RTA 408 tobolky u pacientů s Friedreichovou ataxií - MOXIe
Studie 2. fáze bezpečnosti, účinnosti a farmakodynamiky RTA 408 při léčbě Friedreichovy ataxie (MOXIe)
Friedreichova ataxie je autozomálně recesivní cerebelární ataxie způsobená tripletně opakovanými expanzemi. Příčinnou mutací je expanze repetice trinukleotidu (GAA) v prvním intronu genu pro frataxin, což vede k poruše transkripce frataxinu. Patologické důsledky deficitu frataxinu zahrnují vážné narušení biosyntézy klastrů železo-síra, mitochondriální přetížení železem spojené s buněčnou dysregulací železa a zvýšenou citlivost na oxidační stres.
Charakteristickým znakem Friedreichovy ataxie je narušení antioxidačních obranných mechanismů, které hrají hlavní roli v progresi onemocnění. Studie prokázaly, že signalizace faktoru 2 souvisejícího s erytroidním faktorem 2 (Nrf2) je u pacientů s Friedreichovou ataxií značně narušena. Proto se předpokládá, že schopnost omaveloxolonu (RTA 408) aktivovat Nrf2 a indukovat antioxidační cílové geny je terapeutická u pacientů s Friedreichovou ataxií.
Tato dvoudílná studie bude hodnotit účinnost, bezpečnost a farmakodynamiku omaveloxolonu (RTA 408) při léčbě pacientů s Friedreichovou ataxií.
Část 1: První část této studie bude randomizovaná, placebem kontrolovaná, dvojitě zaslepená studie s eskalací dávky, která má vyhodnotit bezpečnost omaveloxolonu (RTA 408) v různých dávkách u pacientů s Friedreichovou ataxií.
Část 2: Druhá část této studie je randomizovaná, placebem kontrolovaná, dvojitě zaslepená studie s paralelními skupinami k hodnocení bezpečnosti a účinnosti omaveloxolonu (RTA 408) 150 mg u pacientů s Friedreichovou ataxií. Pacienti zařazení do části 2 budou randomizováni v poměru 1:1, aby dostávali omaveloxolon (RTA 408) 150 mg nebo placebo.
Rozšíření: Rozšíření posoudí dlouhodobou bezpečnost a snášenlivost omaveloxolonu (RTA 408) u kvalifikovaných pacientů s Friedreichovou ataxií po dokončení části 1 nebo části 2. Pacienti nebudou při vstupu do studie zaslepeni v části 1 nebo části 2. nástavbové studium. Pacienti budou dostávat otevřený omaveloxolon (RTA 408) v dávce 150 mg jednou denně.

Popis
Kritéria pro zařazení:
- Geneticky potvrdil Friedreichovu ataxii
- Mít upravené skóre FARS ≥20 a ≤80
- Být muž nebo žena ve věku ≥16 let a ≤40 let
- Neměnit cvičební režim během 30 dnů před 1. dnem studie a být ochotni zůstat ve stejném cvičebním režimu během 16týdenního období studie
- Mít schopnost dokončit maximální zátěžové testování
- Umět polykat kapsle
Kritéria vyloučení:
- Máte nekontrolovaný diabetes (HbA1c > 11,0 %)
- Mají hodnotu natriuretického peptidu typu B > 200 pg/ml
- Mít v anamnéze klinicky významné levostranné srdeční onemocnění a/nebo klinicky významné srdeční onemocnění
- Máte známou aktivní plísňovou, bakteriální a/nebo virovou infekci, včetně viru lidské imunodeficience nebo viru hepatitidy (B nebo C)
- Je známo nebo máte podezření na zneužívání drog nebo alkoholu
- Mají klinicky významné abnormality klinické hematologie nebo biochemie, včetně, ale bez omezení na elevace větší než 1,5násobek horní hranice normálu aspartátaminotransferázy nebo alaninaminotransferázy
- mít jakékoli abnormální hodnoty laboratorního testu nebo závažný již existující zdravotní stav, který by podle názoru zkoušejícího mohl pacienta ohrozit zařazením do studie
Užil(a) jste některý z následujících léků během 7 dnů před 1. dnem studie nebo plánujete užívat některý z těchto léků v době účasti ve studii:
- Citlivé substráty pro cytochrom P450 2C8 nebo 3A4 (např. repaglinid, midazolam, sildenafil)
- Středně silné nebo silné inhibitory nebo induktory cytochromu P450 3A4 (např. karbamazepin, fenytoin, ciprofloxacin, grapefruitová šťáva)
- Substráty pro p-glykoproteinový transportér (např. ambrisentan, digoxin)
- Účastnili se jakékoli jiné intervenční klinické studie během 30 dnů před 1. dnem studie

Cycle ergometry using a recumbent stationary bicycle was used to conduct maximal exercise testing and workload was increased incrementally.
Peak work is defined as the workload at which participants reach maximal volition (defined as an inability to continue to exercise due to exhaustion).
A positive change from baseline suggests an improvement.

The CGIC scale is a 7 point scale that requires the clinician to assess how much the participant's illness has improved or worsened relative to a baseline state at the beginning of an intervention.
The CGIC was assessed by completing the following statement "Compared to the participant's condition at the start of the trial, this participant's overall status is", where 1=very much improved, 2=much improved, 3=minimally improved, 4=no change, 5=minimally worse, 6=much worse, 7=very much worse.
Lower score indicates better improvement.
The categories with at least one participant having a CGIC score are reported.

Part 2: Change From Baseline in Performance on a 9-Hole Peg Test (9-HPT) at Week 48

The 9-HPT is a brief, standardized, quantitative test of upper extremity (arm and hand) function.
Both the dominant and nondominant hands were tested twice (2 consecutive trials of the dominant hand, followed immediately by 2 consecutive trials of the nondominant hand).
The participant picks up pegs 1 at a time (9 in total), using 1 hand only, and places them into holes on the board as quickly as possible, in any order until all holes are filled.
Then, without pausing, the participant removes the pegs 1 at a time and returns them as quickly as possible.
The times recorded for the two trials for each hand are averaged, and the reciprocal average value for each hand was used in analyses.
Longer test times reflect more impairment on the participant's upper extremity function, thus a negative change from baseline suggests an improvement.

The T25-FWT is a quantitative mobility and leg function performance test based on time (in seconds) to complete a 25-foot walk.
Participants were instructed to attempt two T25-FWT trials at each visit.
Both walk times were averaged, and the reciprocal average value was used in analyses.
Longer test times reflect more impairment on the participant's ability to walk, thus a negative change from baseline suggests an improvement.

The ADL scale examines the ability of participants to complete everyday tasks (e.g., holding a fork, dressing).
The ADL is a 9-question assessment, with the total score being the sum of 9 questions.
The ADL survey assesses 9 concepts: (1) speech; (2) swallowing; (3) cutting food and handling utensils; (4) dressing; (5) personal hygiene; (6) falling; (7) walking; (8) quality of sitting position; and (9) bladder function.
Each of these is rated on a 5-point scale where 0= normal and 4= severe disability/inability to carry out activity independently, for a total possible score of 0 to 36, with higher scores representing greater disability/dependency.
A negative change from baseline indicates improvement.
