Remodellering van harttransplantaten en effecten van Sirolimus op de voortgang ervan
Transplantaatremodellering van het hart veroorzaakt een slechte kwaliteit van leven, falen van het transplantaat en verhoogde mortaliteit na harttransplantatie. Risicofactoren voor remodellering van cardiale allograften en de progressie ervan zijn slecht gedefinieerd en er is behoefte aan effectieve interventies. Dit is een multifactorieel fenomeen, geassocieerd met verschillende immunologische en niet-immunologische factoren. Dierstudies suggereren dat M-TOR-remming de hermodellering van het harttransplantaat verzwakt, secundair aan neerwaartse regulatie van M-TOR stroomafwaartse doelen en verhoogde autofagie. Er is een gebrek aan gegevens over het effect van Sirolimus, een M-TOR-remmer, op de remodellering van het menselijk hart.
Dit doel van het voorstel is om de prevalentie van remodellering van harttransplantaten op de huidige immunosuppressieve strategieën te identificeren en risicofactoren voor de ontwikkeling ervan te bepalen. Het zal ook moleculaire routes identificeren die verband houden met remodellering van harttransplantaten en de impact van Sirolimus op deze routes bepalen.

Harttransplantatie is een gevestigde behandelingsoptie geworden voor patiënten met hartaandoeningen in het eindstadium en heeft momenteel een overlevingspercentage van 90% na één jaar, een overlevingspercentage van vijf jaar van 70% en een overlevingspercentage van 10 jaar van 50% . De introductie van anti-afstotingsbehandelingen dertig jaar geleden met geneesmiddelen die bekend staan als calcineurineremmers hebben geresulteerd in een significante verbetering van de overleving van harttransplantatiepatiënten. Het grootste deel van deze verbetering vindt echter plaats tijdens het eerste jaar na transplantatie. Na het eerste jaar is het sterftecijfer van ontvangers van een harttransplantatie niet veranderd, wat aangeeft dat de oorzaken van late complicaties de afgelopen drie decennia niet zijn aangetast door verbeteringen in de zorg na de transplantatie. Het wordt duidelijk dat om de late uitkomsten te verbeteren, de focus in het harttransplantatieonderzoek moet worden verlegd naar de preventie en behandeling van late complicaties.
Cardiac allograft remodeling (CAR), of veranderingen in het geometrische patroon van het hart, is een van de meest voorkomende complicaties na harttransplantatie en veroorzaakt vaak een slechte kwaliteit van leven, hartfalen en verminderde overleving. De risicofactoren en mechanismen voor de ontwikkeling en progressie van CAR zijn slecht gedefinieerd en er is geen effectieve behandeling voor deze aandoening. In de voorgestelde studie zullen we de prevalentie, risicofactoren en het effect van CAR op de fysieke capaciteit, hart- en vaatziekten en de overleving van patiënten na een harttransplantatie in kaart brengen. Voor de beoordeling van de hartgeometrie zullen we cardiale magnetische resonantiebeeldvorming (CMRI) gebruiken, een techniek die wordt gebruikt om de interne structuren van het lichaam in detail te visualiseren. CMRI wordt beschouwd als een "gouden standaard" voor het evalueren van de structuur en functie van het hart. We zullen ook de moleculaire en genetische markers evalueren die geassocieerd zijn met de ontwikkeling en progressie van CAR na harttransplantatie.
Het medicijn Sirolimus, een nieuw middel tegen afstoting, kan na harttransplantatie worden gebruikt in plaats van calcineurineremmers. Recente experimentele en dierstudies geven aan dat Sirolimus de veranderingen in de geometrie van het hart na een transplantatie (d.w.z. CAR) kan verzachten en de hartfunctie kan verbeteren. We zullen het effect van Sirolimus op CAR bij mensen beoordelen en moleculaire en genetische markers evalueren die met dit effect geassocieerd zijn.
Het is ons doel om een belangrijk inzicht te geven in de aard van CAR na harttransplantatie en de reactie op het nieuwe anti-afstotingsmedicijn Sirolimus. Deze informatie zal een significante impact hebben op de behandeling van ontvangers van een harttransplantatie en zal zo de kwaliteit van leven verbeteren en de overleving na een harttransplantatie verlengen.

- Alle volwassen ontvangers van een harttransplantatie die een harttransplantatie ondergaan bij UNMC/TNMC.
Uitsluitingscriteria:
- Volwassen harttransplantatiepatiënten met acute afstoting (ISHLT R> graad 2) of acute infectie.

Actieve vergelijker: Cardiale biopsie C4D-kleuring
Een procedure waarbij een heel klein stukje van uw hartspier wordt verwijderd, zodat het in het laboratorium kan worden beoordeeld.
Deze procedure kan worden uitgevoerd om de oorzaak van cardiale myopathie (een verzwakte hartspier) vast te stellen of om te controleren op afstoting na een harttransplantatie.

Actieve vergelijker: Cardiale MRI
Cardiale Magnetic Resonance Imaging (MRI) produceert geen bijwerkingen van de magnetische velden en radiogolven en brengt geen risico op kanker of geboorteafwijkingen met zich mee.
Ernstige reacties op de speciale contrastkleurstoffen die voor MRI worden gebruikt, zijn zeer zeldzaam.
Het MRI-onderzoek vormt bijna geen risico voor de gemiddelde patiënt wanneer de juiste veiligheidsrichtlijnen worden gevolgd, maar bijwerkingen zijn mogelijk en omvatten hoofdpijn, misselijkheid, duizeligheid, verandering in smaak en allergische reactie.
Dergelijke reacties zijn meestal mild en gemakkelijk onder controle te houden met medicatie.

Experimenteel: MTor immunosuppressie
Sirolimus De dosering van Sirolimus is gebaseerd op de bloedspiegels. Om het potentieel van mTOR-immunosuppressivum Sirolimus te beoordelen bij de verzwakking van CAR bij HTx-ontvangers en daardoor de reeds bestaande harttransplantaatfunctie, vasculopathie en inspanningscapaciteit te verbeteren. |
