Friedreich's ataxie is een autosomaal recessieve cerebellaire ataxie die wordt veroorzaakt door triplet-repeat expansies. De oorzakelijke mutatie is een trinucleotide (GAA) herhalingsuitbreiding in het eerste intron van het frataxine-gen, wat leidt tot verminderde transcriptie van frataxine. De pathologische gevolgen van frataxinedeficiëntie zijn onder meer een ernstige verstoring van de biosynthese van ijzer-zwavelclusters, mitochondriale ijzerstapeling gekoppeld aan cellulaire ijzerontregeling en een verhoogde gevoeligheid voor oxidatieve stress.

Een kenmerk van de ataxie van Friedreich is een verslechtering van antioxidatieve verdedigingsmechanismen, die een belangrijke rol spelen bij de progressie van de ziekte. Studies hebben aangetoond dat de signalering van nucleaire factor erytroïde-afgeleide 2-gerelateerde factor 2 (Nrf2) ernstig verstoord is bij patiënten met de ataxie van Friedreich. Daarom wordt verondersteld dat het vermogen van omaveloxolon (RTA 408) om Nrf2 te activeren en antioxiderende doelgenen te induceren, therapeutisch is bij patiënten met de ataxie van Friedreich.

Deze 2-delige studie zal de werkzaamheid, veiligheid en farmacodynamiek van omaveloxolon (RTA 408) evalueren bij de behandeling van patiënten met de ataxie van Friedreich.

Deel 1: Het eerste deel van deze studie zal een gerandomiseerde, placebo-gecontroleerde, dubbelblinde, dosis-escalatie studie zijn om de veiligheid van omaveloxolon (RTA 408) in verschillende doses te evalueren bij patiënten met ataxie van Friedreich.

Deel 2: Het tweede deel van deze studie is een gerandomiseerde, placebogecontroleerde, dubbelblinde studie met parallelle groepen ter evaluatie van de veiligheid en werkzaamheid van omaveloxolon (RTA 408) 150 mg bij patiënten met ataxie van Friedreich. Patiënten die deelnamen aan Deel 2 zullen 1:1 gerandomiseerd worden om omaveloxolon (RTA 408) 150 mg of placebo te krijgen.

Uitbreiding: De verlenging beoordeelt de veiligheid en verdraagbaarheid op lange termijn van omaveloxolon (RTA 408) bij gekwalificeerde patiënten met ataxie van Friedreich na afronding van deel 1 of deel 2. Patiënten zullen niet worden ontblind om de behandeling in deel 1 of deel 2 te bestuderen bij binnenkomst in de uitbreiding studie. Patiënten krijgen eenmaal daags open-label omaveloxolon (RTA 408) van 150 mg.

Inclusiecriteria:

  1. Heb de ataxie van Friedreich genetisch bevestigd
  2. Heb een gewijzigde FARS-score ≥20 en ≤80
  3. Man of vrouw zijn en ≥16 jaar en ≤40 jaar oud zijn
  4. Geen wijzigingen in het trainingsregime hebben binnen 30 dagen voorafgaand aan studiedag 1 en bereid zijn om hetzelfde trainingsregime te blijven volgen tijdens de studieperiode van 16 weken
  5. De mogelijkheid hebben om maximale inspanningstests te voltooien
  6. Capsules kunnen slikken

Uitsluitingscriteria:

  1. Ongecontroleerde diabetes hebben (HbA1c >11,0%)
  2. Heb een B-type natriuretische peptidewaarde >200 pg/mL
  3. Een voorgeschiedenis hebben van klinisch significante linkszijdige hartziekte en/of klinisch significante hartziekte
  4. Een actieve schimmel-, bacteriële en/of virale infectie hebben gekend, waaronder humaan immunodeficiëntievirus of hepatitisvirus (B of C)
  5. Bekend of vermoed actief drugs- of alcoholmisbruik
  6. Klinisch significante afwijkingen van klinische hematologie of biochemie hebben, inclusief maar niet beperkt tot verhogingen van meer dan 1,5 keer de bovengrens van normaal van aspartaataminotransferase of alanineaminotransferase

The 9-HPT is a brief, standardized, quantitative test of upper extremity (arm and hand) function. Both the dominant and nondominant hands were tested twice (2 consecutive trials of the dominant hand, followed immediately by 2 consecutive trials of the nondominant hand). The participant picks up pegs 1 at a time (9 in total), using 1 hand only, and places them into holes on the board as quickly as possible, in any order until all holes are filled.
