RTA 408 Kapseln bei Patienten mit Friedreich-Ataxie - MOXIe
Eine Phase-2-Studie zur Sicherheit, Wirksamkeit und Pharmakodynamik von RTA 408 bei der Behandlung von Friedreich-Ataxie (MOXIe)
Die Friedreich-Ataxie ist eine autosomal-rezessive zerebelläre Ataxie, die durch Triplett-Repeat-Expansion verursacht wird. Die ursächliche Mutation ist eine Trinukleotid (GAA)-Wiederholungsexpansion im ersten Intron des Frataxin-Gens, was zu einer beeinträchtigten Transkription von Frataxin führt. Zu den pathologischen Folgen eines Frataxin-Mangels gehören eine schwere Störung der Eisen-Schwefel-Cluster-Biosynthese, eine mitochondriale Eisenüberladung gekoppelt mit einer zellulären Eisen-Dysregulation und eine erhöhte Empfindlichkeit gegenüber oxidativem Stress.
Ein Kennzeichen der Friedreich-Ataxie ist die Beeinträchtigung der antioxidativen Abwehrmechanismen, die eine wichtige Rolle beim Fortschreiten der Krankheit spielen. Studien haben gezeigt, dass die vom Nuklearfaktor Erythroid abgeleitete 2-bezogene Faktor 2 (Nrf2)-Signalübertragung bei Patienten mit Friedreich-Ataxie stark beeinträchtigt ist. Daher wird angenommen, dass die Fähigkeit von Omaveloxolon (RTA 408), Nrf2 zu aktivieren und antioxidative Zielgene zu induzieren, bei Patienten mit Friedreich-Ataxie therapeutisch ist.
Diese zweiteilige Studie wird die Wirksamkeit, Sicherheit und Pharmakodynamik von Omaveloxolon (RTA 408) bei der Behandlung von Patienten mit Friedreich-Ataxie bewerten.
Teil 1: Der erste Teil dieser Studie wird eine randomisierte, placebokontrollierte, doppelblinde Dosiseskalationsstudie zur Bewertung der Sicherheit von Omaveloxolon (RTA 408) in verschiedenen Dosierungen bei Patienten mit Friedreich-Ataxie sein.
Teil 2: Der zweite Teil dieser Studie ist eine randomisierte, placebokontrollierte, doppelblinde Parallelgruppenstudie zur Bewertung der Sicherheit und Wirksamkeit von Omaveloxolon (RTA 408) 150 mg bei Patienten mit Friedreich-Ataxie. Patienten, die in Teil 2 aufgenommen werden, werden 1:1 randomisiert, um 150 mg Omaveloxolon (RTA 408) oder Placebo zu erhalten.
Verlängerung: Die Verlängerung wird die langfristige Sicherheit und Verträglichkeit von Omaveloxolon (RTA 408) bei qualifizierten Patienten mit Friedreich-Ataxie nach Abschluss von Teil 1 oder Teil 2 bewerten. Die Patienten werden bei Eintritt in die Studienbehandlung in Teil 1 oder Teil 2 nicht entblindet Erweiterungsstudium. Die Patienten erhalten Open-Label-Omaveloxolon (RTA 408) mit 150 mg einmal täglich.

