Studie van gerealiseerde kosten na percutane coronaire interventie met ontslag op dezelfde dag (SOCRATES)
Een gerandomiseerde kostenminimaliseringsanalyse waarbij ontslag op dezelfde dag wordt vergeleken met een overnachting in het ziekenhuis na electieve en urgente percutane coronaire interventie met een laag risico.

Gedetailleerde beschrijving
Dit is een prospectief gerandomiseerd klinisch onderzoek van ontslag op dezelfde dag versus overnachting voor patiënten die een ongecompliceerde electieve of urgente PCI met een laag risico ondergaan met ongecompliceerd post-PCI-herstel. Het resultaat is de totale kosten die door patiënten zijn gemaakt vanaf het moment van presentatie in het indexziekenhuis tot en met 30 dagen post-PCI-follow-up.
Het uitvoeren van PCI-procedures en het verlenen van post-PCI-zorg zal in overeenstemming zijn met de routinematige institutionele praktijken en zal niet protocolgestuurd zijn.
Toegestane patiënten worden toegewezen aan SDD- of ON-verblijf met behulp van een blokrandomisatieschema. Randomisatie vindt plaats zodra de patiënt een ongecompliceerde post-PCI-observatieperiode van 6 uur heeft doorlopen.

Beschrijving
Inclusiecriteria:
- Opeenvolgende patiënten die electief of een laag risico ondergaan (d.w.z. in de setting van onstabiele angina pectoris maar geen myocardinfarct zonder ST-segmentelevatie) urgente PCI.
- Geen sociodemografische factoren die SDD zouden uitsluiten (beperkende factoren zijn onder meer geen familielid/vriend beschikbaar om bij de patiënt te blijven tot het telefoontje van de verpleegkundige de volgende dag, geen werkende telefoon, geen betrouwbaar vervoer, thuis > 60 minuten van medische infrastructuur, niet in staat om te verkrijgen/betalen voor medicijnen).
- PCI-procedures uitgevoerd tussen 04/08/14 en 03/08/16.
- Kortetermijnrisico op overlijden in het ziekenhuis, transfusie of contrast-geïnduceerde nefropathie tijdens de ziekenhuisopname van de index op of onder 1% met behulp van Blue Cross Blue Shield of Michigan Cardiovascular Consortium (BMC2) PCI-registerrisicovoorspellingsmodellen.
- Geen complicaties in het laboratorium (langdurige angina pectoris, acute sluiting, geen reflow, significante of onbehandelde dissectie, vaatperforatie, occlusie van zijtak, gebruik van reddingsglycoproteïne IIb/IIIa-remmer, ventriculaire tachycardie of fibrillatie die cardioversie/defibrillatie vereist, tamponnade, longoedeem, beroerte of voorbijgaande ischemische aanval, shock).
- Contrastvolume/berekende creatinineklaringsverhouding < 3
- Geen terugkerende pijn op de borst, kortademigheid, hemodynamische instabiliteit, bloeding of vasculaire complicaties gedurende 6 uur in herstelgebied na PCI
Uitsluitingscriteria:
- Gebruik van roterende atherectomie
- Gebruik van een glycoproteïne IIb/IIIa-remmerinfusie na PCI

Experimenteel: Zelfde dag ontslag
Patiënten die zowel een ongecompliceerde PCI als een ongecompliceerde observatieperiode van 6 uur doormaakten, worden willekeurig toegewezen aan ontslag op dezelfde dag.

Patiënten die zijn gerandomiseerd naar SDD, worden op de dag van hun PCI naar huis ontslagen, ten minste 6 uur nadat deze is voltooid.
Ze krijgen de volgende dag en na 30 dagen een vervolgtelefoontje van een studiecoördinator om na te gaan welke kosten (indien van toepassing) zijn gemaakt en of er nadelige gevolgen zijn opgetreden.

Patiënten die een ongecompliceerde PCI doormaakten, evenals een ongecompliceerde observatieperiode van 6 uur, zullen willekeurig worden toegewezen aan een overnachting, die over het algemeen als standaardzorg wordt beschouwd.

Patiënten die gerandomiseerd zijn om te overnachten, worden de volgende ochtend ontslagen om na 30 dagen een vervolgtelefoontje te ontvangen om na te gaan welke kosten (indien van toepassing) zijn gemaakt en of er nadelige gevolgen zijn opgetreden.
