Diagnostische en therapeutische toepassingen in microarrays bij orgaantransplantatie

Multicentrisch observatieonderzoek om de diagnostische moleculaire kenmerken in de klinische setting van niertransplantaatbiopsies te analyseren
De huidige standaard voor op biopsie gebaseerde diagnoses van disfunctie van niertransplantaties is de Banff-classificatie, die een willekeurige internationale consensus vertegenwoordigt.
Recente gegevensgestuurde benaderingen met behulp van moleculaire en conventionele technologieën geven aan dat louter consensus vaak leidt tot onjuiste diagnoses met potentiële schade voor patiënten als gevolg van ongepaste behandeling.
Om aan deze onvervulde behoefte te voldoen en de diagnostiek op het gebied van orgaantransplantatie te verbeteren, heeft het Alberta Transplant Applied Genomics Centre (ATAGC) een nieuw diagnostisch systeem ontwikkeld dat de moleculaire en histopathologische kenmerken van transplantatiebiopten, plus klinische en laboratoriumparameters, combineert om de eerste geïntegreerd diagnostisch systeem.
De huidige studie zal dit systeem valideren en verfijnen in 500 prospectief niet-geselecteerde biopsieën voor klinische indicaties uit Amerikaanse, Canadese en Europese centra naast de 300 reeds verzamelde biopsieën.
Vanwege een aanzienlijke interesse en steun van deelnemende centra, wordt de studie verder uitgebreid tot 1500 prospectieve biopsieën.
Dit is dus de uitbreiding van de INTERCOM-studie (INTERCOMEX).
Naast het aantonen van de haalbaarheid en waarde van dit systeem in routinematige patiëntenzorg en klinische proeven, zal de studie een transparant en gebruiksvriendelijk rapportageformaat ontwikkelen en optimaliseren om deze informatie aan clinici te communiceren en gedetailleerde feedback te verkrijgen over hoe dit systeem het beste kan verbeteren patiëntenzorg.

De onderzoekspopulatie omvat patiënten met een functionerende niertransplantatie die standaard een biopsie ondergaat voor klinische indicaties om de oorzaak van hun transplantaatdisfunctie vast te stellen (verslechtering van de transplantaatfunctie, vertraagde transplantaatfunctie, proteïnurie).
Beschrijving
Inclusiecriteria:
- Alle ontvangers van een niertransplantatie van ≥18 jaar oud die een nierbiopsie ondergaan voor klinische indicaties, zoals bepaald door hun arts of chirurg, komen in aanmerking voor deelname aan het onderzoek.
Uitsluitingscriteria:
- Patiënten worden uitgesloten van deelname aan het onderzoek als ze weigeren deel te nemen of niet in staat zijn geïnformeerde toestemming te geven.
