Surveillancemonitoring voor ART-toxiciteitsonderzoek bij niet-hiv-geïnfecteerde kinderen geboren uit met hiv geïnfecteerde vrouwen (SMARTT)

Gedetailleerde beschrijving
Veel antiretrovirale therapie (ART) medicijnen die aan een zwangere vrouw worden gegeven, passeren de placenta en kunnen worden gedetecteerd in het vruchtwater en navelstrengbloed, wat resulteert in aanzienlijke blootstelling van de foetus. Daarom bestaat er bezorgdheid over de toxiciteit van de geneesmiddelen bij de foetus en het kind. Het is opmerkelijk dat geen van de momenteel goedgekeurde ART-medicijnen voor de preventie van overdracht van HIV van moeder op foetus in zwangerschapscategorie A van de Food and Drug Administration (FDA) valt (geen risico voor de foetus vastgesteld in adequaat gecontroleerde studies bij mensen). Er is dus een voortdurende behoefte om de toxiciteit van ART bij de preventie van HIV-overdracht te onderzoeken voor de toxiciteit op korte termijn van nieuwere middelen en combinaties, evenals de onbeantwoorde vragen over toxiciteit op langere termijn en subtiele nadelige effecten.
De studie zal een registerbenadering gebruiken om actieve surveillance uit te voeren bij kinderen < 12 jaar bij inschrijving. Het optreden van afwijkingen door blootstelling aan ART in de baarmoeder en/of in de eerste twee levensmaanden zal worden gezocht in meerdere domeinen, waaronder metabolisch, groei, cardiaal, neurologisch, neurologische ontwikkeling, gedrag, taal en gehoor. Klinische en laboratoriumgegevens zullen worden onderzocht op afwijkingen door middel van een hiërarchie van evaluaties: ongewenste voorvallen (AE) zullen worden geïdentificeerd → geselecteerde AE's zullen vooraf gedefinieerde aanvullende evaluaties activeren → significante waarnemingen zullen worden gedefinieerd als gevallen → er zal een patroon van significante studiebrede gevallen worden bepaald gedefinieerd als signalen. De incidentie van deze van belang zijnde gebeurtenissen zal in de loop van de tijd en door middel van een ART-regime worden gevolgd en worden vergeleken met historische gegevens die kunnen wijzen op een signaal. Sommige signalen kunnen worden getest met behulp van bestaande en/of eerder verzamelde gegevens, terwijl andere signalen kunnen wijzen op de behoefte aan aanvullende hypothesegestuurde onderzoeken buiten SMARTT om.
De doelstellingen van SMARTT zijn:
- Om het optreden van potentiële ART-gerelateerde toxiciteiten te schatten door middel van een doorlopend surveillancesysteem onder HIV-niet-geïnfecteerde kinderen van moeders met HIV-infectie met en zonder blootstelling aan ART in utero en/of in de eerste twee levensmaanden en het voorkomen van deze te vergelijken uitkomsten met andere gegevensbronnen en met blootstellingen aan ART; En
- Hypothesegestuurde onderzoeken actief aanmoedigen om te bevestigen dat de signalen het gevolg zijn van blootstelling aan ART in utero en/of in de eerste twee levensmaanden. Merk op dat het volledige ontwerp en de uitvoering van deze studies mogelijk buiten het bereik van de SMARTT-studie valt, maar door SMARTT zal worden gefaciliteerd.
De specifieke doelstellingen van SMARTT zijn:
- Om een Static Surveillance Cohort te creëren om domeinspecifieke gegevensverzameling bij kinderen uit te breiden: 1) eerder ingeschreven in een van de goedgekeurde onderzoeken voor inschrijving in SMARTT; 2) eerder ingeschreven in een andere pediatrische hiv/aids-cohortstudie met goedkeuring van de SMARTT Protocol Chair, of 3) niet eerder ingeschreven in een goedgekeurde studie maar met gelijkwaardige gegevens beschikbaar in het medisch dossier;
- Een Dynamic Surveillance Cohort creëren om domeinspecifieke gegevens te onderzoeken van kinderen die onlangs zijn blootgesteld aan ART in utero en/of in de eerste twee levensmaanden;
- Een jongvolwassenencohort creëren om langetermijnresultaten te bestuderen bij SMARTT-deelnemers die voorheen deelnamen aan de statische en dynamische cohorten.
- Voor elk domein een reeks "triggers" identificeren die een "signaal" van mogelijke ART-toxiciteit definiëren en het optreden van deze signalen vergelijken met eerder verzamelde gegevens en door blootstelling aan ART; En
- Aanmoedigen en faciliteren van de ontwikkeling van hypothesegestuurde onderzoeken om te evalueren of een "signaal" het resultaat is van blootstelling aan ART in utero en/of in de eerste twee levensmaanden.

Inclusiecriteria:
Dynamisch cohort:
- Aan HIV blootgestelde levende foetus langer dan of gelijk aan 23 weken zwangerschap of een levend kind geboren na 22 weken zwangerschap. Baby's die zijn blootgesteld en niet zijn blootgesteld aan ART zullen worden ingeschreven.
- Elk kind dat geboren is uit een met hiv besmette moeder kan worden ingeschreven in afwachting van de vaststelling van de hiv-infectiestatus van het kind. Baby's die hiv-positief blijken te zijn, zullen echter uit de studie worden gehaald en zullen worden doorverwezen voor zorg buiten deze studie om. De HIV-infectiestatus wordt bepaald aan de hand van de Diagnosis of Lack of Infection in HIV-Exposed Children.
- ART-blootstellingsgegevens per trimester van de zwangerschap moeten beschikbaar zijn als ART wordt blootgesteld.
- Binnenkomst voor de geboorte tot een leeftijd van < 72 uur.
- Bereidheid van ouder/wettelijke voogd om kind schriftelijk toestemming te geven voor deelname aan onderzoek.
- Bereidheid van biologische moeder om zich in te schrijven bij eerste inschrijving van haar kind.

De beoordeling van seksueel gedrag en middelengebruik zal worden uitgevoerd met behulp van een Audio Computer Assisted Survey Instrument (ACASI).
ACASI maakt gebruik van computer- en stemopnames zodat de deelnemer elke vraag en antwoordlijst hoort (via koptelefoon) en ziet (op het scherm).
Het is bewezen dat het gebruik van ACASI de responsbias door de aanwezigheid van een interviewer minimaliseert.