Einschlusskriterien:
- Habe Friedreichs Ataxie genetisch bestätigt
- Haben Sie einen modifizierten FARS-Score ≥20 und ≤80
- Sie müssen männlich oder weiblich und ≥ 16 Jahre und ≤ 40 Jahre alt sein
- Nehmen Sie innerhalb von 30 Tagen vor Studientag 1 keine Änderungen am Trainingsprogramm vor und seien Sie bereit, während des 16-wöchigen Studienzeitraums dasselbe Trainingsprogramm beizubehalten
- Haben Sie die Fähigkeit, maximale Belastungstests durchzuführen
- Kapseln schlucken können
Ausschlusskriterien:
- Unkontrollierten Diabetes haben (HbA1c > 11,0 %)
- einen natriuretischen Peptidwert vom B-Typ > 200 pg/ml aufweisen
- Haben Sie eine Vorgeschichte von klinisch signifikanter linksseitiger Herzerkrankung und / oder klinisch signifikanter Herzerkrankung
- Haben Sie eine bekannte aktive Pilz-, Bakterien- und/oder Virusinfektion, einschließlich Human Immunodeficiency Virus oder Hepatitis-Virus (B oder C)
- Bekannten oder vermuteten aktiven Drogen- oder Alkoholmissbrauch
- Haben Sie klinisch signifikante Anomalien der klinischen Hämatologie oder Biochemie, einschließlich, aber nicht beschränkt auf Erhöhungen von mehr als dem 1,5-fachen der oberen Normgrenze von Aspartat-Aminotransferase oder Alanin-Aminotransferase
- Haben Sie einen anormalen Labortestwert oder eine schwerwiegende vorbestehende Erkrankung, die nach Ansicht des Prüfarztes den Patienten durch die Aufnahme in die Studie gefährden würde
innerhalb von 7 Tagen vor Studientag 1 eines der folgenden Medikamente eingenommen haben oder die Einnahme eines dieser Medikamente während der Zeit der Studienteilnahme planen:

Cycle ergometry using a recumbent stationary bicycle was used to conduct maximal exercise testing and workload was increased incrementally.
Peak work is defined as the workload at which participants reach maximal volition (defined as an inability to continue to exercise due to exhaustion).
A positive change from baseline suggests an improvement.

The minimum score is 0 and the maximum score is 99.
A lower score indicates better neurological function.

The CGIC scale is a 7 point scale that requires the clinician to assess how much the participant's illness has improved or worsened relative to a baseline state at the beginning of an intervention.
The CGIC was assessed by completing the following statement "Compared to the participant's condition at the start of the trial, this participant's overall status is", where 1=very much improved, 2=much improved, 3=minimally improved, 4=no change, 5=minimally worse, 6=much worse, 7=very much worse.
Lower score indicates better improvement.

Part 2: Change From Baseline in Performance on a 9-Hole Peg Test (9-HPT) at Week 48

The 9-HPT is a brief, standardized, quantitative test of upper extremity (arm and hand) function.
Both the dominant and nondominant hands were tested twice (2 consecutive trials of the dominant hand, followed immediately by 2 consecutive trials of the nondominant hand).
The participant picks up pegs 1 at a time (9 in total), using 1 hand only, and places them into holes on the board as quickly as possible, in any order until all holes are filled.
Then, without pausing, the participant removes the pegs 1 at a time and returns them as quickly as possible.
The times recorded for the two trials for each hand are averaged, and the reciprocal average value for each hand was used in analyses.
Longer test times reflect more impairment on the participant's upper extremity function, thus a negative change from baseline suggests an improvement.

The T25-FWT is a quantitative mobility and leg function performance test based on time (in seconds) to complete a 25-foot walk.
Participants were instructed to attempt two T25-FWT trials at each visit.
Both walk times were averaged, and the reciprocal average value was used in analyses.
Longer test times reflect more impairment on the participant's ability to walk, thus a negative change from baseline suggests an improvement.

The ADL scale examines the ability of participants to complete everyday tasks (e.g., holding a fork, dressing).
The ADL is a 9-question assessment, with the total score being the sum of 9 questions.
The ADL survey assesses 9 concepts: (1) speech; (2) swallowing; (3) cutting food and handling utensils; (4) dressing; (5) personal hygiene; (6) falling; (7) walking; (8) quality of sitting position; and (9) bladder function.
Each of these is rated on a 5-point scale where 0= normal and 4= severe disability/inability to carry out activity independently, for a total possible score of 0 to 36, with higher scores representing greater disability/dependency.
A negative change from baseline indicates improvement.
