Studie van de respons op een neoadjuvante chemotherapie op basis van de antitumorale immuunrespons bij gelokaliseerde borstkanker (BREAST IMMUN)
Dit is een prospectieve, niet-gerandomiseerde studie die tot doel heeft de respons op een neoadjuvante chemotherapie te evalueren volgens de antitumor immuunrespons bij gelokaliseerde borstkanker.
40 patiënten zullen in de studie worden opgenomen. Ze krijgen chemotherapie: 3 of 4 antracyclinecycli of 3 of 4 FEC-100-cycli gevolgd door 3 of 4 taxaancycli.
Trastuzumab wordt toegevoegd aan taxaan voor HER2+/Neu+ patiënten. Vervolgens worden patiënten geopereerd en krijgen ze een adjuvante behandeling, die beide afhangen van de waardering van de onderzoeker.
Er wordt bloed afgenomen op de eerste dag van de eerste chemotherapiecyclus, op de eerste dag van de derde cyclus, bij de operatie, 6 maanden na de operatie en in geval van recidief.
Tumormonster zal zoveel mogelijk worden verzameld bij diagnose en bij operatie.
Patiënten worden gedurende 5 jaar gevolgd.

Beschrijving
Inclusiecriteria:
- Histologisch bewezen gelokaliseerde borstkanker vereiste anthracycline-chemotherapie +/- trastuzumab vóór de operatie
- Leeftijd > 18 jaar
- Chemotherapie met 3 antracyclinecycli om te beginnen (doxorubicine of epirubicine)
- Elke eerdere behandeling voor deze kanker
- Prestatiestatus <= 1
- Overeenkomst voor de bewaring van biologische monsters
- Gedekt door een medische verzekering
- Ondertekend schriftelijk toestemmingsformulier
- Beschikbaarheid van tumormonster verzameld bij diagnose
Uitsluitingscriteria:
- Eerdere operatie voor borstkanker
- Al onder chemotherapie voor het eerste bloedmonster
- Eerdere antitumorale behandeling
- Onder immunosuppressieve behandeling
- Onder corticoïden gedurende de 15 dagen vóór inschrijving
- Voorgeschiedenis van gelijktijdige kanker, behalve als deze gedurende ten minste 5 jaar is genezen
- Geschiedenis van lymfoom of borstsarcoom
- Voorgeschiedenis van chronische ontstekingsziekte of auto-immuunziekte, hepatitis B of C of immuundisfunctieziekte (waaronder hiv-positieve aids) bekend
- Geschiedenis van een andere ziekte die niet overeenkomt met deze studie
- Vrijheidsberoving door rechtbank of administratieve beslissing
- Zwangere vrouwen of vrouwen die borstvoeding geven of die geen gebruik maken van effectieve anticonceptiemethoden voor vrouwen in de vruchtbare leeftijd
