Overvågningsovervågning for ART toksicitetsundersøgelse i HIV-uinficerede børn født af HIV-inficerede kvinder (SMARTT)

Mange antiretroviral terapi (ART) medicin givet til en gravid kvinde krydser placenta og kan påvises i fostervandet og navlestrengsblodet, hvilket resulterer i betydelig føtal eksponering. Derfor er der bekymring for stoffernes toksicitet hos fosteret og spædbarnet. Det er bemærkelsesværdigt, at ingen af de aktuelt godkendte ART-medicin til forebyggelse af moder til føtal overførsel af HIV er i Food and Drug Administration (FDA) Graviditetskategori A (ingen føtal risiko konstateret i tilstrækkeligt kontrollerede humane undersøgelser). Der er således fortsat behov for at undersøge toksiciteten af ART i HIV-transmissionsforebyggelse for den kortsigtede toksicitet af nyere midler og kombinationer såvel som de ubesvarede spørgsmål om langsigtet toksicitet og subtile bivirkninger.
Undersøgelsen vil bruge en registertilgang til at udføre aktiv overvågning blandt børn under 12 år ved indskrivning. Forekomster af abnormiteter fra ART-eksponering i utero og/eller i de første to måneder af livet vil blive eftersøgt i flere områder, herunder metaboliske, vækst-, hjerte-, neurologiske, neuro-udviklingsmæssige, adfærd, sprog og hørelse. Kliniske og laboratoriedata vil blive undersøgt for abnormiteter gennem et hierarki af evalueringer: uønskede hændelser (AE) vil blive identificeret → udvalgte AE'er vil udløse foruddefinerede yderligere evalueringer → signifikante observationer vil blive defineret som tilfælde → et mønster af signifikante undersøgelsesdækkende tilfælde vil blive defineret som signaler. Forekomsten af disse hændelser af interesse vil blive overvåget over tid og ved ART-regimen og sammenlignet med historiske data, der kan tyde på et signal. Nogle signaler kan testes ved hjælp af eksisterende og/eller tidligere indsamlede data, mens andre signaler kan indikere behovet for yderligere hypotese-drevne undersøgelser uden for SMARTT.
Målene for SMARTT er:
- At vurdere forekomsten af potentielle ART-relaterede toksiciteter gennem et løbende overvågningssystem blandt HIV-uinficerede børn født af mødre med HIV-infektion med og uden eksponering for ART in utero og/eller i de første to måneder af livet og sammenligne forekomsten af disse resultater med andre datakilder såvel som ved ART-eksponeringer; og
- At aktivt tilskynde til hypotesedrevne undersøgelser for at bekræfte, at signalerne skyldes ART-eksponering in utero og/eller i de første to måneder af livet. Bemærk, at det fulde design og udførelse af disse undersøgelser kan ligge uden for SMARTT-undersøgelsens omfang, men vil blive faciliteret af SMARTT.
De specifikke mål for SMARTT er:
- For at oprette en statisk overvågningskohorte for at udvide domænespecifik dataindsamling hos børn enten 1) tidligere tilmeldt en af de godkendte undersøgelser til tilmelding til SMARTT; 2) tidligere tilmeldt et andet pædiatrisk HIV/AIDS-kohortestudie med SMARTT Protocol Chair-godkendelse, eller 3) ikke tidligere tilmeldt en godkendt undersøgelse, men med tilsvarende data tilgængelige i journalen;
- At oprette en dynamisk overvågningskohorte til at undersøge domænespecifikke data fra børn, der nyligt er blevet eksponeret for ART in utero og/eller i de første to måneder af livet;
- At oprette en ungdomskohorte for at studere langsigtede resultater hos SMARTT-deltagere, der tidligere var tilmeldt de statiske og dynamiske kohorter.
- At identificere et sæt "triggere" for hvert domæne, der definerer et "signal" for mulig ART-toksicitet og sammenligne forekomsten af disse signaler med tidligere indsamlede data og ved ART-eksponering; og
- At tilskynde til og lette udviklingen af hypotese-drevne undersøgelser for at evaluere, om et "signal" er resultatet af ART-eksponering in utero og/eller i de første to måneder af livet.

Inklusionskriterier:
Dynamisk kohorte:
- HIV-eksponeret levende foster større end eller lig med 23 ugers svangerskab eller et levende spædbarn født efter 22 ugers svangerskab. Spædbørn, der er udsat for og ueksponeret for ART, vil blive tilmeldt.
- Ethvert spædbarn født af en HIV-inficeret mor kan tilmeldes, indtil spædbarnets HIV-infektionsstatus er fastslået. Spædbørn, der viser sig at være HIV-positive, vil dog blive afbrudt fra undersøgelsen og vil blive henvist til pleje uden for denne undersøgelse. HIV-infektionsstatus vil blive bestemt ved hjælp af diagnosticering af manglende infektion hos HIV-eksponerede børn.
- ART eksponeringsdata pr. trimester af graviditeten skal være tilgængelige, hvis ART eksponeres.
- Indgang før fødslen til og med < 72 timers alderen.
- Forælder/værges vilje til at give skriftlig tilladelse til, at barnet kan deltage i undersøgelsen.
- Den biologiske mors vilje til at tilmelde sig ved den første indskrivning af sit barn.
