Volgens de recente resultaten over de uitkomst van een geëscaleerd daunorubicine-gebaseerd protocol voor volwassen ALL dat is uitgevoerd door 'Adult ALL Working Party of the Korean Society of Hematology' (gegevens werden aangekondigd op de jaarlijkse bijeenkomst van ASCO in 2010, Chicago, IL), Het CR-percentage van 90,6% was bevredigend, maar de 2-jaars / 3-jaars ziektevrije overleving (DFS) was teleurstellend (respectievelijk 43,6% en 39,9%), wat betekent dat de adequate post-remissietherapie om minimale restziekte onder controle te krijgen na het bereiken van CR is erg belangrijk om de uitkomst van volwassen ALL te verbeteren.
Allogene hematopoëtische celtransplantatie (AlloHCT) wordt aanbevolen als post-remissietherapie voor patiënten met volwassen ALL, en er is geprobeerd om de TRM-frequentie te verlagen door conditionering met verminderde intensiteit. Veel patiënten kregen echter consolidatiechemotherapie in plaats van alloHCT vanwege de afwezigheid van een HLA-gematchte donor, leeftijdsbeperking en gecombineerde comorbiditeiten (van de 190 patiënten die in ons eerder genoemde onderzoek waren opgenomen, ontving slechts 52,3% alloHCT).
Recentelijk lijkt de stagnatie in de behandeling van volwassen ALL te zijn bereikt, mogelijk als gevolg van een grens voor verdere intensivering van de chemotherapeutische dosis. Dosis-escalatiestrategie heeft veel problemen bij de acceptatie voor de behandeling van volwassen ALL in termen van verhoogde morbiditeit en mortaliteit, om nog maar te zwijgen van de werkzaamheid van dergelijke strategieën. Nieuwe, bij voorkeur niet-chemotherapeutische benaderingen (misschien gerichte therapie) zijn daarom dringend nodig.
Voor Ph(+) ALL heeft de introductie van BCR/ABL-tyrosinekinaseremmer het behandelingsresultaat verbeterd met aanvaardbare toxiciteiten. Het toepassen van een vergelijkbare strategie op Ph(-) ALL, gericht op leukemie-oppervlakte-antigenen met monoklonale antilichamen, is een andere veelbelovende strategie.
Het is bekend dat CD20-expressie van ten minste 20% wordt aangetroffen bij 22-48% van pre-B ALL, en lijkt verband te houden met een slechte prognose, hoewel er controverses zijn bij pediatrische patiënten. Gebaseerd op de significante verbetering van het resultaat bij B-cel NHL, suggereren voorlopige gegevens met betrekking tot het gebruik van rituximab in eerstelijnstherapie voor CD20-positieve precursor B-cel ALL dat het gebruik ervan gunstig kan zijn. Vooral monoklonale antilichamen worden verondersteld effectiever te zijn in combinatie met chemotherapie en behandeld in de staat van minimale resterende ziekte, wat suggereert dat het interessant is om rituximab in combinatie met de huidige chemotherapie van volwassen ALL te evalueren. Recente gegevens over de werkzaamheid van rituximab-gecombineerde chemotherapie toonden aan dat de CR- en OS-percentages superieur waren met de aangepaste hyperCVAD- en rituximab-regimes in vergelijking met standaard hyper-CVAD (70% versus 38%, p<0,001) bij jongere (leeftijd < 60 jaar). ) CD20-positieve subset, hoewel het een analyse was van verschillende patiëntengroepen die met verschillende regimes werden behandeld5.

Beschrijving
Inclusiecriteria:
- Patiënten die eerder onbehandeld waren en die ALL of hoog-risico lymfoblastisch lymfoom hadden.
- Patiënten bij wie de leukemieblastcellen ≥20% van de CD20-antigenen tot expressie brengen op het moment van diagnose
- Geen eerdere chemotherapie voor leukemie (gebruik van hydroxyurea of leukaferese is toegestaan.)
- Geschatte levensverwachting van meer dan 3 maanden
- ECOG-prestatiestatus van 2 of lager, Karnofsky-schaal > 60
- Adequate hartfunctie (EF>45%) op echocardiogram of hartscan (MUGA-scan)
- 15 jaar en ouder.
- Adequate nierfunctie (creatinine <1,5 mg/dL)
- Adequate leverfunctie. (Bilirubine <1,5 mg/dL, transaminasenwaarden<3 keer de bovengrens van de normaalwaarde [5 keer voor patiënten met levermetastasen of hepatomegalie]). Zelfs als het initiële niveau de bovengrenzen overschrijdt, zal de patiënt acceptabel zijn als de niveaus op dag 8 voldoen aan de inclusiecriteria.
- Alle patiënten gaven schriftelijke geïnformeerde toestemming volgens de richtlijnen van de commissie voor menselijk onderzoek van elke instelling.
Uitsluitingscriteria:
- Acute biphenotypische leukemie, acute biklonale leukemie of acute gemengde leukemie
- Aanwezigheid van significante ongecontroleerde actieve infectie
- Aanwezigheid van ongecontroleerde bloeding
- Elke naast elkaar bestaande ernstige ziekte of orgaanfalen
- Patiënten met een psychiatrische stoornis of een ernstige verstandelijke beperking die therapietrouw onmogelijk maakt en geïnformeerde toestemming onmogelijk maakt
- Vrouwen die borstvoeding geven, zwangere vrouwen, vrouwen in de vruchtbare leeftijd die geen adequate anticonceptie willen
- Patiënten met een diagnose van eerdere maligniteit, tenzij ziektevrij gedurende ten minste 5 jaar na therapie met curatieve intentie (behalve curatief behandelde niet-melanome huidkanker, in situ carcinoom of cervicale intra-epitheliale neoplasie)
