In onze studie omvatten de onderzoekers prospectief patiënten met darmkanker die een electieve chirurgische ingreep nodig hebben. Alle soorten colonresectie werden uitgevoerd volgens standaardprocedures. Richtlijnen voor preoperatief onderzoek volgens NGICG (Norwegian Gastro-Intestinal Cancer Group) om de ziekte te beoordelen, de meest geschikte patiënten te selecteren en verdere behandeling te plannen.
Microcirculatie van de dikke darmwand werd beoordeeld door dynamische laser-geïnduceerde-fluorescentie-video-angiografie (IC-VIEW, PULSION Medical Systems AG, München, Duitsland) van indocyaninegroen (ICG). ICG is een in water oplosbare tricarbocyaninekleurstof die na intraveneuze injectie sterk aan plasma-eiwitten bindt en uitsluitend in de intravasculaire ruimte wordt gedistribueerd. Bovendien bevat dit systeem een laser (energie Pi = 0,16 W, golflengte = 780 nm) die excitatie/verlichting veroorzaakt van het fluorescentielicht van intravasculair plasmagebonden ICG. Dit licht heeft een spectraal bereik nabij-infraroodenergie (NIR) met een maximum bij 805 nm en zendt fluorescentie uit bij 835 nm. Het passeert een infraroodfilter op een digitale videocamera en resulteert in het opnemen van real-time fluorescerende beelden van perfusie van plasmagebonden ICG in de kleine plexus van bloedvaten in de darmwand. Daarnaast demonstreert het ook perfusie van omliggende structuren zoals appendix epiploic en pericolic vet. De maximale penetratie van de laser in weefsel is 3-5 mm en de algemene normale dikte van de colonwand is 3-5 mm. Deze methode maakt het een representatieve tracer voor weefselperfusie van de voorste darmwand.

Beschrijving
Inclusiecriteria:
- Patiënten met darmkanker die een electieve chirurgische ingreep nodig hebben.
Uitsluitingscriteria:
- Zwangerschap
- Kinderen of patiënten jonger dan 18 jaar
- Gevorderd nier- of leverfalen
- Eerdere allergische reacties op ICG en jodide
- Chronische bloedarmoede
- Actieve hematologische aandoening
- Vrouwen in de vruchtbare leeftijd hadden een negatieve zwangerschapstest nodig om opgenomen te worden
- Patiënten met eerdere colectomie of anorectale chirurgie werden ook uitgesloten vanwege anatomisch letsel op de mesenteriale circulatie

In ons wetenschappelijk experiment evalueren we de microcirculatie in de dikke darmwand en anastomose met lasergeïnduceerde fluorescentievideo-angiografie van ICG.
Door deze techniek te implementeren, kunnen we de gemiddelde pixelintensiteit van het fluorescentiesignaal meten in het interessegebied van de darm, wat de weefselperfusie in dit respectieve deel van de darmwand vertegenwoordigt.
