Overvåkingsovervåking for ART toksisitetsstudie i HIV-uinfiserte barn født av HIV-infiserte kvinner (SMARTT)

Mange antiretroviral terapi (ART) medisiner gitt til en gravid kvinne krysser placenta og kan oppdages i fostervannet og navlestrengsblodet, noe som resulterer i betydelig føtal eksponering. Derfor er det bekymring for toksisiteten til stoffene hos fosteret og spedbarnet. Det er bemerkelsesverdig at ingen av de for tiden godkjente ART-medisinene for forebygging av overføring av HIV fra mor til foster er i Food and Drug Administration (FDA) graviditetskategori A (ingen fosterrisiko fastslått i tilstrekkelig kontrollerte studier på mennesker). Derfor er det fortsatt behov for å undersøke toksisiteten til ART i HIV-overføringsforebygging for kortsiktig toksisitet av nyere midler og kombinasjoner, så vel som de ubesvarte spørsmålene om langsiktig toksisitet og subtile bivirkninger.
Studien vil bruke en registertilnærming for å gjennomføre aktiv overvåking blant barn < 12 år ved innskrivning. Forekomster av abnormiteter fra ART-eksponering i utero og/eller i de to første månedene av livet vil bli ettersøkt i flere domener, inkludert metabolsk, vekst, hjerte, nevrologisk, nevroutvikling, atferd, språk og hørsel. Kliniske data og laboratoriedata vil bli undersøkt for abnormiteter gjennom et hierarki av evalueringer: uønskede hendelser (AE) vil bli identifisert → utvalgte AE vil utløse forhåndsdefinerte tilleggsevalueringer → signifikante observasjoner vil bli definert som tilfeller → et mønster av signifikante studieomfattende tilfeller vil bli definert som signaler. Forekomsten av disse hendelsene av interesse vil bli overvåket over tid og ved ART-regime, og sammenlignet med historiske data som kan tyde på et signal. Noen signaler kan testes ved bruk av eksisterende og/eller tidligere innsamlede data, mens andre signaler kan indikere behovet for ytterligere hypotesedrevne studier utenfor SMARTT.
Målene til SMARTT er:
- Å estimere forekomsten av potensielle ART-relaterte toksisiteter gjennom et pågående overvåkingssystem blant HIV-uinfiserte barn født av mødre med HIV-infeksjon med og uten eksponering for ART in utero og/eller i de to første levemånedene og sammenligne forekomsten av disse resultater med andre datakilder så vel som ved ART-eksponeringer; og
- Å aktivt oppmuntre til hypotesedrevne studier for å bekrefte at signalene skyldes ART-eksponering in utero og/eller i de to første månedene av livet. Vær oppmerksom på at den fullstendige utformingen og gjennomføringen av disse studiene kan ligge utenfor SMARTT-studien, men vil bli tilrettelagt av SMARTT.
De spesifikke målene til SMARTT er:
- For å opprette en statisk overvåkingskohort for å utvide domenespesifikk datainnsamling hos barn, enten 1) som tidligere er registrert i noen av de godkjente studiene for registrering i SMARTT; 2) tidligere registrert i en annen pediatrisk HIV/AIDS-kohortstudie med SMARTT Protocol Chair-godkjenning, eller 3) ikke tidligere registrert i en godkjent studie, men med tilsvarende data tilgjengelig i journalen;
- Å lage en dynamisk overvåkingskohort for å undersøke domenespesifikke data fra barn som nylig er eksponert for ART in utero og/eller i de to første månedene av livet;
- Å opprette en ungdomskohort for å studere langsiktige resultater i SMARTT-deltakere som tidligere var registrert i de statiske og dynamiske kohortene.
- Å identifisere et sett med "triggere" for hvert domene som definerer et "signal" om mulig ART-toksisitet og sammenligne forekomsten av disse signalene med tidligere innsamlede data og ved ART-eksponering; og
- Å oppmuntre og lette utviklingen av hypotesedrevne studier for å evaluere om et "signal" er et resultat av ART-eksponering i utero og/eller i de to første månedene av livet.

Inklusjonskriterier:
Dynamisk kohort:
- HIV-eksponert levende foster større enn eller lik 23 ukers svangerskap eller et levende spedbarn født etter 22 ukers svangerskap. Spedbarn eksponert og ueksponert for ART vil bli registrert.
- Ethvert spedbarn født av en HIV-infisert mor kan bli registrert i påvente av fastsettelse av spedbarnets HIV-infeksjonsstatus. Spedbarn som er funnet å være HIV-positive vil imidlertid bli avbrutt fra studien og vil bli henvist til behandling utenfor denne studien. HIV-infeksjonsstatus vil bli bestemt ved hjelp av diagnosen mangel på infeksjon hos HIV-eksponerte barn.
- ART eksponeringsdata etter trimester av svangerskapet må være tilgjengelig hvis ART eksponert.
- Innreise før fødsel til og med < 72 timers alder.
- Forelders/foresattes vilje til å gi skriftlig tillatelse til at barnet kan delta i studien.
- Biologisk mors vilje til å melde seg inn ved første innmelding av barnet sitt.
