Intrathecaal magnesium voor chirurgie op dezelfde dag (MAGLIDIT)
Intrathecaal magnesium geassocieerd met lidocaïne vermindert de incidentie van het voorbijgaande neurologische syndroom na een operatie op dezelfde dag
5% hyperbare lidocaïne heeft zijn nut voor anesthesie op dezelfde dag (SDS) verloren omdat het verantwoordelijk wordt gehouden voor het zogenaamde voorbijgaande neurologische syndroom (TNS). Deze entiteit komt met name voor bij patiënten die geopereerd zijn aan een lithotomie en knie-artroscopiepositie en bij obese patiënten. Het is een goedaardige, matig pijnlijke (graad 3-4 van de 10) en zelfbeperkend in de tijd, maar verontrustend genoeg om te worden vermeden in gevallen van chirurgie op dezelfde dag. Andere lokale anesthetica zijn niet concurrerend met algemene anesthesie op tijd om uit de SDS-eenheid naar huis te worden ontslagen. Het magnesiumion staat bekend om zijn beschermende eigenschappen op cellen met elektrische activiteit. Het doel is om te bevestigen dat het magnesiumion dat als adjuvans aan intrathecale (IT) lidocaïne wordt toegevoegd, de incidentie van TNS kan tegenwerken. Als deze hypothese zou kunnen worden bevestigd, zouden de praktische en theoretische consequenties verstrekkend zijn.
De methode om ons doel te bereiken zou een dubbelblinde, gerandomiseerde klinische studie zijn waarbij twee groepen intrathecale lidocaïne worden overwogen: met en zonder toegevoegd IT-magnesium.

Beschrijving
Inclusiecriteria:
- Patiënten die dezelfde dag worden geopereerd
- Geopereerd in lithotomie- of artroscopiepositie of zwaarlijvige patiënten
Uitsluitingscriteria:
- American Society of Anesthesia 3 of 4 patiënten
- Jonger dan 18 of ouder dan 80 jaar
- Actieve lumbale pijn
- Een tekort aan chronische neurologische ziekte
- Een tekort aan myopathie
- Zwangere vrouw
- Voorgeschiedenis van allergische of toxische reactie op lidocaïne, magnesium of fentanyl

Voorbijgaand neurologisch syndroom komt met name voor bij patiënten die geopereerd zijn aan lithotomie en artroscopie van de knie en bij obese patiënten.
Het is een goedaardige, matig pijnlijke (graad 3-4 van de 10) bil-benenpijn, zelfbeperkend in de tijd, maar storend genoeg om te worden vermeden in gevallen van een operatie op dezelfde dag.
