoek en gedurende drie maanden na de laatste dosis bevacizumab
- Die patiënten die actief MLD en/of CB ondergaan op het moment van deelname aan de studie en tot 4 weken voorafgaand aan deelname
- Niet in staat om schriftelijke geïnformeerde toestemming te geven of om te voldoen aan onderzoeksprocedures
- Baseline urine-eiwit: creatinineverhouding > 1,0
- Bekend bewijs van een bloedingsdiathese of coagulopathie
