De Oncopanel Pilot (TOP)-studie
Het BCCA Oncopanel is een klinische test die wordt ontwikkeld om de genotypestatus van een prospectief gedefinieerde reeks genen te bepalen.
Het doel van deze pilotstudie is om de haalbaarheid en het effect op de klinische besluitvorming van de Oncopanel-test te beoordelen. In aanmerking komende patiënten zijn patiënten met gevorderde long-, colorectale, melanoom- en GIST-kankers en patiënten met gediagnosticeerde maligniteiten die in aanmerking komen voor klinische onderzoeken.

Somatische mutaties in solide tumoren vertegenwoordigen een gevestigde manier om maligniteiten te karakteriseren voor prognostische, diagnostische en therapeutische doeleinden. Mutaties in EGFR-, KRAS-, BRAF- en KIT- en PDGFRA-genen sturen therapie bij patiënten met respectievelijk gevorderde long-, colorectale, melanoom- en GIST-tumoren. Bekende of nieuwe mutaties in andere genen kunnen ook van klinisch belang zijn, maar worden niet geïdentificeerd door de huidige genotypering die wordt aangeboden aan BC Cancer Agency (BCCA)-patiënten. Bovendien zijn talrijke kandidaat-genen betrokken als potentiële prognostische en voorspellende biomarkers bij patiënten met solide tumoren. Als zodanig is het Oncopanel een klinische test die wordt ontwikkeld om de genotypestatus van een prospectief gedefinieerde reeks genen te bepalen. De volgende klinisch relevante set genen en exons zijn opgenomen in het Oncpanel: KRAS, EGFR, BRAF, NRAS en HRAS, PIK3CA Signal Transduction Pathway Genes, RAS-RAF-MEK-MAPK Pathway, HER2, IDH1 and IDH2, ALK, TP53, c-KIT, STAT1&3 en PDGFRA. Aanvullende tests op het tumormateriaal omvatten ook analyse van specifieke genvarianten die verband houden met bijwerkingen of respons op therapie.
Talrijke studies hebben de aanwezigheid van circulerend tumor-DNA (ctDNA) gedocumenteerd bij patiënten met gevorderde en vroege maligniteiten (20-22). Het vermogen om standaard kankermutaties te diagnosticeren met een op bloed gebaseerde test (een "vloeibare biopsie") is nog niet vastgesteld, maar biedt duidelijke voordelen. Het ontstaan van "resistentie"-mutaties die ontstaan in de metastatische tumor of tijdens de therapie is goed gedocumenteerd (21, 22). Een bloedbiopsie kan een nauwkeurigere bepaling van de genetische kenmerken van de tumor vertegenwoordigen dan een DNA-monster uit een archief. Adequate weefselspecimens kunnen moeilijk te verkrijgen zijn van sommige patiënten met gediagnosticeerde maligniteiten, met name longkanker. Een bloedbiopsie kan een minder invasieve en snellere manier zijn om zowel standaard als translationele kankermutaties te diagnosticeren.

Beschrijving
Patiënten met gearchiveerd tumorweefsel en een bekende voorgeschiedenis van invasieve maligniteiten komen in aanmerking als ze aan een of meer van de volgende criteria voldoen:
- Gevorderde dikkedarmkanker en komt in aanmerking voor standaard KRAS-testen,
- Gevorderde niet-kleincellige longkanker en komt in aanmerking voor standaard EGFR-testen,
- Gevorderd melanoom en in aanmerking komend voor standaard BRAF-testen,
- Gastro-intestinale stromale tumoren (GIST's) die in aanmerking komen voor standaard c-KIT- en PDGFRA-testen,
- Wordt overwogen om mogelijk in aanmerking te komen voor klinische proeven.
