La ataxia de Friedreich es una ataxia cerebelosa autosómica recesiva causada por expansiones repetidas de tripletes. La mutación causante es una expansión repetida de trinucleótidos (GAA) en el primer intrón del gen de la frataxina, lo que conduce a una transcripción alterada de la frataxina. Las consecuencias patológicas de la deficiencia de frataxina incluyen una alteración grave de la biosíntesis de grupos de hierro y azufre, una sobrecarga de hierro mitocondrial junto con una desregulación del hierro celular y una mayor sensibilidad al estrés oxidativo.
Un sello distintivo de la ataxia de Friedreich es el deterioro de los mecanismos de defensa antioxidantes, que juegan un papel importante en la progresión de la enfermedad. Los estudios han demostrado que la señalización del factor 2 relacionado con el factor 2 derivado de eritroides (Nrf2) del factor nuclear está gravemente alterada en pacientes con ataxia de Friedreich. Por lo tanto, se supone que la capacidad de la omaveloxolona (RTA 408) para activar Nrf2 e inducir genes diana antioxidantes es terapéutica en pacientes con ataxia de Friedreich.
Este estudio de dos partes evaluará la eficacia, la seguridad y la farmacodinámica de la omaveloxolona (RTA 408) en el tratamiento de pacientes con ataxia de Friedreich.
Parte 1: La primera parte de este estudio será un estudio aleatorizado, controlado con placebo, doble ciego, de escalada de dosis para evaluar la seguridad de la omaveloxolona (RTA 408) en varias dosis en pacientes con ataxia de Friedreich.
Parte 2: La segunda parte de este estudio es un estudio aleatorizado, controlado con placebo, doble ciego, de grupos paralelos para evaluar la seguridad y eficacia de omaveloxolona (RTA 408) 150 mg en pacientes con ataxia de Friedreich. Los pacientes inscritos en la Parte 2 serán aleatorizados 1:1 para recibir 150 mg de omaveloxolona (RTA 408) o placebo.
Extensión: la extensión evaluará la seguridad y la tolerabilidad a largo plazo de la omaveloxolona (RTA 408) en pacientes calificados con ataxia de Friedreich luego de completar la Parte 1 o la Parte 2. No se dejará de cegar a los pacientes para estudiar el tratamiento en la Parte 1 o la Parte 2 al ingresar al estudio de extensión. Descripción
Criterios de inclusión:
- Haber confirmado genéticamente la ataxia de Friedreich
- Tener una puntuación FARS modificada ≥20 y ≤80
- Ser hombre o mujer y ≥16 años y ≤40 años
- No tener cambios en el régimen de ejercicios dentro de los 30 días anteriores al día 1 del estudio y estar dispuesto a permanecer en el mismo régimen de ejercicios durante el período de estudio de 16 semanas.
- Tener la capacidad de completar la prueba de esfuerzo máximo.
- Ser capaz de tragar cápsulas.
Criterio de exclusión:
- Tiene diabetes no controlada (HbA1c >11,0%)
- Tener un valor de péptido natriurético tipo B >200 pg/mL
- Tener antecedentes de enfermedad cardíaca del lado izquierdo clínicamente significativa y/o enfermedad cardíaca clínicamente significativa
- Tener una infección fúngica, bacteriana y/o viral activa conocida, incluido el virus de la inmunodeficiencia humana o el virus de la hepatitis (B o C)
- Tener abuso activo conocido o sospechado de drogas o alcohol
- Tener anormalidades clínicamente significativas de hematología clínica o bioquímica, incluidas, entre otras, elevaciones superiores a 1,5 veces el límite superior normal de aspartato aminotransferasa o alanina aminotransferasa
- Tener algún valor de prueba de laboratorio anormal o condición médica preexistente grave que, en opinión del investigador, pondría al paciente en riesgo para la inscripción en el estudio.
Ha tomado alguno de los siguientes medicamentos dentro de los 7 días anteriores al Día 1 del estudio o planea tomar alguno de estos medicamentos durante el tiempo de participación en el estudio:
- Sustratos sensibles al citocromo P450 2C8 o 3A4 (p. ej., repaglinida, midazolam, sildenafilo)
- Inhibidores o inductores moderados o fuertes del citocromo P450 3A4 (por ejemplo, carbamazepina, fenitoína, ciprofloxacina, jugo de toronja)

Cycle ergometry using a recumbent stationary bicycle was used to conduct maximal exercise testing and workload was increased incrementally.
Peak work is defined as the workload at which participants reach maximal volition (defined as an inability to continue to exercise due to exhaustion).
A positive change from baseline suggests an improvement.

The Friedreich Ataxia Rating Scale (FARS) is a neurological-exam-based rating scale with five sections: Bulbar (section A), Upper Limb Coordination (section B), Lower Limb Coordination (section C), Peripheral Nervous System (section D), and Upright Stability (section E). mFARS is the sum of sections A (score 0 to 11), B (score 0 to 36), C (score 0 to 16), and E (score 0 to 36).
The minimum score is 0 and the maximum score is 99.
A lower score indicates better neurological function.

The CGIC scale is a 7 point scale that requires the clinician to assess how much the participant's illness has improved or worsened relative to a baseline state at the beginning of an intervention.
The CGIC was assessed by completing the following statement "Compared to the participant's condition at the start of the trial, this participant's overall status is", where 1=very much improved, 2=much improved, 3=minimally improved, 4=no change, 5=minimally worse, 6=much worse, 7=very much worse.
Lower score indicates better improvement.
The categories with at least one participant having a CGIC score are reported.

Part 2: Change From Baseline in Performance on a 9-Hole Peg Test (9-HPT) at Week 48

The 9-HPT is a brief, standardized, quantitative test of upper extremity (arm and hand) function.
Both the dominant and nondominant hands were tested twice (2 consecutive trials of the dominant hand, followed immediately by 2 consecutive trials of the nondominant hand).
The participant picks up pegs 1 at a time (9 in total), using 1 hand only, and places them into holes on the board as quickly as possible, in any order until all holes are filled.
Then, without pausing, the participant removes the pegs 1 at a time and returns them as quickly as possible.
The times recorded for the two trials for each hand are averaged, and the reciprocal average value for each hand was used in analyses.
Longer test times reflect more impairment on the participant's upper extremity function, thus a negative change from baseline suggests an improvement.

The T25-FWT is a quantitative mobility and leg function performance test based on time (in seconds) to complete a 25-foot walk.
Participants were instructed to attempt two T25-FWT trials at each visit.

Longer test times reflect more impairment on the participant's ability to walk, thus a negative change from baseline suggests an improvement.

The ADL scale examines the ability of participants to complete everyday tasks (e.g., holding a fork, dressing).
The ADL is a 9-question assessment, with the total score being the sum of 9 questions.
The ADL survey assesses 9 concepts: (1) speech; (2) swallowing; (3) cutting food and handling utensils; (4) dressing; (5) personal hygiene; (6) falling; (7) walking; (8) quality of sitting position; and (9) bladder function.
Each of these is rated on a 5-point scale where 0= normal and 4= severe disability/inability to carry out activity independently, for a total possible score of 0 to 36, with higher scores representing greater disability/dependency.
A negative change from baseline indicates improvement.
